Association between obesity and statin use on mortality and hospital encounters in atrial fibrillation.

BACKGROUND: Obesity increases risk of atrial fibrillation (AF) at least in part due to pro-inflammatory effects, but has been paradoxically associated with improved mortality. Although statins have pleiotropic anti-inflammatory properties, their interaction with obesity and clinical outcomes in AF is unknown. We explored the relationship between BMI, statin use, and all-cause mortality and AF/congestive heart failure (CHF)-related encounters, hypothesizing that statin exposure may be differentially associated with improved outcomes in overweight/obesity. METHODS: This was a single center retr…

Clinical Impact of Early Rhythm Control and Healthy Lifestyles in Patients With Atrial Fibrillation.

BACKGROUND: There are limited data regarding the combined effect of early rhythm control (ERC) and healthy lifestyle (HLS) behaviors on the risk of ischemic stroke in patients with atrial fibrillation (AF). OBJECTIVES: This study sought to evaluate how the combination of ERC and HLS behaviors affects the risk of ischemic stroke in patients with AF. METHODS: Using the Korean National Health Insurance database, we included patients with new-onset AF between 2009 and 2016 (n = 208,662). Patients who received rhythm control therapy within 2 years after AF diagnosis were defined as the ERC group.…

Symptoms Burden as a Clinical Outcomes Assessment in Heart Failure Patients With Atrial Fibrillation.

BACKGROUND: Safe and effective pharmacologic therapy for atrial fibrillation (AF) in heart failure (HF) is an unmet need. In AF clinical trials, the standard primary endpoint of time to first symptomatic AF event (TTFSE) has several disadvantages, which could theoretically be overcome by measurement of AF-specific symptoms burden during an entire follow-up period. OBJECTIVES: The authors sought to develop and validate a method of measuring symptom burden of AF in a HF population. METHODS: The authors constructed a patient-reported outcome instrument (AFSQ [Atrial Fibrillation Symptoms Questi…

Structural and electro-anatomical characterization of the equine pulmonary veins: implications for atrial fibrillation.

INTRODUCTION/OBJECTIVES: Spontaneous pulmonary vein (PV) activity triggers atrial fibrillation (AF) in humans. Although AF frequently occurs in horses, the origin remains unknown. This study investigated the structural and electro-anatomical properties of equine PVs to determine the potential presence of an arrhythmogenic substrate. ANIMALS, MATERIALS AND METHODS: Endocardial three-dimensional electro-anatomical mapping (EnSite Precision) using high-density (HD) catheters was performed in 13 sedated horses in sinus rhythm. Left atrium (LA) access was obtained retrogradely through the carotid …

The functional substrate in patients with atrial fibrillation is predictive of recurrences after catheter ablation.

BACKGROUND: Enhanced characterization of the atrial electrical substrate may lead to better comprehension of atrial fibrillation (AF) pathophysiology. OBJECTIVE: With the use of high-density substrate mapping, we sought to investigate the occurrence of functional electrophysiological phenomena in the left atrium and to assess potential association with arrhythmia recurrences after catheter ablation. METHODS: Sixty-three consecutive patients with AF referred for ablation were enrolled. Analysis of conduction abnormalities relied on two acquired left atrial electroanatomic maps (sinus and atri…

Using Machine Learning to Predict the Duration of Atrial Fibrillation: Model Development and Validation.

BACKGROUND: Atrial fibrillation (AF) is a progressive disease, and its clinical type is classified according to the AF duration: paroxysmal AF, persistent AF (PeAF; AF duration of less than 1 year), and long-standing persistent AF (AF duration of more than 1 year). When considering the indication for catheter ablation, having a long AF duration is considered a risk factor for recurrence, and therefore, the duration of AF is an important factor in determining the treatment strategy for PeAF. OBJECTIVE: This study aims to improve the accuracy of the cardiologists' diagnosis of the AF duration, …

Oral Anticoagulation Use in Individuals With Atrial Fibrillation and Chronic Kidney Disease: A Review.

Atrial fibrillation (AF) is highly prevalent in patients with chronic kidney disease (CKD). It is associated with an increased risk of stroke, which increases as kidney function declines. In the general population and in those with a moderate degree of CKD (creatinine clearance 30-50 mL/min), the use of oral anticoagulation to decrease the risk of stroke has been the standard of care based on a favorable risk-benefit profile that had been established in seminal randomized controlled trials. However, evidence regarding the use of oral anticoagulants for stroke prevention is less clear in patien…

In-hospital outcomes and postdischarge mortality in patients with acute coronary syndrome and atrial fibrillation.

BACKGROUND: It is unclear whether advances in management of acute coronary syndrome (ACS) and introduction of novel oral anticoagulants have changed outcomes in patients with ACS with concomitant atrial fibrillation (AF). OBJECTIVE: This study aimed to examine the incidence of AF in patients admitted for ACS and to evaluate its association with adverse outcomes, given the recent advances in management of both diseases. METHODS: Natural language processing search algorithms identified AF in patients admitted with ACS across 13 Northwell Health Hospitals from 2015 to 2021. Hierarchical general…

Causes of death in patients with atrial fibrillation anticoagulated with rivaroxaban: a pooled analysis of XANTUS.

AIMS: Anticoagulation can prevent stroke and prolong lives in patients with atrial fibrillation (AF). However, anticoagulated patients with AF remain at risk of death. The aim of this study was to investigate the causes of death and factors associated with all-cause and cardiovascular death in the XANTUS population. METHODS AND RESULTS: Causes of death occurring within a year after rivaroxaban initiation in patients in the XANTUS programme studies were adjudicated by a central adjudication committee and classified following international guidance. Baseline characteristics associated with all-…

Positive antiphospholipid antibodies increase the risk of ischemic stroke in patients with atrial fibrillation.

BACKGROUND: Antiphospholipid antibodies (aPL), including lupus anticoagulant, antibodies against β2 glycoprotein I (anti-β2GPI), and anticardiolipin (aCL) antibodies are associated with ischemic stroke (IS). Their prevalence and clinical relevance in atrial fibrillation (AF) remain unclear. OBJECTIVES: To assess whether aPL are associated with increased risk of IS in AF patients despite anticoagulation. METHODS: We conducted a post hoc analysis of aPL using blood samples from 243 consecutive AF patients enrolled in a cohort study. Markers of a prothrombotic state, including endogenous thromb…

Impact of prior oral anticoagulation on admission stroke severity in patients with atrial fibrillation.

BACKGROUND AND AIMS: In patients with atrial fibrillation, oral anticoagulation therapy is indicated for both primary and secondary prevention of stroke/systemic embolism. Though direct oral anticoagulants with greater safety and efficacy than warfarin were introduced into clinical practice at the beginning of the last decade, even now not all patients with AF have adequate preventative anticoagulant treatment. The primary goal of this study was to evaluate the impact of prior use of oral anticoagulants on admission stroke severity in those with AF. Other aims were, inter alia, to assess the t…

[The 2024 ESC guidelines for management of atrial fibrillation : AF-CARE as new credo].

The new 2024 ESC guidelines on the management of patients with atrial fibrillation (AF) introduces the AF-CARE pathway as the central acronym for AF management. As a result, the management of Comorbidities and risk factors is moving to the forefront of AF management. However, the new guideline also includes important changes in Avoidance stroke and thromboembolism, such as a modified risk score (CHA2DS2-VA) and a statement on anticoagulation in subclinical atrial fibrillation. There are also changes in the concepts of Rhythm and rate control with an upgrading of rhythm control and catheter abl…
