[Outcomes of modified lateral orbital wall decompression using ultrasonic bone removal].

OBJECTIVE: This study presents a comparative analysis of outcomes of lateral orbital wall decompression performed using ultrasonic bone removal with standard and modified techniques. MATERIAL AND METHODS: The study included 78 patients (109 orbits) with exophthalmos without visual impairment (subgroups 1A and 1B) and with optic neuropathy (ON) due to thyroid eye disease (TED) (subgroups 2A and 2B). Lateral wall decompression (LWD) was performed using ultrasonic bone removal with a modified (n=58, patient subgroups 1A and 2A) or standard (n=51, subgroups 1B and 2B) technique. Postoperative eva

Vertical Lid Split Orbitotomy for Management of Orbital Compartment Syndrome-A Clinical Case Series.

PURPOSE: This study aims to investigate the early use of a vertical lid split orbitotomy for the management of orbital compartment syndrome. METHODS: This was a retrospective study of all patients in the authors' center who had a vertical lid split procedure for orbital compartment syndrome. Intraocular pressure was measured before and after completion of the procedure as a surrogate for orbital pressure. Information regarding the mechanism of injury as well as the ophthalmic exam was reviewed. RESULTS: Ten patients were included, 3 of which were bilateral, for a total of 13 eyes. The averag

Elective Treatment of Large-Bowel Obstruction in Asymptomatic Sigmoid Volvulus.

BACKGROUND: Sigmoid volvulus is an uncommon cause of intestinal obstruction representing the 5% of all Western cases, associated with old age and a history of neurological and psychiatric condition. Generally, its diagnosis is established by clinical and radiologic findings. It often represents an emergency and it is commonly associated with pain, vomit and abdominal tenderness. CASE PRESENTATION: We present a case of a 59 years old man, admitted to our emergency department, showing an abdominal X-Ray reporting a distention of large bowel, which was required due to presence of multiple diarrhe

Coronary Renal Shunt with Splenectomy (CRSS) for Selective Variceal Decompression.

BACKGROUND: Distal splenorenal shunt and coronary caval shunt are commonly used for selective decompression of esophagogastric varices, but they may not solve severe hypersplenism and their application may be hampered by the presence of splenic venous thrombosis or a left gastric vein (LGV) situated deeply behind the pancreas. On the other hand, some patients have an LGV entering the splenic vein (SV). We tried to work out a new selective shunt for this group of patients. METHODS: Sixteen patients with severe hypersplenism and esophagogastric varices received coronary renal shunt using the SV

The Influence of Antiplatelet Drug Medication on Spine Surgery.

BACKGROUND: The incidence of cardiovascular and neurovascular diseases has been increasing with the aging of the population, and antiplatelet drugs (APDs) are more frequently used than in the past. With the average age of spinal surgery patients also increasing, there has been a great concern on the adverse effects of APD on spine surgery. To our knowledge, though there have been many studies on this issue, their results are conflicting. In this study, we aimed to determine the influence of APDs on spine surgery in terms of intraoperative bleeding and postoperative spinal epidural hematoma com

Technique of supraclavicular decompression for neurogenic thoracic outlet syndrome.

The supraclavicular approach to scalenectomy and first rib resection has been modified since the original description in 1985. The incision is 1 to 2 cm above the clavicle, 1 cm lateral to the midline, and 5 to 7 cm long. Subplatysmal skin flaps are created. The sternocleidomastoid muscle is mobilized on its lateral edge and retracted but not divided. The scalene fat pad is split vertically, the omohyoid muscle excised, and the C5 nerve root dissected free. The accessory phrenic nerve is identified, if present, arising medially from C5, and preserved. The rest of the plexus is dissected free,

Open carpal tunnel release with use of a nasal turbinate speculum.

Incomplete release of the transverse carpal ligament (TCL) and median nerve injury are complications of carpal tunnel release (CTR). In this article, we describe a modified mini-open release using a fine nasal turbinate speculum to aid in the proximal release with direct visualization of the proximal limb of the TCL and the distal volar forearm fascia (DVFF). The technique begins with a 2.5-cm palmar longitudinal incision, and standard distal release of the TCL is completed. A fine nasal turbinate speculum is inserted into the plane above the proximal limb of the TCL and the DVFF. A long-handl

Subcutaneous anterior transposition versus decompression and medial epicondylectomy for the treatment of cubital tunnel syndrome.

A review of the literature often fails to uncover the best procedure for the treatment of cubital tunnel syndrome. This article compares 2 frequently used methods (subcutaneous anterior transposition vs decompression and medial epicondylectomy) for their effectiveness in relieving both subjective and objective symptoms of cubital tunnel syndrome. Between August 1991 and October 1993, nineteen patients underwent surgical decompression by a single surgeon for ulnar neuropathy at the elbow. Factors evaluated included upper extremity range of motion, elbow valgus stress, grip strength, pinch, 2-po

Incisionless Hartmann's procedure: an innovative minimal access technique for surgical treatment of sigmoid volvulus in debilitated patients with faecal incontinence.

Sigmoid volvulus requires decompression and subsequent surgical correction, and is often seen in debilitated patients. In an effort to decrease the physiological burden of surgery in these high-risk patients, we report an innovative minimal access technique for the definitive treatment of decompressed sigmoid volvulus in patients with concomitant faecal incontinence. A retrospective chart review of a series of two consecutive patients who had undergone a minimal access Hartmann's procedure (HP) between November 2005 and October 2006 was performed. A single skin incision of < or = 4 cm at the p

Isolated caruncular approach for orbital decompression.

PURPOSE: To evaluate the effectiveness of the isolated caruncular approach to orbital decompression of thyroid ophthalmopathy. METHODS: In a retrospective, noncomparative, interventional case series, we reviewed the records of 29 patients (48 orbits) who had thyroid ophthalmopathy and had undergone orbital decompression using the caruncular approach. The medial wall was decompressed in two patients (three orbits), and the medial and inferior walls were decompressed in 27 patients (45 orbits). RESULTS: The mean retrodisplacement achieved was 2.7 mm of decompression of the medial wall, and 4.2

Teflon-induced granuloma following treatment of trigeminal neuralgia by microvascular decompression. Report of two cases.

The authors report two cases of Teflon-induced granuloma occurring as a result of microvascular decompression using Teflon wool for the treatment of trigeminal neuralgia (TN). Teflon, which is used to separate a compressing vessel from the root entry zone (REZ) of the trigeminal nerve at the brainstem, is presumed to be an inert material. In the two cases reported here, however, Teflon induced a foreign body reaction at the REZ, causing recurrence of TN. The patients' pain was cured by complete decompression or partial sensory rhizotomy of the trigeminal sensory root at reoperation. Teflon-ind

The rotator cuff. Large and massive tears. Technique of open repair.

Symptomatic large and massive rotator cuff tears can be successfully managed with operative repair. Five factors that facilitate favorable results include: (1) performing an adequate subacromial decompression; (2) maintaining the integrity of the deltoid origin; (3) mobilizing the torn tendons, performing an interval slide when indicated; (4) repairing the tendons to bone; and (5) carefully staging and supervising the rehabilitation program.

Changes in proptosis and eyelid position following orbital fat decompression for thyroid eye disease.

BACKGROUND: Proptosis and eyelid retraction in thyroid eye disease (TED) affect both cosmetic appearance and psychological wellbeing. This study aimed to evaluate the outcomes of orbital fat decompression in patients presenting with proptosis and eyelid retraction. METHODS: We retrospectively reviewed 40 patients (79 eyes) with non-active TED who exhibited both proptosis and eyelid retraction. All patients underwent orbital fat decompression through a lower transconjunctival incision with release of the lower eyelid retractor to access the intraorbital space. Orbital fat was removed from the m

Are there any potential risk factors for infection following an incidental durotomy during spine surgery?

Incidental durotomy is a complication of spinal surgery that carries a risk of persistent CSF leak. One major concern with persistent CSF leakage is the increased risk of infection. Fifty-six consecutive patients who underwent spine surgery complicated by incidental durotomy between 2017 and 2022. We compared 28 patients who developed a postoperative infection (IG) and 28 patients who did not (CG). The median age was 40.6 years in the IG and 57.7 years in the CG (p = 0.04). The index procedures were decompression for stenosis or discectomy for herniated disc; 18 patients had associated instrum

Smoking is a significant contributor to intraoperative blood loss in metastatic spinal tumor surgery: a propensity score analysis.

BACKGROUND: Metastatic spinal tumors pose a significant challenge regarding intraoperative blood loss. Identifying risk factors for intraoperative blood loss is crucial for appropriate surgical planning and early intervention. However, current studies have not comprehensively evaluated risk factors for predicting intraoperative blood loss. This study aims to determine whether smoking significantly contributes to intraoperative blood loss among metastatic spinal tumors and to investigate other potential risk factors. METHODS: This study analyzed 252 patients with metastatic spinal disease who
