Unknowns about the neurophysiology of normal and disordered swallowing have stimulated exciting and important research questions. Previously, these questions were answered using clinical and animal studies. However, recent technologic advances have moved brain-imaging techniques such as functional magnetic resonance imaging (fMRI) to the forefront of swallowing neurophysiology research. This systematic review has summarized the methods and results of studies of swallowing neurophysiology of healthy adults using fMRI. A comprehensive electronic and hand search for original research was conducte

Viral infection remains an important cause of morbidity and mortality in transplant recipients. The risk of viral infection in these patients depends on several factors, such as the type of organ transplanted, the intensity of immunosuppression, and the recipient's susceptibility. In additional to direct effects, viral infection cause indirect effects, including greater risk of replication of other viruses, graft rejection, opportunistic infections and other specific entities for each type of transplant. These indirect effects result from the immunomodulatory activity of some viruses, such as

Arthroscopic surgery of the ankle has become indispensable method in the armamentarium of the modern orthopaedic surgeon. Technological advancement and thorough understanding of the anatomy have resulted in improved ability to perform arthroscopy of the ankle. The method is minimally invasive and it allows the direct visualization of intra-articular structures without arthrotomy or malleolar osteotomy. Anterior or posterior approach may be used, and various indications have become generally accepted: anterior soft tissue or bony impingement, loose bodies, osteochondral defects, synovitis (rheu

In the absence of curative options, therapy for aggressive forms of systemic mastocytosis (SM) has relied in the use of cytoreductive agents, mainly interferon-alpha (IFN-alpha) and cladribine. However, responses are transient and only occur in a subset of patients. Gain-of-function mutations at codon 816 of the KIT protooncogene lead to constitutively active Kit receptor molecules, which are central to the pathogenesis of SM. Recent advances in the understanding of the molecular underpinnings of SM have led to the development of small molecules targeting mutant Kit tyrosine kinase isoforms th

Clostridium perfringens is one of the etiologic agents of gas gangrene that can occur when a wound is contaminated with soil. Type A C. perfringens can cause foodborne and nonfoodborne gastrointestinal illnesses due to an enterotoxin (CPE) produced by some strains during sporulation. We developed a quantitative real-time PCR assay based on fluorescence resonance energy transfer hybridization chemistry that targets the C. perfringens-specific phospholipase C (plc) gene and the enterotoxigenic gene (cpe) with the LightCycler and the Ruggedized Advanced Pathogen Identification Device (R.A.P.I.D.)

BACKGROUND: This article describes the characteristics of missed lower urologic injuries at a level 1 trauma center using advanced trauma life support protocols. Between 1991 and 1996, 635 patients were treated for traumatic pelvic fractures. For the 43 patients with missed urologic diagnoses, the pelvic fracture pattern, initial evaluation, average delay to diagnosis and treatment, reason for the delay, and manner in which the lower urinary tract injury was discovered and treated were identified.
RESULTS: Of 43 patients with pelvic fracture, 10 (23%) with concomitant urologic injury had init

Disease mapping and risk assessment are important tasks in the area of medical and veterinary epidemiology. The development of methods for mapping diseases has progressed considerably in recent years. Geographical Information Systems (GIS), Remote Sensing (RS), and Spatial Analysis represent new tools for the study of epidemiology, and their application to parasitology has become more and more advanced, in particular to study the spatial and temporal patterns of diseases. The present review highlights the usefulness of GIS and RS in veterinary parasitology in order to better know the epidemiol

Ras mutation has been detected in approximately 20-30% of all human carcinomas, primarily in pancreatic, colorectal, lung and bladder carcinomas. The indirect inhibition of Ras activity by inhibiting farnesyltransferase (FTase) function is one therapeutic intervention to control tumor growth. Here we report the preclinical anti-tumor activity of our most advanced FTase inhibitor (FTI), ABT-100, and a direct comparison with the current clinical candidates. ABT-100 is a highly selective, potent and orally bioavailable FTI. It broadly inhibits the growth of solid tumors in preclinical animal mode

Tuberculosis vaccine research began with the search for a vaccine that might be better than, and thus could replace, the current Bacillus Calmette Guérin (BCG) vaccine. Over the last fifteen years or so, intense research effort has led to the identification of a number of novel tuberculosis (TB) vaccines which can be divided into 4 categories: genetically modified mycobacteria, protein, plasmid DNA and viral. However, it is increasingly believed that the current BCG vaccine will continue to be used as a childhood vaccine and that more effort should be directed to developing appropriate boostin

Patients with hereditary hemostatic disorders, characterized by a tendency to bleeding or thrombosis, constitute a serious challenge in the dental practice. Advances in the medical diagnosis of hemostatic disorders have exposed dental professionals to new patients not amenable to the application of the management protocols associated with other, more well-known, disorders. It is the aim of this paper to review the evidence, to highlight the areas of major concern, and to suggest management regimens for patients with hereditary hemostatic disorders. An extensive review has been made (PubMed, Sc

Timed artificial insemination (TAI) is a very important tool in production systems, as it has a direct impact on cost-efficiency by reducing labour resulting from oestrus detection. However, to make TAI commercially feasible, hormonal treatments need to assure acceptable pregnancy rates and be economically viable. Recent advances in the knowledge of ovarian physiology (e.g. determination of follicular waves, sensitivity of the early corpus luteum) in small ruminants allowed the development of new treatments focused on an efficient estimation and synchronisation of the time of ovulation of the

Major advances have recently been made in our understanding of the genetic bases of monogenic inherited epilepsies. Direct molecular diagnosis is now possible in numerous inherited symptomatic epilepsies. Progress has also been spectacular with respect to several idiopathic epilepsies that are caused by mutations in genes encoding subunits of ion channels or neurotransmitter receptors. Although these findings concern only a few families and sporadic cases, their potential importance is great, because these genes are implicated in a wide range of more common epileptic disorders and seizure type

The rich sensory innervation of the ankle and foot is manifest through the numerous communicating branches linking the neural trunks, particularly the superficial peroneal and sural nerves on the anterolateral aspect of the hindfoot. The 35 communicating branches seen in 55 dissections (58%) were proximal in half of the cases, lying in the malleolar and lateral tarsal regions, and distal in the other half, in the metatarsal region. The communicating branch was straight in 25 cases and curved in 11. The average distances of the communicating branch from the crest of the lateral malleolus and th

The hereditary dysautonomias (H-Dys) are a large group of disorders that affect the autonomic nervous system. Research in the field of H-Dys is very challenging, because the disorders involve interdisciplinary, integrative, and "mind-body" connections. Recently, medical scientists, NIH/NINDS representatives, and several patient support groups gathered for the first time in order to discuss recent findings and future directions in the H-Dys field. The H-Dys workshop was instrumental in promoting interactions between basic science and clinical investigators. It also allowed attendees to have an

BACKGROUND: Digital necrosis is an uncommon paraneoplastic syndrome. Fewer than 50 cases have been sporadically reported through the literature over the last 30 years.
CASE REPORT: We describe the unfortunate case of a 62-year-old woman with a locally advanced squamous cell carcinoma of the tonsil. She was admitted to the hospital for symptomatic hypercalcemia and was found to have metastatic disease. She subsequently had digital necrosis develop. Despite numerous attempts to control the rapidly progressive ischemia, little objective evidence of response was noted. Her primary disease did not

In Tunisia, as in most african countries, Pneumocystis carinii pneumonia (PCP) is considered to be rare in HIV-infected patients. Frequencies of 8.6% and 21% have been reported. We examined 27 broncho-alveolar lavage specimens collected from HIV-infected tunisian individuals with respiratory symptoms over 4 years (1994-1997), by cyto centrifugation, Giemsa and Gomori-Grocott stain. Pneumocystis carinii (P carinii) was present in 9 cases, accounting for 33.3% of all specimens. Investigation of the reasons for the differences between african reports is necessary to establish appropriate therapeu

BACKGROUND: Adhesions are frequently seen in the deranged temporomandibular joint (TMJ). In the early stage of the degenerative process the adhesion does not seem to be a significant factor in reduced mandibular movement. This is reversed in advanced degenerative joint disease where the adhesion requires release to achieve a satisfactory clinical result. Correlation of clinical history and findings within the joint should increase understanding of limited jaw movement.
METHODS: Arthroscopic examination of deranged TMJs was performed on 75 patients with limited jaw opening and correlated with

Thromboembolic diseases remain the main cause of death in Western societies despite current antithrombotic treatments. Recent advances in the molecular bases of haemostasis have highlighted new targets for novel antiplatelet or anticoagulant agents. Considering antiplatelet agents, selective antagonists of specific receptors (von Willebrand factor, collagen or thrombin receptors) are effective in thrombosis models and direct ADP receptor antagonists and nitric-oxide-releasing aspirin are in Phase I-II clinical trials. Concerning anticoagulants, inhibitors of tissue-factor-induced clotting acti

Thrombotic microangiopathy (TMA) is a known complication of HIV infection. Endothelial cell injury appears to be the primary event causing platelet activation and deposition in the microvasculature. Direct cytopathic roles of HIV as well as other factors such as malignancy, drugs, and infectious agents have been implicated in the pathogenesis of HIV-TMA. Although the the majority of patients present in a more advanced stage of HIV disease, TMA can be the initial presenting symptom of HIV infection. Clinical features are those of idiopathic TMA, and the diagnosis should be suspected in any pati
