A new bipolar system for performing operative hysteroscopy in normal saline.

We evaluated the efficacy and safety of the VersaPoint bipolar vaporization system during hysteroscopic treatment of 10 women with symptomatic submucous leiomyomata, endometrial polyps, uterine septa, and synechiae in a prospective, uncontrolled pilot study at two Canadian university-affiliated teaching hospitals. Patients were two women with recurrent pregnancy loss associated with a diagnosis of uterine septa, one with infertility, one with recurrent pregnancy loss and synechiae, and six with menorrhagia associated with either leiomyomata or endometrial polyps. Electrodes were inserted throu

The use of 2 ipsilateral ureteral stents for relief of ureteral obstruction from extrinsic compression.

PURPOSE: We present our early experience with the novel approach of placing 2 parallel stents simultaneously in extrinsically obstructed ureters in which single stents had failed. The increased stiffness of 2 stents reduces kinking and luminal compression, and the potential space between the stents likely preserves flow around as well as through them. MATERIALS AND METHODS: Four patients recently presented with ureteral obstruction secondary to nonurinary tract malignancies. Previous stenting with a single 6F Double-J* stent had failed in all cases. Three patients experienced flank pain and 1

[Endocrine reactions, circulatory and resuscitation behavior in ketamine-midazolam anesthesia. A comparative study of ketamine racemate vs. (S)-ketamine in knee surgery].

UNLABELLED: Clinically used ketamine is a racemic mixture of two isomers, (S)- and (R)-ketamine, in equal amounts. Previous investigations showed the anaesthetic potency of (S)-ketamine to be three times higher than that of (R)-ketamine. The aim of this study was to compare the effects of (S)-ketamine/midazolam and racemic ketamine/midazolam on endocrine and cardiovascular parameters, recovery, and side effects in unpremedicated patients during knee surgery. METHODS: 41 patients scheduled for elective knee surgery were investigated in a prospective, double-blind, and randomised design. For in

Should upper ureteral calculi be manipulated before extracorporeal shock wave lithotripsy? A prospective controlled trial.

Whether all upper ureteral stones must be manipulated before extracorporeal shock wave lithotripsy (ESWL*) is an ongoing controversy. In a prospective trial, symptomatic patients with solitary upper ureteral calculi less than 1 year in duration were alternated between ESWL in situ and pre-ESWL stone manipulation. Pretreatment excretory urograms were assessed for stone size and degree of proximal hydroureteronephrosis, which was graded from zero (no dilatation) to 3 (severe dilatation). Stone manipulation was done with the patient under intravenous sedation and local anesthesia. A total of 4,00

Maternal and fetal effects of prophylactic hydration with crystalloids or colloids before epidural anesthesia.

Prophylactic intravenous hydration decreases the incidence and severity of hypotension due to obstetric epidural anesthesia. This study assesses whether infusion of normal serum albumin (NSA) offers any advantages over Ringer's lactate (RL) solution. Sixty patients scheduled for elective cesarean sections were divided into three equal groups. Group 1 received 1200 ml of RL; group 2, 700 ml of RL plus 500 ml of 5% NSA; and group 3, 1000 ml of RL plus 200 ml of 25% NSA. In group 1, the concentrations of serum albumin (SA) decreased from 3.5 +/- 0.3 (mean +/- SD) to 3 +/- 0.2 g/dL (P less than 0.

A safe and efficient analgesic approach for percutaneous transluminal angioplasty: Fentanyl plus targeted local anesthesia.

BACKGROUND: Stenosis is a common complication of arteriovenous access (AV), for which percutaneous transluminal angioplasty (PTA) is a well-established intervention. Nevertheless, balloon dilation commonly induces pain. Common analgesic approaches during PTA encompass moderate intravenous sedation and analgesia, brachial plexus block, and local infiltrative anesthesia, each with its own limitations. A need currently exists for a safe, simple, and effective analgesic approach during PTA. METHODS: This retrospective observational study was conducted between January 2020 and July 2022 at a singl

Modulatory role of endogenous adrenaline in propofol-related nociceptive responses in rats.

INTRODUCTION: Propofol is a short-acting intravenous hypnotic anesthetic widely used for sedation and the induction and maintenance of general anesthesia. Although it reliably produces unconsciousness, its intrinsic analgesic efficacy remains a subject of debate. Accumulating evidence indicates that propofol suppresses catecholaminergic activity, and inhibition of endogenous adrenaline may therefore limit its ability to modulate nociceptive processing. Accordingly, this study aimed to investigate the nociceptive suppressions of subhypnotic and anesthetic doses of propofol in rats and their rel

A Novel Modified Ultrasound-Guided Venipuncture Technique for Non-Tunneled PICC Insertion in a Non-Operating Room Anesthesia (NORA) Setting: A Technical Report with Real-World Experience.

Background: Peripherally inserted central catheters (PICCs) are widely used for medium- and long-term intravenous therapies but remain associated with mechanical and thrombotic complications, particularly during venipuncture and guidewire insertion. The growing use of Non-Operating Room Anesthesia (NORA) environments, where anesthesiologists frequently perform ultrasound-guided vascular access under conditions of limited resources and support, underscores the need for simple, reproducible, and inherently safe techniques. The objective of this technical note is to describe a modified ultrasound

Regional Anesthesia as Primary Strategy in Aesthetic Breast Augmentation Surgery: Clinical Outcomes in 627 Sedated Patients Undergoing Pectoral Nerves (PECS I-II) and Parasternal Blocks.

BACKGROUND: Post-operative pain after breast augmentation remains unpredictable, and regional anesthesia alone is seldom described. We evaluated an opioid-free protocol combining pectoral nerves (PECS I-II) and parasternal blocks with intravenous sedation. METHODS: Between January 2020 and December 2024, 627 women (mean age 35 y) underwent bilateral primary augmentation under ultrasound-guided fascial-plane blocks plus sedation. VAS pain scores were recorded at predefined intervals up to 72 h. Descriptive statistics, cubic modelling of pain trajectories and Kruskal-Wallis tests compared impla

Evidence Map of Pharmacologic and Non-Pharmacologic Perioperative Strategies for Managing Acute Postoperative Pain After Laparoscopic Surgery, 2012-2025: The M-PALS Collaborative.

Background: Effectively managing acute postoperative pain after laparoscopic surgery (M-PALS) is essential to optimize outcomes, enhance recovery, and mitigate opioid-related risks. We aimed to systematically map evidence on effectiveness and harms of pharmacologic and non-pharmacologic interventions for M-PALS. Methods: We searched three databases (2012-2025) for randomized clinical trials (RCTs) that reported postoperative opioid use and pain-related outcomes. We assessed study quality using the Cochrane Risk of Bias (ROB)-2 tool. Results: From 7638 citations, we included 101 RCTs. Postopera

Impact of continuous serratus plane analgesia on quality of recovery after video-assisted thoracic surgery: a prospective double-blind, randomized, placebo-controlled trial.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) offers numerous advantages over open surgery; nevertheless, it remains associated with significant postoperative pain. Multiple regional anesthesia techniques have been explored, and the serratus anterior plane block (SAPB) has emerged as a promising option; however, evidence on the use of continuous SAPB infusion for quality of recovery (QoR-40) remains limited. We hypothesized that continuous SAPB infusion, combined with multimodal analgesia, could improve QoR-40 after VATS. METHODS: In this prospective, double-blind, randomized contro

Role of perioperative diaphragmatic point-of-care ultrasound in assessing postoperative pulmonary complications in brachial plexus repair surgeries with preexisting hemidiaphragm paresis: A prospective observational study.

BACKGROUND AND AIMS: A brachial plexus injury (BPI) with phrenic nerve involvement causes hemidiaphragm paresis and subclinical respiratory problems. Surgical traction and anesthetic agents employed during brachial plexus repair may further worsen diaphragm function, complicating extubation and increasing postoperative pulmonary complication (PPC) risk. Point-of-care ultrasound (POCUS) provides rapid, bedside assessment of diaphragm function but has not been evaluated in BPI repair surgery. Hence, this study evaluated the role of perioperative diaphragmatic POCUS (PDPOCUS) for determining PPC
