Is there a relationship between the extent of tonsillar ectopia and the severity of the clinical Chiari syndrome?

BACKGROUND: Chiari 1 malformation is diagnosed if the cerebellar tonsils extend at least 5 mm below the opisthion-basion line. OBJECTIVE: To examine the correlation of the extent of tonsillar ectopia with the prevalence and severity of the symptoms associated with the Chiari malformation. METHODS: Patients (N = 428) were grouped according to the extent of tonsillar ectopia on the mid-sagittal MRI image (group 1, 0-< 3 mm; group 2, 3-5 mm; group 3, > 5 mm). Groups were compared regarding demographics, symptoms, neurological signs, pain score, and response to HADS and sf-36 questionnaires. Res

Reducing post-tonsillectomy haemorrhage rates through a quality improvement project using a Swedish National quality register: a case study.

PURPOSE: Tonsillectomy (TE) is one of the most frequently performed ENT surgical procedures. Post-tonsillectomy haemorrhage (PTH) is a potentially life-threatening complication of TE. The National Tonsil Surgery Register in Sweden (NTSRS) has revealed wide variations in PTH rates among Swedish ENT centres. In 2013, the steering committee of the NTSRS, therefore, initiated a quality improvement project (QIP) to decrease the PTH incidence. The aim of the present study was to describe and evaluate the multicentre QIP initiated to decrease PTH rates. METHODS: Six ENT centres, all with PTH rates a

[Effect of resection of adenoids and tonsillectomy on immune function in children with obstructive sleep apnea hypopnea syndrome].

Objective:To remove the immune function of children with obstructive sleep apnea hypopnea syndrome by resection of adenoids and tonsillar. Method: The fasting peripheral venous blood of children with OSAHS and healthy children were collected. The lymphocyte subsets, NK cells and immunoglobulin levels were compared between the two groups. The fasting blood samples of OSAHS children were collected again after 2d, 30d, 60d and 90d after operation. The changes of lymphocyte subsets, NK cells and immunoglobulin levels were observed after operation. Result: Before operation, at the 0.05 test level,

Tonsillar Plasmacytoma: clues on magnetic resonance imaging.

BACKGROUND: Malignant plasma cell proliferation may present as a disseminated disease (multiple myeloma), a solitary plasmacytoma of bone, or an extramedullary plasmacytoma of soft tissue. The latter plasmacytomas represent approximately 3% of all plasma cell proliferations, and 80% develop in the head-and-neck region. The unexpected clinical presentation of such masses may be present. CASE PRESENTATION: Here, we report a rare case of primary tonsillar plasmacytoma in a 42-year-old female. The patient presented with asymmetric tonsillar hypertrophy that was resistant to antibiotherapy. Upon f

Isolated Unilateral Soft Palate Palsy Following Tonsillopharyngitis Caused by Epstein-Barr Virus Infection.

Soft palate palsy with or without pharyngeal or laryngeal paralysis or facial palsy has been described in the literature fewer than 40 times. Of these cases, isolated unilateral soft palate palsy (occurring without any other neurologic manifestation) has been rare. Most cases of isolated soft palate palsy were termed idiopathic in the past. In a few cases, a concomitant viral infection such as varicella zoster, herpes simplex, measles, and coxsackie A9 was diagnosed. We describe a case of isolated soft palate palsy, following a pharyngotonsillitis caused by Epstein-Barr virus. This viral infec…

Radioautographic study of palatine tonsillar cells.

Proliferative and functional (protein producing) activities of palatine tonsillar cells were studied by light radioautography (semithin sections). 3H-Thymidine incorporation demonstrated the dynamics of renewal of palatine tonsillar cell populations in various pathological states. 3H-Uridine labeling objectively evaluated functional activity of different cells and more accurately showed distribution of destructive processes in the organ.

Pseudo bilateral tonsilloliths: a case report and review of the literature.

Tonsilloliths are very rare concretions found in the tonsillar crypt. They are usually single and unilateral, but occasionally may be multiple or bilateral. Small concretions in the tonsils are common, but well formed giant unilateral or bilateral tonsilloliths are extremely uncommon. Only two cases of bilateral tonsilloliths have so far been reported in the literature. A case of unilateral tonsillolith, mimicking bilateral tonsilloliths taken with the orthopantogram (OPT) in a 57-year-old Malaysian Indian female with squamous cell carcinoma of the oral cavity is described. Although the OPT is

Tonsillectomy combined with steroid pulse therapy: a curative therapy for IgA nephropathy.

As IgA nephropathy (IgAN) was originally regarded as a benign condition, the indication of corticosteroids has been highly restricted because of their side effects, and has been used for a specific subgroup of IgAN patients with consideration of the risk/benefit ratio. During the last decade, however, with the recognition that the overall long-term prognosis of IgAN is a non-benign condition, more aggressive treatments, including high dose corticosteroids and tonsillectomy, have been used for a wider subgroup of patients with IgAN. Since 1988, we have treated IgAN patients with steroid semi-pu

Postoperative tonsillectomy pain in pediatric patients: electrocautery (hot) vs cold dissection and snare tonsillectomy--a randomized trial.

OBJECTIVE: To determine the effect of the method of tonsillectomy on postoperative pain in pediatric patients. DESIGN: Prospective, randomized, single-blind, controlled clinical trial. SETTING: A university pediatric hospital in Aberdeen, Scotland. PATIENTS: A volunteer sample of 54 children, aged 3 to 12 years, with recurrent tonsillitis or symptomatic adenotonsillar hypertrophy. Two patients withdrew consent. INTERVENTIONS: Twenty-six children underwent a nonelectrical (ie, cold) dissection tonsillectomy with cold steel instruments, 5 of whom also had adenoidectomy by curettage. Monopola…

Granuloma development in cattle after intratonsilar inoculation with Mycobacterium bovis.

OBJECTIVE: To examine the temporal development of tuberculous lesions in cattle inoculated with Mycobacterium bovis. ANIMALS: 15 mature crossbred cows obtained from a herd with no history of M bovis infection. PROCEDURE: Inoculation of cattle was done by intratonsilar instillation of 1.48 X 10(5) to 5.4 X 10(7) colony-forming units of M bovis strain 2045T. At 3 to 4 hours, 4 weeks, 6 weeks, and 8 weeks after inoculation, tissues were examined for gross and microscopic lesions and processed for isolation of M bovis. RESULTS: Retropharyngeal lymph nodes from cattle examined 4 weeks after inoc
