[Autointoxication with 'suicide powder'].

We present two patients who were treated for an intentional overdose of sodium nitrite. When ingested sodium nitrite leads to severe methaemoglobinaemia, resulting in severe hypoxia (as methaemoglobin does not transport oxygen), vasodilation and hypotension. Symptoms include cyanosis, headache, nausea, convulsions, coma and death. When measured by pulse oximetry, patients with a sodium nitrite intoxication and severe methaemoglobinaemia generally have an oxygen saturation of around 85%. This value is unreliable as the oxygen content of the blood is often extremely low - this can be confirmed b

In vivo characterization of histological and immunological response of allergic protein of Metapenaeus dobsonii using an adjuvant free BALB/c mice model.

Shrimp allergy, a common form of food allergy is an adverse immunological response to shrimp proteins. BALB/c mice was sensitized by an adjuvant free oral administration of purified tropomyosin, from Metpenaeus dobsonii to characterize intestinal histological responses and immunological protein recognition pattern as it is unpractical in human subjects. Sensitized mice with higher dose of tropomyosin expressed symptoms of anaphylaxis including puffiness around eyes and snout, no activity, tremor and convulsion after challenge. The responses of high level of sera IgE, tropomyosin specific IgE a

[Apparent life threatening events in infants].

Apparent lifethreatening events in infants. The apparent life-threatening event (ALTE) of the infant is a classic symptom in pediatric emergencies. It is defined as a sudden and unexpected episode involving tone disorders and coloring (pallor, cyanosis) occurring usually before 6 months. It's severity, rare, is not correlated with the anxiety aroused in the adults around the child, but can be objectified by biological signs of tissue damage. Various etiologies can lead to ALTE, most often benign. The investigation is mainly based on history and clinical examination. Some very rare cases may be

Hypothermia due to limbic system involvement and longitudinal myelitis in a case of Japanese encephalitis: a case report from India.

Japanese encephalitis (JE) is an infectious encephalitis prevalent in Asia. It usually presents with fever, headache, convulsions and extrapyramidal symptoms. Limbic system involvement and hypothermia though common in autoimmune encephalitis have never been reported in JE. We report a case of an 18-year-old girl with no previous comorbidities who presented to us with a history of fever and headache for 1 week duration. She developed bilateral lateral rectus palsy and asymmetric flaccid weakness of all four limbs, after 2 days of admission, which was followed by altered sensorium and intermitte

Novel sulfamides and sulfamates derived from amino esters: Synthetic studies and anticonvulsant activity.

We report herein the design and optimization of a novel series of sulfamides and sulfamates derived from amino esters with anticonvulsant pr … ntylenetetrazole. These results verify the ability of the computational model for designing new anticonvulsants structures with anti-MES activity. Additionally, we evaluated the capacity of the synthesized structures to bind to the benzodiazepine binding site (BDZ-bs) of the γ-aminobutiric acid receptor (GABAA receptor). Some of them showed medium to low affi …

Nonconvulsive seizure control in the intensive care unit.

Nonconvulsive seizures (NCS) occur in as many as 20 % of comatose critically ill patients. These seizures need to be treated; however, the urgency with which this must be done and the medications that should be used are unclear. Often, data from treatment of convulsive status epilepticus (SE) is used to determine the best therapy for NCS. This may lead to "overtreatment" with sedating medications that prolongs hospitalization and worsens outcome. Nonsedating antiepileptic drug (AED) use is favored by many neurologists as the side effect profile is superior to sedating medications. Though limit

The safety and efficacy of fosphenytoin for the treatment of status epilepticus.

Fosphenytoin, a water-soluble prodrug of the antiepileptic drug phenytoin, is entirely and rapidly converted to this antiepileptic drug. The mechanism of action of fosphenytoin is related to the blockade of voltage-operated sodium channels. It was developed in order to obtain a phenytoin-like drug with improved water solubility. Its maximal plasma concentration is achieved within 90-190 min following intramuscular administration with bioavailability being complete after intravenous injection. The main indications for fosphenytoin are the treatment of convulsive status epilepticus and the preve

Endoscopic monoportal removal of a choroid plexus papilloma in the posterior third ventricle in a child.

Currently, only a few reports describe the minimally invasive removal of choroid plexus papillomas (CPPs) and, to the best of the authors' knowledge, no reports detail the resection of such a papilloma through an endoscopic approach in infants. The authors here describe the endoscopic removal of a third ventricle CPP in a child. A 5-month-old male infant presented with progressive macrocephaly, vomiting, and convulsions. A lesion in the posterior third ventricle was detected on brain MRI. Because of the patient's very young age, neuroendoscopy was used as the least invasive technique. The tumo

[Clinical course and serial brain MRI findings in a patient with Lissauer form of general paresis].

A 60-year-old man presented with progressive dementia and generalized convulsions. An initial MRI revealed a widespread high-intensity area with a mass effect in the right frontal and temporal lobes on T2-weighted images. Findings on digital subtraction angiography were normal. Serum and CSF tests showed high titers of antibodies to Treponema pallidum, which helped to distinguish neurosyphilis from glioma. He was initially treated with penicillin injection; however, it caused liver dysfunction and penicillin was switched to erythromycin. Even after antibiotic therapy for 2 months, his dementia

Cerebral Vein Thrombosis after Spinal Anaesthesia with Pregnancy.

Thrombosis of cerebral veins or thrombosis of cortical and deep veins that empty into the sinuses can cause serious neurological syndromes. Cerebral vein thrombosis is seen, especially between the ages of 20-35. The predisposing factors in 80% of patients can be determined. Pregnancy, postpartum period and spinal anaesthesia are among the predisposing factors. Diagnosis of the disease is difficult due to the variety of clinical signs and symptoms. Headache is the most common reason for admission to the hospital. Thromboembolic events in pregnancy are an important reason of maternal morbidity a

[Electroclinical characteristics of a patient with ring chromosome 20 syndrome].

INTRODUCTION: The ring chromosome 20 syndrome (r20) is a rare genetic disorder with a late diagnosis. CASE REPORT: A 17 year old boy with drug-resistant epilepsy of 14 years of evolution, which has moderate mental retardation, behavioral alterations and seizures consisting of complex non-convulsive status and generalized seizures during wakefulness, along with more subtle epileptic manifestations during sleep. Karyotype in peripheral blood showed the existence of a ring chromosome 20, whose breakpoints were p13q13.3, presenting a mosaicism 46,XY[23]/46,XY,r(20)(p13q13.3)[25]. CONCLUSIONS: Th

[An elderly patient with large cell neuroendocrine carcinoma(LCNEC)for whom chemotherapy with irinotecan and split-dose cisplatin(CDDP)proved very effective].

Large cell neuroendocrine carcinoma(LCNEC)is a rare malignant disease with a poor and the clinical outcome. Although no standard chemotherapy regimen has been developed, LCNEC should be treated in a manner similar to that used for small cell lung cancer because of their similar chemosensitivity and aggressive characteristics, even though LCNEC is classified as non-small cell lung cancer. A 78-year-old man was admitted with the complaint of convulsions on the left side of his body. A chest computed tomographic(CT)scan on admission demonstrated a 4 cm mass in a left lung S9. Hyperthermia-induced seizures: development of hyperthermia-prone and hyperthermia-resistant rats.

Febrile seizures (FS), as a multifactorial and genetic disease, are the most common type of convulsive event in infants and young children. Their genetic basis, however, remains elusive. To investigate the genetic mechanisms involved in FS and to identify novel susceptibility genes, we developed two new strains of rats that are hyperthermia-prone (HP, lower seizure threshold) and hyperthermia-resistant (HR, higher seizure threshold) using an established model of hyperthermia-induced seizures combined with selective breeding process. With each subsequent generation, the morbidity of the FS grad

Scleromyxedema: a case report and review of the literature.

Scleromyxedema (SM) is a sclerotic variant of lichen or papular mucinosis in which lichenoid papules and scleroderma-like features are both present. It is a rare deposition disorder characterized by generalized papular and sclerodermoid eruptions, mucin deposition, increased fibroblast proliferation, fibrosis, and monoclonal gammopathy (also known as paraproteinemia) mainly of the immunoglobulin G-lambda type in the absence of thyroid disease. It usually affects middle-aged adults and shows no gender or racial predilection. In addition to the skin findings and paraproteinemia, patients with SM

Methylphenidate and continuous spike and wave during sleep in a child with attention deficit hyperactivity disorder.

Attention-deficit/hyperactivity disorder is the most common neurobehavioral disorder in children and frequently associated with epilepsy. For patients with both conditions, methylphenidate remains a mainstay in the treatment of behavioral problems. Most studies demonstrate that methylphenidate is effective in treating children with well-controlled epilepsy, and that methylphenidate does not increase the risk of having seizures in patients with EEG abnormalities without epilepsy. However, in patients with active seizures, the results are somewhat contradictory. This article presents the case of

Protein-losing enteropathy following the Fontan procedure in a child with intestinal lactase deficiency treated with lactose-free diet.

UNLABELLED: A 12-year-old girl presented with a protein-losing enteropathy. Symptoms started 4 weeks after undergoing the Fontan procedure at the age of 1.5 years for mitral atresia, ventricular septal defect, and double-outlet right ventricle. Upon referral for 3 weeks of rehabilitation after multiple interventional measures and drug treatments, she appeared in a dystrophic state, with decreased plasma protein and electrolyte levels along with occasional tetanic convulsions. Blood glucose levels after a lactose tolerance test were markedly reduced. The introduction of a lactose-free diet was
