Mechanisms of orthostatic hypotension and supine hypertension in Parkinson disease.

Non-motor aspects of Parkinson disease (PD) are now recognized to be important both clinically and scientifically. Among these facets are abnormalities in blood pressure regulation. As much as 40% of PD patients have orthostatic hypotension (OH), which is usually associated with supine hypertension (SH). Symptoms of OH range from light-headedness to falls with serious trauma. SH, while typically asymptomatic, poses a significant increased risk for cardiovascular morbidity and mortality. Neuroimaging, neurochemical, and neuropharmacological studies indicate cardiac and extra-cardiac sympathetic

Affective neural circuitry and mind-body influences in asthma.

Individuals with asthma have twice the risk of developing mood and anxiety disorders as individuals without asthma and these psychological factors are associated with worse outcomes and greater need for medical intervention. Similarly, asthma symptom onset and exacerbation often occur during times of increased psychological stress. Remission from depression, on the other hand, is associated with improvement in asthma symptoms and decreased usage of asthma medication. Yet research aimed at understanding the biological underpinnings of asthma has focused almost exclusively on the periphery. An e

Age and culture modulate object processing and object-scene binding in the ventral visual area.

Behavioral differences in the visual processing of objects and backgrounds as a function of cultural group are well documented. Recent neuroimaging evidence also points to cultural differences in neural activation patterns. Compared with East Asians, Westerners' visual processing is more object focused, and they activate neural structures that reflect this bias for objects. In a recent adaptation study, East Asian older adults showed an absence of an object-processing area but normal adaptation for background areas. In the present study, 75 young and old adults (half East Asian and half Wester

Complete recovery of visual functions after multimodality treatment for intrinsic chiasmatic-hypothalamic astrocytoma--case report.

A 21-year-old female was admitted with complaints of severe impairment of vision. The visual acuity was 0.02 in both eyes along with residual visual fields and central scotomas. Neuroimaging disclosed a chiasmatic-hypothalamic glioma. Surgery was performed with partial removal of the intrachiasmatic part of the intrinsic tumor. The histological diagnosis was fibrillary astrocytoma. Progressive recovery of vision began in the first week after surgery. Adjuvant treatment included one course of fractionated radiation therapy and six courses of chemotherapy. Complete recovery of visual acuity occu

Clinical and neuroimaging study of central nervous system in congenital myotonic dystrophy.

We present the clinical and neuroimaging findings of five patients (four males, one female; mean age 12 years) affected by congenital myotonic dystrophy and the correlation with their molecular genetic analysis. At birth all five presented severe muscular weakness and hypotonia, associated with feeding difficulties and respiratory distress. In the same patients, congenital clubfoot or more generalized arthrogryposis was also evident. Lymphocyte DNA was characterized in each by a CTG repeat longer than 1300 in the region of the myotonic dystrophy gene in chromosome 19. The patients' neurologica

Developmental dyslexia and animal studies: at the interface between cognition and neurology.

Recent findings in autopsy studies, neuroimaging, and neurophysiology indicate that dyslexia is accompanied by fundamental changes in brain anatomy and physiology, involving several anatomical and physiological stages in the processing stream, which can be attributed to anomalous prenatal and immediately postnatal brain development. Epidemiological evidence in dyslexic families led to the discovery of animal models with immune disease, comparable anatomical changes and learning disorders, which have added needed detail about mechanisms of injury and plasticity to indicate that substantial chan

Etiology of eyelid retraction in children: a retrospective study.

Eyelid retraction in an adult occurs usually secondary to Graves' ophthalmopathy, but in children the diagnosis is often obscure. To our knowledge, there are no reports reviewing the relative frequency of the various etiologies of eyelid retraction in children. We reviewed the records of our Pediatric Service from 1976 to 1991 and identified 16 children with eyelid retraction. The eyelid retractions were unilateral in 11 cases and bilateral in five cases, and were the result of five different identifiable causes. Unilateral eyelid retraction was associated with congenital aberrant innervation

Thyroid eye disease presenting after cataract surgery.

Strabismus presenting after cataract surgery has been attributed to a variety of disorders, some of which are unrelated to the surgery. When thorough ophthalmologic and neurologic examinations fail to elucidate a definitive etiology, the motility disturbance is often ascribed to operative trauma to the orbital soft tissues. In a series of 58 patients with strabismus presenting after cataract surgery, eight (14%) were found to have previously unsuspected thyroid eye disease. Three of the eight patients had past histories of systemic dysthyroidism, but none reported diplopia prior to cataract ex

Glymphatic dysfunction as a potential driver of cerebral iron deposition in Parkinson's disease.

Cerebral iron deposition is a pathological hallmark of Parkinson's disease. Notably, Parkinson's disease patients exhibit the characteristic neuroimaging features of enlarged choroid plexus volume (CPV) and diminished glymphatic function. While previous research has focused on potential iron influx dysregulation in Parkinson's disease, the critical question of whether impaired iron clearance stemming from choroid plexus and glymphatic system dysfunction constitutes a key mechanism underlying pathological iron accumulation remains unexplored. Therefore, the aim of this study was to investigate

Molecular subtypes of the Alzheimer's disease spectrum: Multimodal biomarker integration, mechanistic validation, and adaptive clinical translation.

Alzheimer's disease exhibits considerable heterogeneity in its clinical progression, neuropathological features, and underlying etiological mechanisms. However, current clinical diagnosis and treatment primarily rely on positron emission tomography and evidence-based cerebrospinal fluid biomarkers, with less emphasis on molecular subtypes, thereby limiting meaningful subtype stratification and personalized therapeutic interventions. Given advances in large-scale multi-omics technologies, single-cell genomics, and molecular imaging, research on the molecular subtypes of Alzheimer's disease is g

Role of Serotonin in the Neurobiology of Schizophrenia and Association With Negative Symptoms.

IMPORTANCE: The involvement of the serotonin system in the pathophysiology of schizophrenia has been proposed for over 60 years, but there has been no prior study to test if there is altered serotonin release in vivo in schizophrenia (to the authors' knowledge). OBJECTIVE: To investigate serotonin release in vivo in schizophrenia and its association with negative symptoms. It was hypothesized a priori that frontal cortex serotonin release capacity would be lower in schizophrenia compared with healthy controls and that this would be associated with more severe baseline negative symptoms. Effects of amyloid pathology and neurodegeneration on cognitive change in cognitively normal adults.

Understanding short-term cognitive decline in relation to Alzheimer's neuroimaging biomarkers in early stages of the development of neuropathology and neurodegeneration will inform participant recruitment and monitoring strategies in clinical trials aimed at prevention of cognitive impairment and dementia. We assessed associations among neuroimaging measures of cerebral amyloid pathology, a hallmark Alzheimer's neuropathology, hippocampal atrophy, and prospective cognition among 171 cognitively normal Baltimore Longitudinal Study of Aging participants (baseline age 56-95 years, 48% female, 562

Outcomes of unrelated umbilical cord blood transplantation for X-linked adrenoleukodystrophy.

Adrenoleukodystrophy (ALD) is an X-linked disorder caused by a defect in the metabolism of long chain fatty acids leading to demyelination, neurodegeneration, and death. The disease typically presents in young boys and adolescent boys. Allogeneic bone marrow transplantation has been used to halt progression of the disease. However, many patients lack suitable HLA- matched related donors and must rely on unmatched donors for a source of stem cells. The purpose of this study was to evaluate outcomes of unrelated donor umbilical cord blood transplantation after chemotherapy-based myeloablative co

Role of CHA2DS2-VASc score for severity and outcome prediction in acute ischemic stroke: a cross-sectional study from tertiary care center of Nepal.

INTRODUCTION: Acute ischemic stroke (AIS) is a major cause of hospital admission and is responsible for a large proportion of disabilities. The CHA2DS2-VASc score was developed to improve the accuracy of stroke risk stratification in patients with non-valvular atrial fibrillation. It includes major risk factors for ischemic stroke and may be useful in prognostic risk stratification before and after the event of acute ischemic stroke. This study was conducted to study the role of CHA2DS2-VASc score in severity and outcome prediction of acute ischemic stroke. METHODS: A cross-sectional study wa

Understanding disease progression and improving Alzheimer's disease clinical trials: Recent highlights from the Alzheimer's Disease Neuroimaging Initiative.

INTRODUCTION: The overall goal of the Alzheimer's Disease Neuroimaging Initiative (ADNI) is to validate biomarkers for Alzheimer's disease (AD) clinical trials. ADNI is a multisite, longitudinal, observational study that has collected many biomarkers since 2004. Recent publications highlight the multifactorial nature of late-onset AD. We discuss selected topics that provide insights into AD progression and outline how this knowledge may improve clinical trials. METHODS: We used standard methods to identify nearly 600 publications using ADNI data from 2016 and 2017 (listed in Supplementary Mat

Laterality effects in functional connectivity of the angular gyrus during rest and episodic retrieval.

INTRODUCTION: The angular gyrus (AG) is consistently reported in neuroimaging studies of episodic memory retrieval and is a fundamental node within the default mode network (DMN). Its specific contribution to episodic memory is debated, with some suggesting it is important for the subjective experience of episodic recollection, rather than retrieval of objective episodic details. Across studies of episodic retrieval, the left AG is recruited more reliably than the right. We explored functional connectivity of the right and left AG with the DMN during rest and retrieval to assess whether connec

Visual impairment and periventricular leukomalacia in children: A systematic review.

BACKGROUND: Thanks to Magnetic Resonance Imaging (MRI) it is now possible to diagnose lesions of the central nervous system (CNS) such as periventricular leukomalacia (PVL) from the first days of life. However, there are still few studies aimed at describing the relationship between MRI and the outcome of visual function in patients with PVL. AIM: To systematically review and investigate the relationship between MRI neuroimaging and visual impairment arising from PVL. METHODS AND PROCEDURES: Three electronic databases (PubMed, SCOPUS, Web of Science) were consulted from 15 June 2021-30 Septe

The glymphatic system as a mechanistic bridge between migraine and sleep: Evidence from clinical studies and animal models.

Migraine is a highly prevalent, disabling neurological disorder often accompanied by sleep disturbances; however, the mechanisms linking sleep disturbance to migraine susceptibility remain unclear. The glymphatic system, a sleep-modulated perivascular clearance network facilitated by astrocytic aquaporin-4 (AQP4) and functionally connected to meningeal lymphatic drainage, is a potential mechanistic link between migraine and sleep disturbances. This review synthesizes evidence from clinical neuroimaging, cerebrospinal fluid and blood biomarker studies, and preclinical animal models to evaluate

Associations between metabolic syndrome and regional brain iron depositions and cognitive function in middle-aged and older adults: A two multinational cohort study.

INTRODUCTION: The alterations of regional brain iron in patients with metabolic syndrome (Mets) and its relationship with cognitive function remain unclear. METHODS: These analyses utilized data from two prospective cohorts, from the UK Biobank (UKB) (21,346 participants) and from Jinan, China (224 participants). We capitalized brain iron in the striatum and thalamus of UKB. Then voxel-based analysis of quantitative susceptibility mapping (QSM) was used to detect regional susceptibility value alteration in our cohort and their relationship with cognitive function was assessed using linear reg
