Clinical ophthalmologists consider each retinal disease as a completely unique entity. However, various retinal diseases, such as uveitis, age-related macular degeneration, diabetic retinopathy, and primary open-angle glaucoma, share a number of common pathogenetic pathways. Whether a retinal disease initiates from direct injury to the blood-retinal barrier (BRB) or a defect/injury to retinal neurons or glia that impairs the BRB secondarily, the BRB is a pivotal point in determining the prognosis as self-limiting and recovering, or developing and progressing to a clinical phenotype. The presen…

Frequently, when imaging retinal vasculature with optical coherence tomography angiography (OCTA) in diseased eyes, there are unavoidable ob … identify shadowed areas in OCTA of healthy subjects as well as patients with diabetic retinopathy, uveitis and age-related macular degeneration. The aim is to exclude these areas from analysis so that the overall OCTA parameters are minimally affected by shadow artifacts.

Rheumatoid arthritis, ankylosing spondylitis and psoriatic arthritis are associated with potentially sight-threatening inflammatory eye disease. Although the ocular manifestations associated with ankylosing spondylitis and psoriatic arthritis are similar, such as anterior uveitis, this differs from rheumatoid arthritis where dry eye, peripheral ulcerative keratitis and scleritis are the major ocular complications. Apart from causing sight loss, these conditions are painful, debilitating, often recurrent or chronic and may require long-term therapy. Treatments such as ocular lubricant, topical …

Chikungunya fever, a viral disease epidemic in some parts of the world is newly introduced in the Americas. This is of considerable international concern, with a growing incidence owing to developing urbanization, tourism, and trade. Ocular manifestations of chikungunya fever are not frequent, but of great relevance. Common manifestations include conjunctivitis, optic neuritis, iridocyclitis, episcleritis, retinitis and uveitis. Diagnostic and monitoring investigations would include optical coherence tomography, fundus fluorescein and indocyanine green angiography, visual field analysis, and e…

Ocular toxoplasmosis is the most common cause of posterior uveitis in the United States and worldwide. It commonly follows a relapsing course with the potential for multiple adverse visual sequelae and, rarely, blindness. Both immune-competent and immune-suppressed populations can develop severe relapsing disease. There is considerable debate regarding the initial treatment of ocular toxoplasmosis in both groups, with multiple antiparasitic agents commonly used with little evidence to support any particular regimen. The precise role of corticosteroids in treatment also has yet to be rigorously…

Renal-ocular syndrome is a rare cause of tubulointerstitial renal diseases. A case of 52 years old woman with acute renal injury in the course of acute tubulointerstitial nephritis and uveitis (TINU syndrome) is presented. For diagnosis of kidney disease a kidney biopsy and renal tubular function tests were used. Progression of the renal disease was the cause of the general intensive steroid treatment inclusion. In a short time after treatment initiation, a significant improvement of renal function was observed. Until the time of handing over the article (5 months), no relapse of the disease w…

A 33-year-old woman complained of unilateral eyelid edema and blurred vision. Initial ophthalmic examination disclosed anterior chamber reaction with keratic precipitates on the cornea, without posterior abnormalities. Anterior uveitis was treated. Despite that, patient showed rapidly progressive unilateral vision loss with optic nerve swelling. Systemic workup was inconclusive, as well as cranial magnetic resonance imaging and cerebrospinal fluid examination. Based on the hypothesis of optic neuritis, intravenous methylprednisolone pulse was performed with no success. During the following day…

HTLV-1 is one of the two pathogenic human retroviruses that causes adult T-cell leukemia(ATL), HTLV-1-associated myelopathy(HAM) and HTLV-1 uveitis (HU). Treatment of ATL is still very difficult today, thus search for new agents that target specific molecules are under way. For treatment of HAM and HU, suppression of HTLV-1 replicationappears to be important, as well as prevention of ATL development. Considering the common characteristics as human retroviruses, application of anti-HIV drugs appears to be a reasonable strategy, although it is not yet clinically tested for HTLV-1. Research of ag…

PURPOSE: To report a case of presumed ocular tuberculosis where a fluorine-18 fluorodeoxyglucose (FDG) PET/CT scan was used to image the chest.
DESIGN: Single case report.
METHODS: A 35-year-old female patient with recurrent posterior uveitis was examined and investigated.
RESULTS: A complete blood study was normal. A tuberculosis quantiferon gold test was positive (5.25 IU). CT scan studies of the chest were normal. She was advised to undergo a PET/CT scan. Increased uptake of tracer was seen in right paratracheal, precarinal, and bilateral hilar nodes and in the left choroid.

Three foals aged between 21 and 44 days were presented for evaluation of pyrexia and lameness. Each had subcutaneous abscessation associated with osteomyelitis of a rib at the costochondral junction and pathological fracture of the affected rib; one foal had more than one rib affected. Other localised sites of infection included other sites of osteomyelitis, omphalophlebitis, uveitis, enterocolitis, and calcaneal bursitis. The subcutaneous abscessation and rib osteomyelitis was treated surgically in two foals. Salmonella typhimurium was isolated from the subcutaneous abscess in one foal and fr…

Behçet's disease is a multisystem inflammatory disorder, characterized by recurrent oral and genital ulceration and uveitis. Additionally, skin lesions, vasculitis, and arthritis may occur. The disease is chronic, with exacerbations and remissions. Treatment of Behcet's disease is symptomatic depending on the symptoms and their severity. Oral ulcers are seen in 98% of patients with Behçet's disease. Some aspects of oral health, such as the amount of plaque and the presence of periodontal disease, and possibly also caries, are more prevalent in patients with the disease when compared to healthy…

Behçet's syndrome (BS) is a multisystem disease of unknown etiology, characterized by chronic relapsing cardinal symptoms of orogenital ulcers, uveitis, and different skin lesions. Its major pathologic feature is vasculitis. Neuro-Behçet's syndrome (NBS) is defined as a constellation of neurologic manifestations with characteristic neuropathologic findings, usually confirmed by ancillary investigations, in patients who meet the diagnostic criteria for BS. Neurologic manifestations of the syndrome are more common in male patients and have been reported to occur in anywhere from 5% to 50% of BS …

Vogt-Koyanagi-Harada (VKH) syndrome or uveomeningitic syndrome is a disease affecting several organs: eye (bilateral uveitis, exudative retinal detachments), ear (tinnitus, dysacousia), skin and hair (vitiligo, alopecia, poliosis) and the nervous system (meningism, headache, pleocytosis in cerebrospinal fluid). The etiology remains unknown but it is probably a cell-mediated autoimmune disorder in individuals genetically susceptible to antigenic components of melanocytes. We report a 25 year old patient with VKH syndrome treated with intravenous steroid therapy and cycles of intravenous immunog…

Ocular involvement of sarcoidosis is frequent, and it is often the initial clinical manifestation of the disease. The most common ocular lesions include granulomatous uveitis associated with iris and trabecular nodules, string of pearl-type vitreous opacities, retinal paerivasculitis mainly affecting veins, and patchy retino-choroidal exudates. A half of the patients with typical ocular lesions suggestive of sarcoidosis did not show the systemic evidence, and they remained as sarcoidosis suspects. Risks of visual deterioration are secondary glaucoma, vitreous opacities, cystoid macular edema, …

Behçet disease in an inflammatory systemic disease, of unknown etiology. Usually it is defined by a triad; buccal ulcerations, genital ulcerations and uveitis. Treatment is based on steroids and in difficult cases cyclosporina or cytotoxic agents could be used. We present a case of Behçet disease in a young male. The first symptoms were retrobulbar optic neuropathy, followed by buccal and genital ulcerations. For beginning the treatment was based on steroids, but the disease was more and more severe, so that the left eye lost its function because of an inflammatory maculopathy and new symptoms…

Posterior scleritis is a rare disease, the clinical signs of which may vary. There is no consensus on the appropriate method of treatment for this disease. Some have advocated treatment with nonsteroidal antiinflammatory drugs, whereas others have reported variable success with potentially more toxic therapy. Patients with posterior scleritis constitute approximately 1% of the uveitis clinic population at the Casey Eye Institute in Portland, Oregon. The cases of 6 patients with posterior scleritis, the diagnosis of which was elusive, are reported. Posterior scleritis was generally confirmed by…

Eyes at the risk of developing severe PVR postoperatively are limited to those affected with retinal detachment associated with horseshoe tears. This study is conducted to determine the relationship of horseshoe tears and PVR. 101 failure cases following surgery of retinal detachment with simple horseshoe tears were reviewed. The PVR group was compared with the group without PVR. The results indicate that vitreous gel liquefaction and posterior vitreous detachment are essential reasons of retinal detachment with horseshoe tears. Vitreous hemorrhage, uveitis, choroidal detachment and multiple o…
