The treatment of subchondral lesions is an area with limited focus within the foot and ankle literature. The literature has shown an association between disruption of the subchondral bone plate and the formation of subchondral cysts. The primary causes of subchondral lesions are acute trauma, repetitive microtrauma, as well as idiopathic means. Evaluation of these injuries should be done carefully and often requires advanced imaging including MRI and computed tomography. The treatment of very distal tibial (pilon) fractures remains one of the great challenges for the orthopaedic traumatologist. It is essential that the treating surgeon have a proper understanding of the fracture, the soft tissue injury, and the skills to deal with these often-complicated injuries to minimize the risk of complications that can occur after these often higher energy injuries. Bone stability can be achieved with both circular ring fixation and plating, and both can yield good results in experienced hands. This debate considers the advantages and disadvantages of each technique.

CASE: Fracture of a sesamoid bone of the thumb is rare. We report a case of such a fracture after hyperextension of the thumb in a 26-year-old woman. After a volar plate injury was excluded, she was treated nonoperatively with 5 weeks of immobilization followed by physical therapy.
CONCLUSION: After reviewing the current literature describing sesamoid fractures of the thumb, we recommend that fracture of the sesamoid of the metacarpophalangeal (MCP) joint should be regarded as an avulsion fracture. The role of surgical intervention should be limited to patients with a painful nonunion.

Aggrecan, encoded by ACAN, is a major proteoglycan component of the extracellular matrix in the growth plate and articular cartilage. Aggrecan provides the hydrated gel structure important for the load-bearing properties of joints and plays a key role in cartilage and bone morphogenesis. At least 25 pathological ACAN mutations have been identified in patients with highly variable phenotypes of syndromic or non-syndromic short stature. This review provides an overview of the current understanding of ACAN and the clinical and genetic findings concerning aggrecan-associated diseases.

Calcaneal tuberosity fractures account for 1% to 3% of all calcaneal fractures. Surgical fixation is particularly challenging owing to osteoporosis and numerous comorbidities and risk factors in this patient population. Numerous techniques have been proposed; however, we describe the use of a locking compression hook plate in the treatment of type 2 fracture patterns. This has the advantage of providing stable fixation in osteoporotic bone, avoiding the disadvantages of soft tissue and metalwork irritation that have been described with other techniques.

Accordion Maneuver is a strong "Bloodless Tool" in the Ilizarov technology, that helps to bring about healing of nonunion of fractures of long bones. A 50 years old male, a rare case of Pycnodysostosis, presented to us with recalcitrant nonunion of the fracture shaft of the femur after two failed surgical attempts, with a broken plate in the last attempt. We treated this very difficult nonunion by Ilizarov monofocal osteosynthesis with Accordion Maneuver that finally yielded union. This appears to be the only case report of treating such a case with Accordion Maneuver.

The distal radius is one of the most commonly fractured long bone. Colles' fracture results from a fall on the dorsiflexed and pronated hand. The dinner-fork deformity is the typical deformity of the Colles' fracture. For patients with no or a little displacement, conservative treatment is applied. The non-bridge type external fixator is applied for patients without an intra articular fracture. For patients with a comminuted fracture, the locking plate (volar approach) is recommended. During the healing period, shoulder, elbow and finger exercise should be insisted.

Autopsy was performed on a fetus of 6 months gestation with sonologic features of skeletal dysplasia to arrive at a pathological diagnosis. Radiograph of fetus was taken which showed short limb bones, curved femora, flattened vertebral bodies with wide intervertebral spaces, bell-shaped chest, short ribs with flared ends which are features described in Thanatophoric dysplasia. This was confirmed by histopathological findings in the epiphyseal growth plate. In this paper, we attempt to describe the radiological and pathological findings of our case with a discussion on comparative literature.

We reviewed 845 cases of benign and 356 cases of malignant cartilaginous tumors from a total of 3067 primary bone tumors in our database. Benign cartilaginous lesions are unique because the epiphyseal plate has been implicated in the etiology of osteochondroma, enchondroma (single or multiple), periosteal chondromas and chondroblastoma. In the first part of this paper, we will review important clinical, radiologic and histologic features of intramedullary cartilaginous lesions in an attempt to support theories related to anatomic considerations and pathogenesis.

New information affecting the diagnosis and treatment of a wide variety of musculoskeletal conditions in children was published within the past year. A new etiology for acquired torticollis as well as a test to distinguish ocular and muscular torticollis were described. Familial predisposition for successful nonoperative treatment of interventional disk herniation was reported. Bone scintigraphy was shown to be helpful in defining the cause of occult gait disturbances, and accurate mapping of premature growth plate closure may soon be possible with magnetic resonance scans.

This article presents some ideas and techniques for surgical reduction of complicated orbit and nasoethmoid complex fractures in young children. Operative techniques for surgical exposure and fracture reduction and fixation have evolved from standard facial incisions and wire fixation to complete exposure of the cranium and orbits via a bicoronal scalp flap, application of rigid plating techniques, and use of cranial bone grafts when necessary. This evolution of surgical techniques has been accompanied by an overall improvement in cosmetic and functional results.

The authors have treated 86 fractures of the lower end of the femur by internal fixation with an A.O. blade-plate. An adequate follow up was made in 71 cases. A classification of the fractures is given. Half of the fractures were either complex or involved the joint. The technique that was used is described. The authors consider that an immediate cancellous autogenous bone graft is worthwhile. 8 cases of non-union, 7 stiff knees and 31 malunions were seen; 70 per cent of cases achieved satisfactory function.

Galectin-3 is expressed in various tissues, including the bone, where it is considered a marker of chondrogenic and osteogenic cell lineages. This study investigated the dose- and time-dependent effects of caffeine consumption throughout puberty in peripubertal rats. A total of 85 male SD rats were randomly divided into four groups: control and caffeine-fed groups with 20, 60, or 120 mg/kg/day through oral gavage for 10, 20, 30, or 40 days. Caffeine decreased body weight gain and food consumption in a dose- and time-dependent manner, accompanied by a reduction in muscle and body fat. Open wedge high tibial osteotomy has become the trend for correction of varus knee deformities. The drawbacks were the need of autogenous bone graft with its associated morbidity, and later the use of bone substitutes with their cost and delayed healing. In this study, a total of 58 consecutive patients underwent high tibial osteotomy with internal fixation by wedge (toothed) plate and screws without bone graft, from 2004 to 2008. Age of the patients ranged from 24 to 65 years. There were 37 women and 21 men. The osteotomy opening size ranged from 8 to 14 mm. The mean follow-up was 38 months.

INTRODUCTION: Infected tibial shaft non-union after internal fixation is a difficult orthopedic problem because infection control, fracture stability, and bone healing must be achieved simultaneously. Ilizarov external fixation is a valuable limb salvage option in such complex cases.
