Minimally invasive ankle arthrodesis with a retrograde locking nail after failed fusion.

A retrograde nail with posterior-to-anterior (PA) locking into os calcis, talus and tibia was used to correct deformity and achieve fusion after failed fusion. A variety of methods have been published to achieve union of the ankle and subtalar joint in a failed fusion situation. We have studied a retrograde locking nail technique through a 2.5-cm incision in the non-weightbearing part of the sole of the foot. Remaining cartilage in the ankle joint, where necessary, was percutaneously removed through an anterior approach and the locking nail was inserted after reaming of os calcis, talus and ti

A new Mel-CAM (CD146)-specific monoclonal antibody, MN-4, on paraffin-embedded tissue.

Mel-CAM (also termed CD146 or MUC18) is a cell-adhesion molecule belonging to the immunoglobulin gene superfamily. It mediates cell-cell interaction through heterophilic Mel-CAM/ligand adhesion. Previous studies showed Mel-CAM immunoreactivity in normal and neoplastic human tissues, but an extensive assessment of Mel-CAM distribution was not performed because of the lack of a Mel-CAM-specific monoclonal antibody that could be used in routinely processed, formalin-fixed, paraffin-embedded tissues. Therefore, we developed a mouse monoclonal antibody, MN-4, that specifically recognized a fixation

Localized retinal morphology and differential light sensitivity in diabetic retinopathy. Methodology and clinical results.

The thesis presents a technique designed to allow a comparison of retinal function as assessed by computerized perimetry with retinal morphology as seen on photographs of the ocular fundus, including results from the practical application of this technique in the study of diabetic retinopathy. The basis of the technique is an optical algorithm that allows angular distances in the visual field to be transformed to match linear distances on fundus photographs and fluorescein angiograms. The visual field data is superimposed onto the corresponding retinal morphology on the photograph on the basis

Two-stage replacement of total and subtotal foot bone defects in Charcot neuroosteoarthropathy using personalised allogenic 3D bone bioimplant.

INTRODUCTION: Charcot diabetic arthropathy is one of the most serious complications in patients with diabetes mellitus with sensorimotor neuropathy of the lower extremities, accompanied by metabolic disturbances and subsequent destruction of bones and joints. There are many methods of surgical treatment of diabetic charcot's foot described in the literature. A review of the literature found no publications describing the surgical treatment of Charcot arthropathy patients using the method of two-stage replacement of total and subtotal foot bone defects with heterotopic allografts using 3D model

What Are the Tradeoffs in Outcomes after Casting Versus Surgery for Closed Extraarticular Distal Radius Fractures in Older Patients? A Statistical Learning Model.

BACKGROUND: Distal radius fractures (DRFs) are one of the most common major fractures. Despite their frequency, the tradeoffs in different outcomes after casting or surgery for closed extraarticular DRFs in older adults are unknown. QUESTIONS/PURPOSES: (1) For adults older than 60 years with closed extraarticular DRFs, what are the tradeoffs in outcomes for choosing casting versus surgery? (2) In what settings would surgery be preferred over casting? METHOD: This is a secondary analysis of data from the Wrist and Radius Injury Surgical Trial (WRIST), a randomized, multicenter clinical trial

Use of platelet-rich fibrin in bladder exstrophy repair: A prospective study.

BACKGROUND: Penopubic fistula formation remains one of the most frequent early complications following bladder exstrophy repair. Platelet-rich fibrin (PRF), an autologous, high-concentration platelet, white blood cells, and fibrin biomaterial that enhances tissue healing and regeneration, has shown potential for surgical applications. However, PRF has not been applied in primary bladder exstrophy repair. This study aimed to assess the effectiveness of autologous PRF membrane applications in primary bladder exstrophy repair in male patients. MATERIAL AND METHODS: This prospective randomized co

Surgical Correction of Thoracolumbar Kyphosis in Achondroplasia: Complications, Pitfalls, and Reflections on the Pursuit of Maximal Realignment in View of Correction Leading to Functional Disability.

Background: Achondroplasia, the most common genetic dwarfism caused by the FGFR3 mutation (autosomal dominant, 80% de novo), results in a disproportionately short stature. Thoracolumbar kyphosis (TLK), combined with characteristic spinal canal stenosis, increases the risk of symptomatic compression, yet the literature lacks clear thresholds for symptom onset or progressive deformity angles. Methods: A 16-year-old female with achondroplasia presented with rapidly progressive kyphosis despite conservative management (bracing and therapy). Over six months, she developed neurogenic claudication; b

Aesthetic lower limb lengthening techniques: a systematic review of efficacy, complications, and patient satisfaction.

BACKGROUND: Aesthetic limb lengthening (ALL) is a cosmetic procedure aimed at enhancing body symmetry and improving self-esteem through the gradual elongation of bones. Unlike functional limb lengthening, which addresses limb length discrepancies, ALL focuses on increasing limb length for aesthetic purposes. While functional limb lengthening has been extensively studied, a comprehensive analysis of ALL outcomes is needed. This systematic review evaluates the efficacy, complications, and patient satisfaction associated with different ALL techniques, including distraction osteogenesis and variou

Evaluation of Foot Osteotomies for Treating Residual Clubfoot Deformities in Ambulatory Children With Arthrogryposis.

INTRODUCTION: Clubfoot is the most common deformity seen in children with arthrogryposis. The primary method of treatment for this deformity is serial casting, which has a greater frequency of incomplete correction and recurrence than in idiopathic clubfoot. For both primary and recurring cases, surgical treatments vary from soft-tissue releases to talectomy. This research aimed to evaluate the effectiveness of bony surgical procedures in correcting clubfoot in ambulatory children with arthrogryposis. METHODS: The study retrospectively examined ambulatory children with arthrogryposis, all of

Outcomes in Treatment of Ankle and Pilon Fractures with Retrograde Tibiotalocalcaneal Nailing Without Articular Preparation in the Setting of Diabetes Mellitus.

BACKGROUND: Treatment of ankle and pilon fractures in the setting of diabetes mellitus (DM) is challenging due to a propensity for postoperative complications. Limb salvage is a primary concern following these injuries, as below knee amputation (BKA) occurs at an unacceptably high rate. Primary retrograde tibiotalocalcaneal (TTC) joint nailing without articular preparation has emerged as a solution to treat diabetics with ankle and pilon fractures to mitigate surgical complications and prevent BKA. The technique minimizes surgical dissection and has previously demonstrated utility in fragility
