[Treatment of distal radius fracture by combination of external fixator and volar approach fixation].

OBJECTIVE: To investigate the operative indication and clinical efficacy of combination of external fixator and volar approach fixation in treating distal radius fracture. METHODS: From March 2000 to March 2005, 28 patients with distal radius fracture were treated with combination of external fixator and volar approach fixation. Dorsal external fixator was used to maintain wrist in functional position, combinated volar plate or Kirschner wire fixation after reduction was achieved. Bone graft was necessary if there were severe comminuted cortical bone or compress of cancellous bone. Of 28 pati

[Angiography of the carotid artery in centro-lateral skull base fractures?].

BACKGROUND: In latero-basal, central or spheno-fronto-orbital skull base fractures the internal carotid artery is more frequently involved in severe lesions than expected. PATIENTS AND METHODS: Between 1996 and 2003 we examined 684 patients with Glasgow Coma Scales (GCS) between 2 and 15, median 7.2, using computed tomography (CT). In suspicion of a latero-basal, central or spheno-fronto-orbital fractur they got an additional high resolution skull base CT. If the bony canal of the internal carotid artery (ICA) was involved the patient underwent digital subtraction angiography (DSA). The ICA l

In-hospital mortality after femoral neck fracture: do internal fixation and hemiarthroplasty differ?

In this article, we examine rates of in-hospital mortality of elderly patients with femoral neck fracture treated with internal fixation or hemiarthroplasty. Data were analyzed for 51,003 patients (> or = 65 years old) admitted with femoral neck fractures to New York state hospitals between 1985 and 1996. The primary outcome examined was in-hospital mortality. Associations between type of surgical procedure and outcome were assessed using a multiple logistic regression model, adjusting for patient age, sex, race, number of comorbidities, and residence in a nursing facility before hip fracture.

[Growth behavior after epiphyseal plate injury: importance of "watertight" osteosynthesis].

BACKGROUND: The frequency figures for epiphyseal plate injuries of long bones given in the literature are inexact and they probably occur with a frequency of 15% of all fractures of the growing skeleton. In order to be able to give correct figures in the future a classification system, such as the LiLa classification should be used, which does not attempt to be oriented to an assumed growth prognosis but is oriented to therapy and makes a strict differentiation between shaft and joint fractures. For epiphyseal joint fractures a differentiation must be made between those where the epiphysis is

Characterization of granulocyte colony-stimulating factor receptor expressed on human lymphocytes.

Maxillofacial injuries in general occur quite commonly following trauma and these injuries if not properly managed can negatively influence both the psychosocial and functional activities of the patient. This is as a result of the centrality of the facial region as a key factor in human identity, aesthetics, and general well-being. Fractures involving the facial skeleton may be isolated or complex. High velocity trauma is usually seen in urban and semi-urban areas while low velocity trauma is the common setting in rural areas. The pattern of faciomaxillary fractures vary with geographical area

Simultaneous head and facial computed tomography scans for assessing facial fractures in patients with traumatic brain injury.

INTRODUCTION: Patients with traumatic brain injury (TBI) may have concomitant facial fractures. While most head injury patients receive head computed tomography (CT) scans for initial evaluation, the objective of our study was to investigate the value of simultaneous facial CT scans in assessing facial fractures in patients with TBI. METHODS: From January 1, 2015 to December 31, 2015, 1649 consecutive patients presenting to our emergency department (ED) with a TBI who received CT scans using the protocol for head and facial bones were enrolled. The clinical data and CT images were reviewed vi

Effect of initial emergency room imaging choice on time to hip reduction and repeat imaging.

OBJECTIVES: Hip dislocations are highly morbid injuries necessitating prompt reduction and post-reduction assessment for fracture and incarcerated fragments. Recent literature has questioned the need for initial pelvic radiographs for acute trauma patients, resulting in computed tomography (CT) scans as the initial evaluation. This study investigates the relationship between choice of pre-reduction imaging and treatment of acute hip dislocations. DESIGN: Retrospective Case-Control. SETTING: Single Academic Level I Trauma Center. METHODS: All acute hip dislocations from 2011 to 2016 were rev

Closed Reduction, Osteotomy, and Fibular Graft Are Effective in Treating Pediatric Femoral Neck Pseudarthrosis After Infection.

BACKGROUND: When treatment of acute septic arthritis of the hip is delayed, severe sequelae can occur. This may take the form of coxa breva, vara, or valga. Pseudoarthrosis of the femoral neck is a rare complication with only a few studies to guide treatment. QUESTION/PURPOSES: In a small series of patients with pseudarthrosis of the femoral neck after acute septic arthritis, we wanted to determine (1) whether femoral neck union can be achieved using a variety of surgical approaches; (2) whether these patients satisfied criteria outlined by Hunka and Choi, defined as a stable hip, a flexion a

Guillain-Barré syndrome following severe head trauma and spine surgery.

Guillain-Barré syndrome (GBS) is an acute-onset inflammatory polyradiculoneuropathy usually triggered by an infectious disease. In some cases, GBS can occur without any preceding infectious episode, like after vaccination, epidural anaesthesia or surgery. A 73 years old woman had head and spine trauma. Body-TDM showed bilateral temporal and right frontal haematomas and fracture of the first lumbar vertebrae. Sextant and kyphoplasty were performed. She presented 14 days after surgery tetraparesis, swallowing difficulties and bilateral facial palsy. Electromyography was consistent with demyelina

Echocardiogram in sternal fracture.

We reviewed the records of 50 consecutive patients presenting with sternal fracture after blunt chest trauma. The relationships between electrocardiogram, creatine kinase MB isoenzyme and echocardiogram (ECHO) were assessed in reference to myocardial contusion. Eleven patients (22%) had 1 or more abnormal cardiac tests but only 3 (6%) had clinically symptomatic myocardial contusion. Seven (22.5%) out of 31 ECHO studies revealed pericardial effusion which was not associated with the severity of all other associated injuries. The incidence of an abnormal ECHO study in patients with isolated ster

Unilateral medial dislocation of the temporomandibular joint.

We present our experience of the rare condition of unilateral medial dislocation of the temporomandibular joint (TMJ) in 11 patients with head trauma who had received a direct lateral blow on the chin. The diagnosis was made by direct coronal CT of the TMJ performed from 6 h to 7 days following the injury. In 6 patients, subcondylar fracture of the ipsilateral mandibular ramus was also demonstrated. A second CT performed 11-16 months following the first one demonstrated pseudoarthrosis of the fractured ramus in these 6 patients. The second CT was identical to the first in the remaining 5 patie

Trampolines in New Zealand: a decade of injuries.

Despite international concern about the safety of trampolines, they have become increasingly popular in New Zealand. While internationally attention has centred on a relatively few cases of catastrophic cervical spine injury, little research effort has been directed at placing these incidents in a wider context. To redress this, a descriptive epidemiological study of trampoline-related injury in New Zealand was undertaken. National hospitalization and mortality data for a 10-year period revealed 2098 hospitalizations and two deaths. The incidence rate for hospitalizations increased from 3.1 pe

Blinded comparison of cranial CT and MR in closed head injury evaluation.

PURPOSE: To compare CT and MR in the evaluation of acute head injury. METHODS: One hundred seven consecutive patients who were referred to the emergency department and underwent both MR and CT cranial examinations within 48 hours were retrospectively reviewed. The films were interpreted by two neuroradiologists blinded to all patient information. RESULTS: The sensitivity of MR was significantly higher than that of CT for the detection of contusion, shearing injury, subdural and epidural hematoma, and sinus involvement. The sensitivity of CT was significantly higher than that of MR for the ev

Neuro-ophthalmological findings in closed head trauma.

Visual abnormalities following closed head trauma are common and can affect any part of the visual pathway. We reviewed 181 consecutive patients referred with visual complaints following closed head trauma. A motor vehicle accident was the most common etiology of trauma in 57% of cases, direct trauma to the head in 15%, and injuries sustained from a fall in 13%. Sixty-three percent of patients lost consciousness and 26% suffered a skull fracture. Thirty-five percent of all patients had visual field defects with functional (tunnel) fields the most common. Over 88% of eyes had 20/20 or better vi

Similarity Simulation on the Movement Characteristics of Surrounding Rock and Floor Stress Distribution for Large-Dip Coal Seam.

The question of how to mine safely in close multi coal seams is the main concern for coal operators, in particular for large-dip coal seams with complex geological and mechanical conditions. This paper presents a detailed similarity simulation on the movement characteristics of the overburden and the stress distribution of underlying strata in terms of a specific coal mine in the Tielieke mining area of the Kubai coalfield via a three-dimensional photogrammetry system and a high-speed static resistance analyzer. The results show that the overburden strata are asymmetrically deformed around the

Are there gap junctions between chief (glomus, type I) cells in the carotid body chemoreceptor? A review.

Since the dye- and electronic couplings between the carotid body chief cells have been demonstrated, the detection and localization of the gap junctions in the carotid body is crucial to understanding the functional mechanism of chemoreception. However, conventional electron microscopy has been unsuccessful in unquestionably detecting ultrastructural features equivalent to the gap junctions, such as close (2 nm in width) membrane appositions in ultrathin sections and aggregations of intramembranous particles in freeze-fracture replicas of the carotid body. We previously reported using a modifi

Deformation characteristics of weakly cemented overburden in Western mining areas in China.

Understanding the deformation behavior of weakly cemented overburden in western coalfields is critical for assessing mining-induced instability and groundwater hazards. Taking the 23 - 2 coal seam of Yili No. 4 Mine as the research object, this study integrates FLAC3D numerical simulations with high-precision transient electromagnetic field monitoring data to quantify the influence of mining thickness and face advance on overburden responses. The actual mining thickness of the target coal seam is 9.5 ~ 10 m, and the mining depth ranges from 150 m to 1000 m. Results reveal a characteristic "pro

Experimental and numerical failure analysis of Thompson hip prosthesis.

BackgroundThere are different surgical procedures to fix femoral head-neck fractures, such as total hip arthroplasty and hemiarthroplasty. Under optimal conditions, the inserted prosthesis should last a long time, possibly a lifetime, with only trace amounts of wear each year due to the friction between the femur bone and the prosthesis stem. However, in some cases, failure due to excessive wear, creep, fatigue, corrosion, etc., can occur.ObjectiveThis study investigates the failure causes of a Thompson hip prosthesis both experimentally and numerically.MethodsThe stem of the prosthesis, which
