Frameworks, Methodologies, and Tools for Evaluating Large Language Models in Digital Mental Health Interventions: Protocol for a Scoping Review.

BACKGROUND: Digital mental health interventions (DMHIs) can help close persistent gaps in access to assessment, prevention, and treatment. Recent advances in generative AI, particularly large language models (LLMs), further expand this promise by enabling natural language understanding, personalization, and empathic interaction across assessment, support, and therapeutic contexts. However, significant evaluation challenges persist, including a lack of standardized constructs and validated instruments, which limit the comparability, reproducibility, and generalizability of the findings. Improving the Screening and Treatment of Hepatitis C in a Rural Primary Care Clinic.

BACKGROUND AND OBJECTIVES: With the potential to lead to liver failure, cirrhosis, and death and the availability of hepatitis C Virus (HCV) treatment with direct-acting antiviral medications, primary care clinicians need to take action to improve screening and treatment of HCV. Current literature demonstrates gaps in knowledge contribute to low HCV screening and treatment rates. The project's purpose is to use a multidisciplinary approach to patient and clinician education to improve HCV care in a rural primary care clinic. METHODS: This quality improvement project involved 1225 adult patien

Afterbirth support provision for women following a traumatic/distressing birth: Survey of NHS hospital trusts in England.

OBJECTIVE: Despite recommendations within postnatal care guidelines, many National Health Service (NHS) hospital trusts in the UK provide an afterbirth, debriefing type service for women who have had a traumatic/distressing birth. Currently there are a lack of insights into what, how, and when this support is provided. The aim of this study was to explore afterbirth provision for women who have had a traumatic/distressing birth in NHS hospital trusts in England. DESIGN: An online survey comprising forced choice and open text comments was disseminated via direct email and social media to NHS h

Genetic architecture of MRI-derived cervical spinal cord morphology reveals sensory-motor axis and systemic disease associations.

The spinal cord is critical to motor, sensory, autonomic function, and increasingly implicated in neurological disease and human health, yet its genetic architecture remains largely unexplored. We perform large-scale phenotyping of the upper cervical spinal cord (C1-C3) structure using brain magnetic resonance imaging from over 80,000 UK Biobank participants, extracting shape metrics including cross-sectional area, diameters, and eccentricity. We identify a total of 179 independent genome-wide significant variants, with cervical spinal cord morphology showing moderate to high SNP-based heritab

Understanding Changes in Child Welfare Service Needs in the United States: A Quarter-Century Perspective.

This study examined how caregivers and child welfare professionals reported service needs over 25 years, focusing on housing, legal, mental health, and substance use supports. Using three cohorts of the National Survey of Child and Adolescent Well-Being (1999-2024; N = 6,102), we estimated longitudinal multilevel logistic regression models to assess trends in reported service needs and associations with child, caregiver, and child welfare professional characteristics. Reported housing and legal needs declined modestly, while reported mental health and substance use needs increased. Caregivers

Advancing precision health discovery in a genetically diverse health system.

Linking genetic data with electronic health records in hospital biobanks promises to advance precision medicine, but limited ancestral diversity constrains discovery and generalizability. We analyzed 93,936 participants from the UCLA ATLAS Community Health Initiative to inform disease prevalence and genetic risk across five continental and 36 fine-scale ancestry groups. We discovered numerous unreported gene-phenotype associations, including FN3K with intestinal disaccharidase deficiency in Europeans and admixed Americans. Polygenic scores (PGS) robustly predicted common diseases, with effects

The engagement-validation loop: Sycophancy, anthropomorphic projection, and clinical risk in informal AI-based mental health support.

Approximately one billion people worldwide live with a mental disorder, yet access to minimally adequate treatment remains low. Against this backdrop, general-purpose generative artificial intelligence (GenAI) systems have rapidly expanded into informal mental health support. OpenAI disclosed that roughly 1.2 million ChatGPT users weekly display indicators of suicidal planning or intent, and meta-analytic evidence supports modest efficacy for AI chatbots in reducing common mental health symptoms. Nevertheless, these tools may pose substantial clinical risks for vulnerable individuals. Two inte

The Association between Structural Racism and Infant Mortality: A Systematic Review.

The evidence that links structural racism to disparities in infant mortality across racial groups needs closer examination, given its central role in achieving health equity. We undertook a systematic review using four databases (MEDLINE, Academic Search Complete, APA PsycInfo, and SocINDEX) to determine whether structural racism is associated with infant mortality rates (IMR). The review used the PRISMA guidelines. Relevant articles between 1955 and 2024 were included. A narrative synthesis was conducted informed by critical race theory. Data quality was assessed using the Critical Appraisal

Beyond Mosquitoes: A Review of Pediatric Vector-Borne Diseases Excluding Malaria and Arboviral Infections.

Vector-borne diseases (VBDs) significantly impact global child health, with mosquito-transmitted infections like malaria and arboviruses accounting for a substantial portion of this burden. However, other arthropod-borne diseases-transmitted by vectors such as ticks, fleas, sand flies, lice, and triatomine bugs-also pose serious health risks to children worldwide. This review specifically excludes mosquito-borne diseases to concentrate on these less-discussed yet clinically important pediatric VBDs. We examine their clinical manifestations, diagnostic challenges, and treatment options, highlig

A Roadmap for the Rational Use of Biomarkers in Oral Disease Screening.

Oral health has witnessed a significant transformation with the integration of biomarkers in early-diagnostic processes. This article briefly reviews the types of biomarkers used in the screening and early detection of oral diseases, particularly oral cancer, periodontal diseases, and dental caries, with an emphasis on molecular biomarkers. While the advent of these biomarkers may represent a leap forward in oral healthcare, it also opens the door to potential overtesting, overdiagnosis, and overtreatment. To inform the selection of novel biomarkers and ensure their rational use in screening t

Attitudes About the Use of Geosocial Networking Applications for HIV/STD Partner Notification: A Qualitative Study.

Meeting sex partners through geosocial networking (GSN) apps is common among men who have sex with men (MSM). MSM may choose not to exchange contact information with partners met through GSN apps, limiting their own and health departments' ability to notify partners of HIV/STD exposure through standard notification methods. Using online focus groups (four groups; N = 28), we explored the perspectives of U.S. MSM regarding offer of partner notification features through GSN apps. Most participants were comfortable with HIV/STD partner notification delivered via GSN apps, either by partner servic

Ebola virus disease: Essential clinical knowledge.

Since its initial outbreak in 1976, Ebola virus disease (EVD) has affected thousands of people, causing severe illness with high mortality rates. In the absence of a vaccine or effective specific treatment, as well as the lack of early diagnostic and detective methods, the EVD outbreak has generated a significant worldwide health concern. Insufficient health-care system resources, deficient infection control measures, and the shortage of appropriate personal protective equipment acted as amplifiers of the outbreak extension, especially in poorly resourced and unprepared communities. Operating

With strengthened guidelines for health care workers, the CDC ups its game against the deadly Ebola virus.

Informed by the cases of two nurses who contracted Ebola virus disease (EVD) while caring for a patient with the disease in Dallas, TX, the Centers for Disease Control and Prevention (CDC) in Atlanta, GA, has unveiled strengthened guidance for health care workers. Further, nursing organizations are pledging to work together to identify gaps and make system-level improvements to protect both patients and caregivers. The CDC's new recommendations emphasize rigorous training for health care workers in how to put on and take off personal protective equipment (PPE), and they state that this activit

An ecological model of the impact of sexual assault on women's mental health.

This review examines the psychological impact of adult sexual assault through an ecological theoretical perspective to understand how factors at multiple levels of the social ecology contribute to post-assault sequelae. Using Bronfenbrenner's (1979, 1986, 1995) ecological theory of human development, we examine how individual-level factors (e.g., sociodemographics, biological/genetic factors), assault characteristics (e.g., victim-offender relationship, injury, alcohol use), microsystem factors (e.g., informal support from family and friends), meso/ exosystem factors (e.g., contact with the le

Quality of life and broader experiences of those with acoustic neuroma: a mixed methods approach.

Background Acoustic neuromas (ANs) are consistently associated with decreased quality of life (QOL) related to the physical and psychosocial impacts of symptoms experienced from the tumour and its treatment. This study explored patient-reported experiences of ANs in New Zealand, with a focus on the impact on QOL and the provision of information, support and services. Methods A mixed methods approach was taken, conducting an online community survey that included the Penn Acoustic Neuroma Quality of Life Scale (N  = 52). Those who indicated interest were offered semi-structured interviews after

Maternal syphilis and accomplishing sexual partner treatment: still a huge gap.

Congenital syphilis (CS) is a preventable epidemic disease for which control is hindered by socioeconomic and health system issues. Inadequately treated maternal syphilis (MaS) commonly has serious adverse obstetric outcomes. This study - which is a cross-sectional study in a Brazilian large urban public Mother and Child hospital, making use of secondary data - evaluated the occurrence of treatment of sexual partners (SP) among women with MaS. The results showed that 771 MaS cases were identified from 2007 to 2014. No information on SP treatment was available in 570 (73.9%) cases. Of the 201 c
