OBJECTIVE: To analyse the clinical and therapeutic consequences of early treatment failure after bacille Calmette-Guérin (BCG) instillation therapy for carcinoma in situ of the bladder.
PATIENTS AND METHODS: A total of 115 patients with carcinoma in situ (Tis) of the bladder were treated by intravesical instillation of living BCG vaccine (Immun BCG Pasteur F). Twenty five patients had primary Tis and 90 had secondary Tis with synchronous or prior superficial papillary tumours. All papillary tumours were resected before instillation of BCG. We have reported the usefulness of percutaneous or transurethral whole layer core biopsy (WLCB) of bladder wall for staging of invasive bladder cancer. We have applied WLCB to 3 cases of paravesical tumor with good results. The first case was in a 3-year-old boy suspected of retrovesical sarcoma. Eleven cases of primary adenocarcinoma of the bladder are described herein. Eight had a glandular pattern, 1 was papillary and 2 were comprised of signet ring cells (one had areas of colloid carcinoma). A phase 1 study of intravesical mitoxantrone was done in patients with superficial bladder tumors recurrent after previous intravesical therapy. Mitoxantrone (5 to 10.5 mg.) was instilled in the bladder via catheter and was left in situ for 2 hours. Each patient received 6 treatments at 1-week intervals. We present a preliminary report on intravesical BCG instillation in four patients. The indications are prevention of new superficial tumors and treatment of carsinoma in situ. 120 mg Connaught BCG-vaccine in 50 cc saline is instillated weekly for a total of six treatments. Follow-up was performed every three months and included cystoscopy and urine cytology. The prophylactic effect is encouraging, even in patients who have previously not responded to intravesical chemotherapy. The electrocoagulation and resection of superficial tumors of the bladder leads to iatrogenic refluxes. The frequency of subsequent vesical recurrences may thus favor tumoral grafts in the upper urinary tract. Fives cases illustrate this risk. They were treated by ureteral resections and by one bilateral, and two unilateral nephroureterectomies. Transurethral electroresection for malignant involvement of the ureteral orifice was carried in 17 cases. In the light of published data the results may be regarded as favourable. It is stated that in case of operability resection of the orifice represents a radical procedure, superior to open techniques, and it has no adverse consequences in the majority of cases.

The aim of this study was to survey the expression of an embryonic cytokine gene, MK, in the normal organs and neoplastic tissues of adults. Antibody-drug conjugates (ADCs) have revolutionized the treatment landscape for metastatic urothelial carcinoma. Encouraging clinical activity of HER2-directed ADCs, including disitamab vedotin, has been observed in early-phase trials. Recent promising results from the RC48-C016 trial raise important questions. We discuss issues that include optimal frontline treatment selection, subsequent-line treatment sequencing, and the generalizability of the findings.

OBJECTIVES: Urothelial carcinoma in situ (CIS) with early papillary formation is terminology sometimes used to suggest incipient high-grade papillary urothelial carcinoma (PUC) but may lead to confusion between true CIS and lateral flat spread of PUC.
METHODS: It remains unclear how pathologists and urologists interpret this scenario, so a survey was circulated to 68 pathologists (group 1 = 28 academic genitourinary pathologists; group 2 = 17 pathologists with a self-reported genitourinary focus; group 3 = 23 pathologists self-reported as not genitourinary specialists) and 32 urologists.

INTRODUCTION: The treatment landscape of metastatic urothelial carcinoma (mUC) has evolved with the emergence of programmed cell death protein 1/ligand 1 (PD-1/L1) inhibitors. This study assessed mUC treatment patterns in Europe.

BACKGROUND: 17 500 persons receive a new diagnosis of urothelial carcinoma of the bladder in Germany each year. Radical cystectomy is performed for muscle-invasive and for non-muscle-invasive, recurrent, high-risk tumors. Because this procedure carries a perioperative complication rate of 30-40% and impairs the patients' quality of life, options have been developed for intravesical and systemic bladder-preserving treatment.
