The effect of radiation therapy on fungal growth: results of in vitro and in vivo studies.

OBJECTIVES: To investigate the effect of radiation therapy (XRT) on the growth of Candida and Cryptococcus. METHODS: (I) In vitro study: suspensions of Candida and Cryptococcus were irradiated by 0-20 Gy, and colony forming units (CFUs) were counted after 2 and 11 days. (II) In vivo study: (A) The XRT effect-Balb/c mice were injected with Candida or Cryptococcus and irradiated by 5 or 10 Gy. Homogenates of their kidneys or brains were cultured and CFUs were counted two days later. (B) Toxicity-Balb/c mice were injected with Candida and irradiated by 5 Gy. Control mice were either injected or

[Taxonomic characterization and antibiotic resistance of pathogens of perinatal infectious inflammatory pathology in infants in Orenburg].

The materials on regional features of the nosologic structure, taxonomic spectrum and antibiotic resistance of the pathogens of perinatal infectious inflammatory pathology (IIP) in infants in Orenburg are presented. From 2002 to 2005 the average level of IIP of 19.0 cases per 1000 living new borns and the dominance of skin inflammatory diseases (vesiculopustulosis in particular) in the nosologic structure among the other forms of IIP (including conjunctivitis and pneumonia) were observed. The prevalence of the gram positive cocci with the leadership of coagulase negative staphylococci among th

Antifungal combination therapy: clinical potential.

Combination antifungal therapy has been an area of research and clinical interest since systemic antifungals became available decades ago. In vitro and clinical data were generated for some of the more common invasive fungal infections, especially candidiasis, but until very recently few clinical studies were performed. The first invasive fungal infection to be examined in clinical trials with adequate statistical power was cryptococcal meningitis and several of these trials stand out as classical studies in the clinical evaluation of combination antifungal therapy. More recently, since the av…

'Decubital candidosis': a study of 26 cases.

BACKGROUND: Candidoses are infections caused by yeasts of the genus Candida. 'Decubital candidosis' is a particular form of cutaneous candidosis that occurs on the dorsal skin of chronically bedridden patients; there are very few studies about this presentation in the medical literature. OBJECTIVE: To study the clinical, mycological and histopathological features of 26 cases of 'decubital candidosis' along with factors that may predispose to it. METHODS: Twenty-six patients were included and their general characteristics and clinical lesions were carefully studied. The presence of candidosis

Prophylaxis of Candida infections in adult trauma and surgical intensive care patients: a systematic review and meta-analysis.

OBJECTIVE: To determine whether systemic antifungal prophylaxis decreases infectious morbidity and mortality in nonneutropenic, critically ill, trauma and surgical intensive care unit (ICU) adult patients. DESIGN: Systematic review and meta-analysis of randomized clinical trials. We used a fixed effect model, with risk ratio (RR) and 95% confidence intervals (CI). PARTICIPANTS: Patients admitted to ICU after surgery or trauma, with multiple risk factors for fungal infections. INTERVENTIONS: Nine studies (seven double blind) with a total of 1,226 patients compared ketoconazole (three) or flu

Diagnosis and treatment of salivary gland disorders.

The keystone of the architecture of the oral cavity is saliva; however, it is rarely acknowledged as a vital physiologic secretion. Saliva plays three major roles in oral and systemic health. It provides host protection, assists in the initiation of food and fluid intake, and enables communication through speech. Without adequate salivary output augmented by a rich assortment of salivary proteins and electrolytes, oral and pharyngeal health declines as well as a person's quality of life. This article will provide a brief summary of the function of saliva, oral and systemic etiologies of saliva

Otomycoses of candidal origin in eastern Slovakia.

Mycological analysis of swabs and scraping samples from the external ear canals of 40 patients with clinically diagnosed otomycosis (10 neonates, 30 adults) revealed the presence of fungi as etiological agents. They were investigated microscopically using 20 % potassium hydroxide, and by cultivation on Sabouraud's glucose agar. The Candida species were identified using the germ-tube test, micromorphology observations of colonies on rice agar, and particularly by the commercial kit AUXAcolor. The following Candida species were identified in the aural material examined: C. albicans (n = 21; 52.5

Vaginal problems? Treat yourself to the right solution.

You have vaginal itching and discharge, a common problem for many women. You assume it's a yeast infection, but don't be too quick to self-diagnose. Before you treat yourself with an over-the-counter (OTC) medication for a yeast infection, educate yourself about hte signs and symptoms. Often, seeing your health care professional will be the correct first step. Although OTC yeast medications generally work against yeast, they don't work in treating other forms of vaginal problems that may have more serious health consequences.

[Pneumocystis carinii pneumonia in patients without acquired immunodeficiency syndrome (AIDS)].

The incidence of Pneumocystis Carinii pneumonia (PCP) may rise due to a more extensive use of immunosuppressive therapy. Information and guidelines in the last decade regarding prevention, diagnosis and treatment of the disease were based upon data collected in patients with Acquired Immuno Deficiency Syndrome (AIDS). However, scientific data regarding risk factors, clinical manifestations and treatment of PCP in patients without AIDS is accumulating. The major risk factors for acquiring PCP are hematological malignancies and treatment with immunosuppressive drugs, especially corticosteroids.

State-of-the-art review of pulmonary fungal infections.

The endemic mycoses are restricted geographically based on environmental and other factors that favor the growth of these organisms in the soil. Histoplasmosis and blastomycosis mostly afflict patients in the Mississippi and Ohio River Valleys whereas coccidioidomycosis occurs primarily in the desert southwest United States. Cryptococcosis also may present as pulmonary disease, particularly in persons with cellular immune impairment. These mycoses are increasing in importance as causes for opportunistic disease in immunocompromised patients, especially those with acquired immune deficiency syn

Epidemiology of fungal infections in solid organ transplant patients.

The epidemiology of fungal infection in solid organ transplant patients is of concern due to the high mortality associated with this complication. Rates of fungal infections vary by type of transplant recipient. Most of these infections occur two to six months after transplantation. Liver transplant recipients are more likely to have early fungal infection which is often due to Candida species. Exogenous and endogenous Candida infection may occur in the immunosuppressed patient in the intensive care unit. Patients with chronic rejection are more likely to have late infection (after six months)

[Candida in dermatology].

Regarding Candida in dermatology, two pathogenetic pathways must be taken into account: 1. on infection of the skin 2. immunological reactions with skin alterations as a result of Candida infection or colonization in the mouth and/or intestine. Case reports describe typical situations of napkin dermatitis, intertriginous candidosis, the intrauterine Candida infection of the foetus, Candida granuloma, Candida folliculitis and Candida paronychia. In the second part results of investigations of patients suffering from psoriasis, atopic dermatitis and urticaria are presented. There were no differe

Terbinafine: broad new spectrum of indications in several subcutaneous and systemic and parasitic diseases.

Immunocompromised patients are at risk of contracting serious fungal infections. The emergence of acquired resistance to azole treatment by opportunistic fungal organisms is increasing and poses a major therapeutic challenge. Treatment of some deep cutaneous and subcutaneous mycoses remains unresolved, relapses are frequent, lack of tolerability of the antifungal drugs becomes an obstacle, and, unfortunately, surgery is, in some cases, the last option. The development of allylamine antifungals, of which terbinafine is the most effective to date, may help to resolve this situation. In vitro, te

[Pancreatic necrosis infection by Candida parapsilosis associated with fungemia].

Pancreatic necrosis infection is the most common complication affecting mortality of severe acute pancreatitis (death rate 80%). Bacterial infections particularly with coliforms or anaerobes account for the majority of cases of infected necrosis. Fungal pancreatic infections with Candida species are rare and often nosocomial. We report herein the first case of pancreatic necrosis infection with Candida parapsilosis associated with fungemia confirmed by molecular typing.

Antibody and/or cell-mediated immunity, protective mechanisms in fungal disease: an ongoing dilemma or an unnecessary dispute?

Historically there has been controversy on the relative importance of antibody- and cell-mediated immune responses in the protection against fungal pathogens. The controversy was fuelled by the difficulties encountered in obtaining consistent results with polyclonal antibody experiments and I inducing long-lasting immune protection by vaccinations with induce stron cell-mediated responses. Recent studies indicate that both antibody- and cell-mediated immune responses can contribute to host protection against Candida albicans and C. neoformans. At the present time the major issue is not the rel

[Patterns of Candida esophagitis in cancer and AIDS patients: histopathological study of 23 patients].

Candida oesophagitis is a common concomitant disease in neutropenic cancer patients after chemotherapie as well as in HIV-patients. In order to characterize the features of oesophagitis in each population, we reviewed the medical history and pathology records of 23 patients (18 cancer-patients, 5 HIV-patients) with culture and autopsy-proven Candida oesophagitis. Histopathological patterns of morphology, invasion, angioinvasion and inflammation were evaluated. Virtually all patients, 17/18 cancer- and 5/5 HIV-patients, had a history of previous mucosal candidosis or candidemia. There was a sig

Fungal infections in patients with acquired immunodeficiency syndrome.

The incidence and severity of fungal infections appear to increase with progression of HIV disease. Because of the significant morbidity and mortality associated with the mycoses discussed, knowledge of the clinical syndromes, early diagnosis, and prompt institution of therapy are crucial for a favorable outcome. For disseminated or invasive fungal infections, suppressive therapy must be continued to prevent relapse.
