Neuropeptide/Receptor expression and plasticity in micturition pathways.

Several motor behaviors such as locomotion, respiration, sexual function, and micturition are generated by rhythmic and stereotyped motor patterns of activity. In most cases, these functions are primarily controlled by signals and neuronal commands that originate from the brainstem and spinal cord. Defined as the storage and periodic elimination of urine, micturition requires a complex neural control system that coordinates the activities of a variety of effector organs including the smooth muscle of the urinary bladder and the smooth and striated muscle of the urethral sphincters. The lower u…

A new method using a bacterium for dissolution of urinary stones.

PURPOSE: A bacterium, Thiobacillus thiooxidans, oxidizes inorganic sulfur compounds to sulfuric acid to obtain energy for growth. We examined dissolution of urinary stones by the bacteria and studied optimal conditions for the dissolution in human urine. METHODS: Thiobacillus thiooxidans (IFO No. 13 701) was purchased from Hakko Kenkyujyo, Co., Osaka, Japan. The bacteria were grown stationary for 7 days at 30 degrees C in medium. The pH of the medium was changed from pH 4.82 to 1.95 for 28 days. Growth of the bacteria was also examined in human urine by addition of different amounts of sodium

When Did They Start? Age of Onset of Toileting Behaviors and Urinary Cues as Reported by Older Women.

BACKGROUND: Lower urinary tract symptoms (LUTS) are prevalent across women's life course. Evidence shows toileting behaviors (TBs) and urinary urge cues are related to LUTS. It is unknown when women start using these behaviors and responding to urinary cues. METHODS: An online survey was administered to 338 women, 65 years of age and older, to assess the age of onset for 20 TBs from the Toileting Behaviors-Women's Elimination Behavior (TB-WEB) questionnaire, 10 urinary urge cues from the Urinary Cues Questionnaire, and urinary urgency and leakage items from the International Consultation on I

Laparoscopic radical prostatectomy: Washington University initial experience and prospective evaluation of quality of life.

BACKGROUND AND PURPOSE: The laparoscopic approach to radical prostatectomy offers an alternative to the open surgical procedure with less morbidity. We prospectively collected data including a validated quality-of-life questionnaires on our first 38 laparoscopic radical prostatectomies (LRPs). The first 10 patients (group 1), second 10 patients (group II), and the most recent 18 patients (group III) were examined separately to study the learning curve for this procedure. In addition, we determined the pattern of recovery of urinary continence, potency, and quality of life. PATIENTS AND METHOD

Evaluation of urinary and faecal continent stomas.

OBJECTIVES: To evaluate the outcome and complications of urinary and faecal continent catheterizable stomas. PATIENTS AND METHODS: Retrospective record review of all patients operated on in our institution within the last 6 years. RESULTS: Fifty-five children received 63 stomas. The mean age at operation was 9.2 years and the mean follow-up period was 2.8 years. The diagnoses were: neurogenic bladder (n=22), bladder or cloacal exstrophy (n=20), anorectal malformation (n=6), persistent cloaca (n=3), Hirschsprung's disease (n=2), posterior urethral valves (n=1) and prune belly syndrome (n=1).

[Suprapubic catheter insertion].

The suprapubic catheter enables a percutaneous drainage of urine. The insertion is made superior of the pubic bone through the abdominal wall into the bladder. It allows a permanent drainage of urine bypassing the urethra. The insertion of a suprapubic catheter requires knowledge and expertise. This paper summarizes the basic background and allows to follow the practical application step by step.

A novel cannulation technique for difficult urethral catheterization.

INTRODUCTION: To propose a novel cannulation technique for difficult urethral catheterization procedures. TECHNIQUE: The sheath tip of an intravenous catheter is cut off, replaced to the needle tip and pushed through the distal drainage side hole to Foley catheter tip, and finally withdrawn for cannulation. In situations making urethral catheterization difficult, a guide wire is placed under direct vision. The modified Foley catheter is slid successfully over the guide wire from its distal end throughout the urethral passage into the bladder. RESULTS: The modified Foley catheter was used suc

Catheter policies for management of long term voiding problems in adults with neurogenic bladder disorders.

BACKGROUND: Management of the neurogenic bladder has the primary objectives of maintaining continence, ensuring low bladder pressure (to avoid renal damage) and avoiding or minimising infection. Options include intermittent urethral catheterisation, indwelling urethral or suprapubic catheterisation, timed voiding, use of external catheter (for men), drug treatment, augmentation cystoplasty and urinary diversion. OBJECTIVES: The primary objective was to determine the effects of different methods of managing long-term voiding problems (persisting after three months) with catheters in patients w

[Evaluation of the irrigation and drainage capacity of 3 bladder irrigation catheters].

OBJECTIVES: To perform a comparative evaluation of three types of continuous bladder irrigation catheters with the aim of determining which of them allows greater irrigation solution inflow and bladder outflow. METHODS: We compared three types of three-way catheters, 22F in caliber, being the material the main difference between them: latex, silicone, or polyvinyl. RESULTS: The polyvinyl catheter showed significant differences both in inflow and outflow in comparison with the other two types of catheters (p = 0.000, ANOVA test with Sheffe's post hoc). Additionally, the latex catheter showed

Detrusor instability.

Detrusor instability is a common cause of urgency and urge incontinence in neurologically normal patients. It is defined as an involuntary phasic detrusor contraction of any pressure associated with symptoms of urge or leakage while the patient is attempting to inhibit micturition. The etiology is unknown but it has been associated with congenital causes, aging, stress incontinence, and bladder outlet obstruction. Diagnosis relies on a urologic history and physical examination, a voiding diary, and a urodynamic evaluation. Treatment is primarily pharmacologic and behavioral, with surgical opti
