Artificial Intelligence and Bioengineering Approaches for Antimicrobial Resistance Prediction.

Background and Objectives: Antimicrobial resistance (AMR) is a major challenge, particularly in intensive care units, where broad-spectrum therapy is often initiated before microbiological confirmation. Artificial intelligence (AI) may improve AMR prediction, but its clinical value depends on integration with bioengineering-enabled digital microbiology. This narrative review examines how AI, bioengineering platforms and digital microbiology can support AMR prediction, clinical decision support and antimicrobial stewardship across the sample-to-decision pipeline. Materials and Methods: A target

Antibiotic resistome in the glacier forelands of polar regions.

UNLABELLED: Antibiotic resistance genes (ARGs) pose a significant threat, exacerbated by climate change impacts on polar regions, particularly melting glaciers and permafrost. While ancient antibiotic resistance exists in the environments, the release and dissemination of ARGs remain poorly understood. This study investigated ARG composition and distribution in 43 metagenomes from Arctic and Antarctic glacier forelands. We identified 154 ARGs, predominantly bacitracin resistance, followed by rifamycin, fosfomycin, vancomycin, tetracycline, and beta-lactam resistance genes. Significant correlat

Silent pandemic in intensive care units: Post-pandemic rise of extensively drugresistant Acinetobacter baumannii and Klebsiella pneumoniae in ventilator-associated pneumonia.

INTRODUCTION: In the post coronavirus disease-2019 era, as the pandemic's impact diminishes, the state of our intensive care units (ICUs) remains as crucial as the well-being of individuals. While numerous studies have explored the pandemic's effects on patients, our focus is to examine its impact on ICUs. MATERIALS AND METHODS: A total of 72 patients who were admitted to the chest diseases ICU due to hypercapnic or hypoxic respiratory failure between October 2018-April 2020 and December 2022-December 2024 and who developed ventilator-associated pneumonia during their follow-up were included

Piperacillin/tazobactam clearance predicted by non-creatinine based estimates of GFR in critically ill adults.

BACKGROUND AND OBJECTIVE: As a renally-eliminated beta-lactam antibiotic, estimated glomerular filtration rate (eGFR) is a central component of piperacillin/tazobactam pharmacokinetics. This study aimed to determine the optimal eGFR equation for inclusion in population pharmacokinetic models for piperacillin and tazobactam among critically ill adults. METHODS: The study included critically ill adults treated with piperacillin/tazobactam at a single academic medical center between 2018 and 2022. Excluded individuals had acute kidney injury, received kidney replacement therapy, or had extracorp

Risk Factors for ESBL-Producing Enterobacteriaceae in Wastewater of Dairy Farms in East Java, Indonesia.

The proliferation of Enterobacteriaceae with extended-spectrum beta-lactamase (ESBL), including Escherichia coli and Klebsiella pneumoniae, poses a significant threat to public health due to their resistance to β-lactam antibiotics. This study focuses on the risk factors of ESBL-producing Enterobacteriaceae in the wastewater of dairy farms in East Java, Indonesia, a major milk-producing region. Utilizing an observational cross-sectional design, data were collected from 342 wastewater samples across six regions with high dairy cattle populations in East Java. Risk factors were assessed through

Changing Bacterial Profiles and Antibiotic Susceptibility Patterns in Spontaneous Ascites Infection: A Shift in Empiric Combination Antibiotic Therapy?

Background/Objectives: This study aimed to determine bacteria that grow in spontaneous ascites infection and their antibiotic susceptibility. Materials and Methods: A total of 291 cases of cirrhosis-related ascites were retrospectively analyzed between January 2007 and September 2015. Simple ascites, spontaneous ascites infection, spontaneous bacterial peritonitis, and culture results were recorded. Due to increasing antibiotic resistance, theoretically appropriate antibiotics were paired, and their susceptibility was evaluated in order to review our empirical antibiotic choices and evaluate n

Pediatric Staphylococcal Scalded Skin Syndrome: A Systematic Review of the Literature to Inform Work-Up and Management.

BACKGROUND: Significant gaps exist in the literature regarding the work-up and management of staphylococcal scalded skin syndrome (SSSS). These include (a) the utility of ancillary testing in improving diagnostic accuracy and informing patient care, and (b) appropriate management strategies for antibiotics, fluids, and skin care. Moreover, no systematic review to date has focused on non-neonatal pediatric SSSS to provide a comprehensive analysis of the literature. METHODS: A systematic review of PubMed and OVID was conducted from 2010 to 2024 using the medical subject heading (MeSH) term 'sta

A case of penicillin-resistant group B Streptococcus isolated from a patient in the UK.

OBJECTIVES: In England, group B streptococci (GBS; Streptococcus agalactiae) are considered universally susceptible to penicillin. Reports from Africa, Asia, North America and a few European countries have described GBS isolates with penicillin MICs above the epidemiological cut-off (0.125 mg/L). Our aim was to characterize a penicillin-resistant GBS (PRGBS) isolate recovered in 2016 from a patient treated with long-term antimicrobials in the UK. METHODS: Antibiotic susceptibility of a referred isolate from a discharging sinus overlying a chronic prosthetic joint infection was determined usin

Agar-Dilution Is Comparable to Broth Dilution for MIC Determination in Streptococcus agalactiae.

BACKGROUND: Streptococcus agalactiae (Group B Streptococcus, GBS) is a leading cause of neonatal sepsis in high-income countries. While intrapartum antibiotic screening reduces this risk, increasing resistance to macrolides and lincosamides in Europe since the 1990s has limited therapeutic options for penicillin-allergic patients. Reports of reduced beta-lactam susceptibility in GBS further emphasise the need for robust antimicrobial resistance (AMR) surveillance. However, broth microdilution (BMD) methods are unsuitable for large-scale antimicrobial susceptibility testing (AST). OBJECTIVE: T

Genomic analysis of Salmonella enterica from cattle, beef and humans in the Greater Tamale Metropolis of Ghana.

Salmonella enterica is a bacterial foodborne pathogen that can infect humans and animals. Proper control of Salmonella requires routine surveillance and interventions across the food-production chain. However, due to limited resources the epidemiology and transmission of non-typhoidal Salmonella serotypes remain poorly understood in several African settings, including within Ghana. Here, we employed bacterial culture and whole genome sequencing (WGS) to investigate the prevalence, virulence and antimicrobial resistance determinants of Salmonella enterica isolates from beef, cattle blood and hu

Genetic Characteristics of Extended-Spectrum Beta-Lactamase-Producing Salmonella Isolated from Retail Meats in South Korea.

Earlier studies have validated the isolation of extended-spectrum beta-lactamase-producing Salmonella (ESBL-Sal) strains from food. While poultry is recognized as a reservoir for Salmonella contamination, pertinent data regarding ESBL-Sal remains limited. Consequently, the Ministry of Food and Drug Safety has isolated Salmonella spp. from retail meat and evaluated their antibiotic susceptibility and genetic characteristics via whole-genome sequencing. To further elucidate these aspects, this study investigates the prevalence, antibiotic resistance profiles, genomic characteristics, and homolog

Inappropriate use of antibiotic enhances antibiotic resistance dissemination in ESBL-EC: Role of ydcz in outer membrane vesicles biogenesis and protein transport.

Extended-spectrumβ-lactam producing Escherichia coli (ESBL-EC) readily colonizes live poultry and serves as a major source of contamination in retail chicken meat, posing significant threats to public health. This study aims to investigate the impact of inappropriate antibiotic use on the dissemination and exacerbation of antibiotic resistance in ESBL-EC and explore the underlying molecular mechanisms. Through experimental analysis, we propose a hypothesis that inappropriate antibiotic use may exacerbate resistance by affecting vesicle formation and protein secretion. Experimental results demo

Promoting penicillin allergy de-labeling for children attending a hematology clinic.

BACKGROUND: Up to 10% of children are reported to be allergic to penicillin, but many allergy labels are unverified and may require formal testing. Inaccurate drug allergy labels are associated with a range of adverse clinical outcomes. Patients with hematological disorders may experience frequent and severe infections; those who have been incorrectly labeled penicillin allergic may benefit from allergy de-labeling (ADL) efforts to facilitate access to beta-lactam antibiotics. We developed a multidisciplinary, pharmacist-driven process that enabled non-allergist trained providers to assess and

A Saudi Woman with Ceftriaxone Induced Fixed Drug Eruption.

BACKGROUND: A fixed drug eruption (FDE) is an immunological cutaneous adverse reaction, classified as a cutaneous adverse drug reaction (CADR) and characterized by well-defined lichenoid lesions that occur at the same site each time. Ceftriaxone is a third-generation antibiotic of cephalosporin antibiotics of the beta-lactam antibiotic family, which has typical in vitro activity against many Gram-negative aerobic bacteria. This is the first clinical case from Saudi Arabia and the fifth in the world to document a woman's experience with recurrent FDE after repeated ceftriaxone use. Case Report.

Non-O1/non-O139 Vibrio cholerae bacteraemia and cholangitis: an unusual case in an oncological patient in Lecco Hospital, Italy.

Only toxigenic serogroups O1 and O139 Vibrio cholerae have been associated with widespread cholera epidemics. Other serogroups (non-O1/non-O139 Vibrio cholerae or NOVC) most often cause sporadic gastrointestinal manifestations. Rarely, NOVC can result in severe extraintestinal manifestations in immunocompromised hosts. Although the presence of Vibrio cholerae is well documented in Mediterranean waters, it is not routinely tested in food sources in European countries. Here we report the case of a 46-year-old woman with a history of Von Hippel-Lindau syndrome who had previously undergone major h
