Hyperammonemia in children: on the crossroad of different disorders.

BACKGROUND: Symptoms of hyperammonemia occur in patients irrespective of the kind of metabolic diseases. Age, metabolic and nutritional status, and decompensation factors such as infections influence clinical manifestations. Prolonged, untreated hyperammonemia leads to brain injury and intellectual disability. Treatment is directed at lowering plasma ammonia. Brain ammonium concentrations are 1.5 to 3.0 times higher than that in blood. REVIEW SUMMARY: The authors discuss the pathophysiology of the symptoms and consequences of hyperammonemia in children, focusing on the metabolic disorders lea

[Lyme disease. Is it so uncommon?].

Lyme disease, caused by Borrelia burgdorferi sensu lato, is a multisystem disease that may occur with rheumatic, dermatological, neurological symptoms and heart diseases. Its most typical lesion is erythema migrans, although not always present. The clinical diagnosis is supported by serology, and treatment is almost always tetracycline in young people and adults. Amoxicillin is most often used in children and pregnant women, and cephalosporins in the presence of neurological signs. The prognosis, with the appropriate treatment, is good. But if not treated promptly, there may be chronic signs,

Neuroimaging in non-accidental head injury in children: an important element of assessment.

Head injury from physical abuse is unfortunately a common occurrence in our society. It is a major cause of mortality and long-term physical and psychological disability in children. Diagnosis of non-accidental head injury may be difficult, as most infants present with non-specific clinical findings and without external signs of trauma. Neuroimaging plays a fundamental role both for medical management and medicolegal aspects of child abuse. It is therefore imperative for the radiologist to promptly recognise the radiological findings of various forms of non-accidental head injury to render a m

Management of the flexible flat foot in the child: a focus on the use of osteotomies for correction.

Pes planus, commonly referred as flat foot, is a combination of foot and ankle deformities. When faced with this deformity in children, the treating surgeon should use a systematic method for evaluation to distinguish normal variation from true pathology, as well as conditions that have a benign natural history versus those that may lead to significant disability if left untreated. Certain deformities will inevitably worsen and therefore require surgery. Common sense clearly supports the indication for a simple procedure, such as an arthroereisis or an osteotomy, performed in the young child a

Postburn sequelae in the pediatric patient: clinical presentations and treatment options.

Each year, more and more children acquire burns that require serious medical attention. A vast number of these burns lead to permanent disfigurement and long-term disability. As health care providers, focus should not only be on the immediate treatment, but also on the long-term outcome of these burns and the required rehabilitation that these burn patients must go through. During the rehabilitation phase of the burn, focus should be placed on how to prevent and treat several sequelae that include hypertrophic scarring, keloids, contractures, heterotopic ossification, leukoderma, and pruritus.

Drug-induced movement disorders in children.

The increased use of stimulants, antipsychotic agents, and antidepressant drugs in children by primary care physicians, psychiatrists, and neurologists has inevitably led to increased numbers of pediatric patients manifesting the side effects of these agents, many of which are movement disorders. Unlike the isolated abnormal involuntary movements associated with drugs prescribed for epilepsy or asthma, movement syndromes (eg, acute dystonic reaction, neuroleptic malignant syndrome, serotonin syndrome, tardive dyskinesia) associated with psychotropic drugs are complex, difficult to recognize, a

Doctor-patient relations in Nazi Germany and the fate of psychiatric patients.

German psychiatrists actively engaged in the forced sterilization and killing of psychiatrically disabled children and adult patients. Academic psychiatrists embraced the Nazi philosophy and led the way in the "final solution" for psychiatric patients. This took place in a climate of widespread racism, virulent anti-Semitism, disillusionment with utopian social reforms, loss of medical confidentiality, devaluation of autonomy, intoxication with collectivism, injured national pride, and economic crisis. In this paper I review the impact on the physician-patient relationship of scientific, socio

Lymphoid hyperplasia of the bowel and its surgical significance in children.

Twelve children with abdominal complaints had lymphoid hyperplasia of the bowel presenting in an acute or chronic form. The etiology is obscure. An infectious process is thought to precipitate the acute form of the disease. Parasites were identified in two patients with the chronic disease. The acute form, with involvement of the appendix or terminal ileum, presents commonly as acute appendicitis. Because of its self-limiting nature, appendectomy with perservation of the terminal ileum is appropriate. When intussusception is present, resection of the ileum is advisable. The chronic form, which

Preschool neurodevelopment in Zika virus-exposed children without congenital Zika syndrome.

BACKGROUND: Children with in utero Zika virus (ZIKV) exposure without congenital Zika syndrome (CZS) are at risk for abnormal neurodevelopment. Preschool-age outcomes for children with antenatal ZIKV exposure have not yet been established. METHODS: Children with in utero ZIKV exposure and non-exposed controls had neurodevelopmental evaluations at age 3-5 years in Sabanalarga, Colombia. Cases did not have CZS and were previously evaluated prenatally through age 18 months. Controls were born before ZIKV arrival to Colombia. Neurodevelopmental assessments included Pediatric Evaluation of Disabil

Intestinal parasites may be associated with later behavioral problems in internationally adopted children.

AIM: At arrival in new home country, internationally adopted children often have intestinal parasites. International adoptees also exhibit more behavioral problems than their biological peers. We examined whether intestinal parasite infections in international adoptees on arrival in Finland are associated with their later behavioral and emotional problems. METHODS: Data for this study were sourced from the Finnish Adoption Study (FinAdo) based on parental questionnaires for all internationally adopted children under 18 years (n = 1450) who arrived in Finland from 1985 to 2007. A total of 1293

Projecting the incidence of burn injuries and its associated economic burden for 195 countries.

BACKGROUND: Burn injuries remain a persistent global health burden affecting low- and middle-income countries. Projections of future incidence and disability-adjusted life years (DALYs) are needed to guide prevention and resource allocation. METHODS: Age-, sex-, and country-specific burn incidence and DALY rates were pulled from the Global Burden of Disease and combined with World Bank population projections for 2030, 2040, and 2050 to generate estimates by country and region. Economic burden was modeled by multiplying DALYs by gross domestic product (GDP) per capita. RESULTS: Globally, proj

Clinical predictors of intubation and prolonged ventilation in pediatric Guillain-Barré syndrome requiring intensive care: A retrospective observational study.

INTRODUCTION: Pediatric Guillain-Barré syndrome (GBS) may progress to respiratory failure requiring mechanical ventilation (MV), yet predictors of intubation, prolonged MV (PMV), and tracheostomy remain poorly defined. We aimed to identify factors associated with intubation and PMV and evaluate the impact of early versus late tracheostomy (>14 days) on outcomes. METHODS: We retrospectively reviewed 75 children with GBS admitted to a Neuro-ICU (2008-2018). Demographic, clinical, electrophysiological, and cerebrospinal fluid data were collected. Outcomes included intubation, PMV (>21 days), tra

Design and prioritization of the French surveillance program of health effects in the context of exposure to endocrine disruptors using a Delphi study.

BACKGROUND: Rapidly increasing evidence has documented that endocrine disruptors (EDs) contribute substantially to disease and disability, particularly neurodevelopmental disorders, metabolic diseases, reproductive disorders, immune and thyroid dysfunctions, as well as hormone-related cancers. These conditions can lead to chronic impairments and disability, such as learning and behavioral difficulties, infertility, long-term treatment dependence, or reduced functional capacity. Biomonitoring studies conducted in France have shown widespread exposure of the general population to several endocri

Unmet Need for Family Planning Service and Associated Factors Among Homeless Women of Reproductive Age Group in Jimma Zone Administrative Towns, Ethiopia.

BACKGROUND: The majority of homeless women (75%) do not wish to have or delay a pregnancy during this difficult time, yet many do not use contraception in developing countries. In Ethiopia, there is limited evidence of unmet need for family planning in settings where homelessness is on the increase. This study aims to assess the unmet need for family planning services among homeless women and identify associated factors in Jimma Town, southwestern, Ethiopia. METHODS: A community-based cross-sectional study was conducted on May 3rd and 4th, 2021. The complete enumeration was done to include 20

Prevalence of iron deficiency and related factors in Spanish adolescents.

Iron deficiency anaemia continues to be the world's most important cause of years lived with disability in children and adolescents. Assessment of iron deficiency traditionally depended on laboratory parameters that may be modified by inflammation states, including obesity, which is nowadays a current condition in adolescent population of high-income countries. The present study ascertains the prevalence of iron deficiency and its related factors in adolescents, using the serum transferrin receptor and the reticulocyte haemoglobin content, in order to avoid this confusing effect of classical p

Outcomes of a Simple Treatment for Complex Regional Pain Syndrome Type I in Children.

BACKGROUND: Chronic Regional Pain Syndrome type I (CRPSI) in children is a disorder of unknown etiology. No standard diagnostic criteria or treatment exists. Published treatment protocols are often time and resource intensive. Nonetheless, CRPSI is not rare and can be disabling. This reports the results of a simple and inexpensive treatment protocol involving no medicines, nerve blockades, physical therapy resources or referrals to pain specialists. The patient is instructed in a self-administered massage and mobilization program. The diagnosis required allodynia (pain on light touch of the sk

The forgotten learning disability: epidemiology of written-language disorder in a population-based birth cohort (1976-1982), Rochester, Minnesota.

OBJECTIVE: The aim of this study was to report the incidence rates and other epidemiologic characterizations of written-language disorder. There have been no epidemiologic studies on the incidence of written-language disorder in the United States, and the use of a population-based birth cohort, longitudinally followed, is the most powerful method for reaching this objective. METHODS: In this population-based, retrospective birth cohort study, subjects included 5718 children born between 1976 and 1982 in Rochester, Minnesota, who remained in the community after 5 years of age. Records from all

The Down Syndrome Profile Emerges Gradually Across Early Development.

BACKGROUND AND AIMS: Down syndrome (DS) is associated with intellectual disability, with particular difficulties in expressive language and gross motor abilities, and relative strengths in receptive language. Here, we examine how this profile arises over the first five years of life. METHODS: A total of 104 children with DS (6-63 months) participated in a standardised developmental assessment (Mullen Scales of Early Learning; MSEL). Developmental trajectories were analysed cross-sectionally and validated with a longitudinal subsample. RESULTS: The trajectories gradually diverged from typical

Baclofen and Botulinum Toxin A Use in Tone Management for Children With Cerebral Palsy: A Review of Current Literature and Research Gaps in Pre- and Perioperative Care.

Cerebral palsy (CP) is a leading cause of motor disability in children. Many children with CP have hypertonia, and some will require orthopedic surgery. Botulinum toxin A (BoNT-A), a muscle relaxant, is commonly pre- or perioperatively injected to improve surgical outcomes and reduce postoperative pain and muscle tone. However, inconsistent evidence supporting its efficacy, potential research bias from industry sponsorship, and numerous adverse effects, such as long-term changes to muscle morphology, highlight the need for a better alternative. Preoperatively increasing the dose of oral baclof
