[Atlantoaxial reduction and fixation guided by the intraoperative CT].

OBJECTIVE: To evaluate the clinical result of atlantoaxial reduction and fixation guided by the intraoperative CT. METHODS: Sixteen cases were retrospectively studied, including seven males and nine females, with the mean age of 49.9 years. Twelve cases were diagnosed as chronic atlantoaxial instability or dislocation, while four cases as acute odontoid fracture and dislocation. Among the sixteen cases, fourteen underwent atlantoaxial fusion, while two underwent temporary atlantoaxial fixation without fusion. The intraoperative CT was used in the setting of: 1. Evaluating the atlantoaxial red

The outcomes of salvage surgery for vascular injury in the extremities: a special consideration for delayed revascularization.

A seven years retrospective study was performed in 45 consecutive vascular injuries in the extremities to investigate the pattern of injuries, managements and outcomes. Motor-vehicle accidents were the leading cause of injuries (80%), followed by industrial injuries (11.1%) and iatrogenic injuries (4.4%). Popliteal and brachial artery injuries were commonly involved (20%). Fifteen (33.3%) patients had fractures, dislocation or fracture dislocation around the knee joint and 6 (13.3%) patients had soft tissue injuries without fracture. Traumatic arterial transection accounted for 34 (75.6%) case…

[Analysis of mechanisms and treatment of hyponatremia in acute spinal cord injuries].

OBJECTIVE: To investigate etiologic factors, mechanisms and treatment of hyponatremia in patients with acute spinal cord injury. METHODS: From January 2005 to July 2010, 57 patients with hyponatremia after acute spinal cord injuries from severe trauma were treated. They included 46 males and 11 females who ranged in age from 26 to 69 years (mean 39.5 years). Of the 57 cases, 55 cases were complicated by cervical dislocation or fracture, and the remaining two cases were without dislocation or fracture. Among them, 28 patients had complete spinal cord injury, 29 had incomplete spinal cord injury

Modified arthroscopic McLaughlin procedure for treatment of posterior instability of the shoulder with an associated reverse Hill-Sachs lesion.

Traumatic posterior shoulder dislocations are often accompanied by an impression fracture on the anterior surface of the humeral head known as a "reverse Hill-Sachs lesion". This bony defect can engage on the posterior glenoid rim and subsequently lead to recurrent instability and progressive joint destruction. We describe a new modified arthroscopic McLaughlin procedure, which allows for filling of the bony defect with the subscapularis tendon and subsequently prevents recurrence of posterior instability. This technique creates a double-mattress suture providing a large footprint for the subs

Ultrasound of the shoulder joint: non "rotator cuff" lesions.

Most of the papers in the literature are on rotator cuff lesions. This paper will deal with a somewhat forgotten part of the shoulder, the joint and the nontendinous structures around the joint. Since 1985, we have performed ultrasound examinations of the shoulder. We have collected many cases and some of them will be performed. The shoulder joint can be partly examined by ultrasound. We can get some information about joint effusions, humeral head cartilage, some part of the labrum, loose bodies, ganglions and fractures around the joint. The acromio-clavicular joint is also part of the examina

Shoulder and humerus trauma.

Three bones and two joints comprise the pectoral girdle. These are the clavicle, scapula, proximal humerus, acromioclavicular joint, and shoulder joint. The pectoral girdle binds the upper extremity to the torso. Thus, injury to any of the pectoral girdle's components can impact the function of the entire extremity. The spectrum of pectoral girdle injuries ranges from a simple acromioclavicular joint sprain to scapulothoracic dissociation. Whereas the former is a painful but self-limiting injury with minimal sequelae, the latter is life threatening. However, the severity of most pectoral girdl

Injuries to the ulnar carpometacarpal region: are they being underdiagnosed?

Thirteen cases with radiographic evidence of injury to the ulnar side of the hand are reviewed. These injuries included intraarticular fractures and/or dislocations of the fourth and fifth carpometacarpal joints and associated bones collectively discussed here as the ulnar carpometacarpal region. In nine the extent of the injury was not appreciated on first presentation. Injuries to this region are often difficult to assess clinically and radiographically even in experienced hands. In our experience computerized tomography (CT) scans in the longitudinal and longitudinal oblique axis best displ

[Epidemiology, treatment and long-term results of spinal fractures at a non-university affiliated central hospital with special reference to fractures of the lumbar spine].

In a retrospective study epidemiology, treatment, clinical and radiological results of 283 patients with spine fractures in a five-year period are presented. The operation rate ranges from 42% of the cervical to 9% of the thoracical and 24% of the lumbar spine. We found good radiological results concerning the correction of the wedge compression and the collapse of the lumbar vertebral body by fixation with an internal fixator. After a follow-up of 2-5 years nearly 80% of conservative as well as operative treated patients had more or less residual back pain. Because there is a correlation betw

Weight-training injuries. Common injuries and preventative methods.

The use of weights is an increasingly popular conditioning technique, competitive sport and recreational activity among children, adolescents and young adults. Weight-training can cause significant musculoskeletal injuries such as fractures, dislocations, spondylolysis, spondylolisthesis, intervertebral disk herniation, and meniscal injuries of the knee. Although injuries can occur during the use of weight machines, most apparently happen during the aggressive use of free weights. Prepubescent and older athletes who are well trained and supervised appear to have low injury rates in strength tr

Neurologic recovery following injuries to the cervical spinal cord and nerve roots.

From this literature, we have made the following conclusions: (1) Complete cord lesions do not recover cord functional motor control. (2) Complete lesions recover one nerve root level of function at the fracture site. (3) Partial lesions recover partially. (4) The less the injury, the greater the recovery. (5) Brown-Sequard lesions recover more than central cord syndromes, which recover more than anterior cord syndromes. (6) Reduction of dislocated facets facilitates nerve root recovery. (7) Better documentation of specific pathology and recovery rates are necessary to determine the surgical b
