DBT-enhanced habit reversal treatment for trichotillomania: 3-and 6-month follow-up results.

BACKGROUND: Maintenance of gains with cognitive-behavioral treatment for trichotillomania (TTM) has historically been problematic. METHODS: We conducted follow-up assessments 3 and 6 months after completion of a 3-month maintenance phase on 10 individuals with DSM-IV-TR TTM who participated in an open trial of a dialectical behavior therapy (DBT)-enhanced habit reversal treatment (HRT). RESULTS: Significant improvement from baseline was reported at 3-and 6-month follow-up on all measures of hair pulling severity and emotion regulation, although some worsening was reported on some measures fr

The central amygdala projection to the substantia nigra reflects prediction error information in appetitive conditioning.

The central amygdala nucleus (CeA) plays a critical role in cognitive processes beyond fear conditioning. For example, intact CeA function is essential for enhancing attention to conditioned stimuli (CSs). Furthermore, this enhanced attention depends on the CeA's connections to the nigrostriatal system. In the current study, we examined the role of the CeA's connections to two midbrain dopamine regions, the substantia nigra pars compacta (SNc) and the ventral tegmental area (VTA), in processing CS information when predictions of reward or nonreward were confirmed or disconfirmed. Initially, tw

Abnormal ventral frontal response during performance of an affective go/no go task in patients with mania.

BACKGROUND: Patients with mania show a behavioral bias toward positive information in an emotional go/no go task. This is the converse of the bias toward negative information seen in unipolar depression that we have recently related to the abnormal function of ventral and medial prefrontal cortices (PFCs). The aim of this study was to investigate the neuronal basis of the bias toward positive information in manic patients. METHODS: During performance of an emotional go/no go task using functional magnetic resonance imaging, 8 manic patients and 11 healthy controls were scanned. The task allow

Enhancing the quality of life of a client with severe developmental disabilities through the application of assistive technology. A case study.

This paper describes a structural sequential process aimed to enhance the quality of life of a 29 year old man through the application of assistive technology (AT). The client had life-long severe multiple disabilities, showed increasing fragility and decreasing functional abilities that resulted, among other problems, in a problem with toileting. The process involved a thorough assessment and evaluation of the client, his physical, cognitive and perceptual skills involved in the performance of the task, requirements of the task, the technology, and the task environment. A clinical reasoning m

Prevention of amitriptyline-induced avoidance impairment by tacrine in mice.

The effects of two cognition enhancers on avoidance impairment induced by the tricyclic antidepressant amitriptyline were assessed during shuttle-box avoidance acquisition and in previously trained mice of the DBA/2 strain. The nootropic agent piracetam (50, 100 or 200 mg/kg, i.p.) had slight or no effect in mice receiving amitriptyline (5 or 10 mg/kg, i.p.). Conversely, the acetylcholinesterase inhibitor tacrine (0.5, 1, 2 or 3 mg/kg, i.p.) prevented the avoidance impairment induced by 5 mg/kg amitriptyline on shuttle-box avoidance acquisition as well as on a previously learned avoidance resp

Exercise intervention for rumination: from neural mechanisms to clinical applications.

Rumination is a core modifiable cognitive risk factor for the onset and maintenance of mood and anxiety disorders. Exercise emerges as a safe, accessible, and scalable non-pharmacological intervention with potential to mitigate rumination and enhance mental health. This review synthesizes evidence on the neurobiological mechanisms and clinical efficacy of exercise interventions for rumination. Evidence suggests that exercise modulates activity in the default mode network and PFC-limbic circuits, and promotes the release of key neurotransmitters and neurotrophic factors. Therapeutic outcomes ar

Ketogenic diet restrains herpes simplex encephalitis via gut microbes.

Herpes simplex virus type 1 (HSV-1) infection-associated herpes simplex encephalitis (HSE) is an occasionally but severe neuronal disease that causes behavioral disorder and impairs cognition. Herein, we demonstrate that the consumption of ketogenic diet (KD), a low-carbohydrate high-fat diet, restricts the neurotropic infection of HSV-1 and HSE progression in mice. KD reduced weight loss, neurodegenerative symptoms, virus production and neuroinflammation, resulting in the enhanced survival rate of HSE mice. Notably, depletion of gut microbes by antibiotics attenuated the protective function o

Amino Acid Uptake, Glucose Metabolism, and Neuroinflammation in John Cunningham Virus Associated Progressive Multifocal Leukoencephalopathy.

A 69-year-old woman presented with progressive dysarthria and cognitive deficits. On MRI, a T2-hyperintense, non-contrast-enhancing lesion was found in the left precentral area. 18F-FET and 18F-FDG PET scans revealed faint amino acid uptake and glucose hypometabolism of the lesion. To assess a neuroinflammatory component, TSPO PET with 18F-GE-180 was performed, where tracer uptake markedly exceeded the T2-hyperintense areas. Histology derived from a stereotactic biopsy findings confirmed John Cunningham virus-associated progressive multifocal leukoencephalopathy. This case underlines that TSPO

RESNA position on the application of power mobility devices for pediatric users.

This paper serves as an update to the previous RESNA Position on the Application of Power Wheelchairs for Pediatric Users with more current and additional scientific literature. This document contains typical clinical applications and best evidence from the literature supporting the application of power mobility (PM) for young children and to assist practitioners in decision-making and justification. It is RESNA' s position that age, limited vision or cognition, behavioral issues, and the ability to walk or propel a manual wheelchair short distances should not, in and of themselves, be used as

The rehabilitative effects on written language of a combined language and parietal dual-tDCS treatment in a stroke case.

In this paper we report the effect of a combined transcranial direct current stimulation (tDCS) and speech language therapy on linguistic deficits following left brain damage in a stroke case. We show that simultaneous electrical excitatory stimulation to the left and inhibitory stimulation to the right parietal regions (dual-tDCS) affected writing and reading rehabilitation, enhancing speech therapy outcomes. The results of a comparison with healthy controls showed that application of dual-tDCS could improve, in particular, sub-lexical transcoding and, specifically, the reading of non-words w

Collaboration in cognitive-behavioral therapy.

Collaborative empiricism, which involves a systemic process of therapist and patient working together to establish common goals in treatment, has been found to be one of the primary change agents in cognitive-behavioral therapy (CBT). This article focuses on the development of a therapeutic relationship and implementation of collaborative empiricism along with the elements that lead to success in treatment. This method is used to uncover patients' automatic thoughts and underlying beliefs in treating an array of emotional and behavioral disorders. The role of the therapist is discussed in deve

Catastrophizing as a cognitive vulnerability factor related to depression in workers' compensation patients with chronic musculoskeletal pain.

The purpose of this study was to examine the role of catastrophizing as a mediator and moderator between life stress and depression in a sample of workers' compensation patients with chronic musculoskeletal pain. Pain intensity, life stress (especially work and financial stress), and catastrophizing contributed significantly to depression. Catastrophizing was found to be partially mediating the relationship between life stress and depression and a moderator between social stress and depression. The results supported the role of catastrophizing as a cognitive vulnerability-stress factor related

AD brain pathology: vascular origins? Results from the HAAS autopsy study.

An increasing number of studies suggest a vascular contribution to Alzheimer's disease (AD). One major question these findings raise is whether vascular disease enhances the formation of AD-like lesions, or whether vascular disease just adds to clinical severity. We examined this question in a fully characterized autopsy sample based on the Honolulu Asia Aging Study. We found that AD markers of neurodegeneration (amyloid plaques, cerebral amyloid angiopathy and neurofibrillary tangles) were no more prevalent in those with neuropathologically defined vascular lesions compared to those without l

Successful outcome of progressive multifocal leukoencephalopathy in a renal transplant patient.

We report the case of a 47-year-old man who developed progressive multifocal leukoencephalopathy (PML) after receiving immuno-suppressive therapy for renal transplantation. The patient presented with a focal seizure and cognitive changes 5 months post-transplantation. He was found to have enhancing lesions in the parietal lobe and typical findings of PML in a brain biopsy. Immunosuppression was discontinued and the neurological symptoms gradually resolved over a period of 4 weeks. The patient is free of any neurological symptoms 36 months after the diagnosis of PML and imaging studies demonstr

Academy of eating disorders international conference.

New information is being acquired and disseminated about eating disorders, particularly in terms of integrating the roles of genes and environment, and new treatment approaches. Although evidence indicates that genes are not more important in the aetiology of anorexia nervosa (AN) than bulimia nervosa, Western culture does appear to be more important in the aetiology of bulimia nervosa than AN. Pathological fear conditioning offers a very useful and experimentally testable theory of the aetiology of AN. New evidence suggests that an enhanced, 'transdiagnostic' form of cognitive behavioural the

Two cases of valproate-induced hyperammonemic encephalopathy without hepatic failure.

We report two children with localization related epilepsies, who presented with somnolence, seizure exacerbation, behavioral alteration, decline in speech and cognitive abilities, and ataxia while being treated with a combination of valproate and topiramate, but had previously tolerated valproate with other antiepileptic drugs. These children had elevated serum ammonia, normal transaminase levels, and generalized slowing of EEG background activity during encephalopathy, which promptly reverted back to normal along with clinical improvement following withdrawal of valproate. A test of the cognitive model of panic: primed lexical decision in panic disorder.

The aim of the present study was to investigate whether individuals with panic disorder are characterised by an enhanced tendency to associate particular somatic sensations with threatening outcomes, compared with nonclinical controls. In order to test this prediction, a modified lexical decision task was employed. Panic disorder subjects and nonclinical control subjects made lexical decisions to neutral word pairs and threatening word pairs. Threatening word pairs consisted of combinations of somatic sensations and catastrophic outcomes (e.g., breathless-suffocate), which have been shown to b

Slow running benefits: Boosts in mood and facilitation of prefrontal cortex function even at very light intensity.

Although running upright has been reported to have positive effects on both physical and mental health, the minimum running intensity/speed that would benefit mood and prefrontal cognition is not yet clear. Here, we tested the hypothesis that a brief bout of very slow running, which is classified as a very light intensity exercise, would enhance positive mood and executive function and increase activation in executive-function-related prefrontal subregions. Twenty-four healthy participants completed a 10-minute very slow running session on a treadmill at 35% V̇o2peak and a resting control sess
