Recurrence of duodenal ulcers during five years of follow-up after cure of Helicobacter pylori infection.

BACKGROUND: Chronic Helicobacter pylori-associated gastritis is now widely accepted as one of the most important pathogenic factors in duodenal ulcer disease. However, little is known about for how long patients remain free of duodenal ulcer relapses after H. pylori infection has been cured. In the present study, we investigated remission time during a 5-year follow-up period after anti-H. pylori treatment. METHODS: The patients were randomly allocated to treatment with either a combination of 3 x 600 mg bismuth subsalicylate and 2 x 1000 mg amoxycillin or 3 x 600 mg bismuth subsalicylate mon…

Antral G-cell and D-cell numbers in Helicobacter pylori infection: effect of H. pylori eradication.

BACKGROUND: It has recently been recognized that Helicobacter pylori infection is associated with abnormalities in the regulation of gastrin secretion. We investigated whether there was a relationship between H. pylori infection and G-cell and D-cell numbers. METHODS: The numbers of antral G cells and D cells were compared between 20 patients with duodenal ulcer and 24 volunteers, 12 with and 12 without H. pylori infection. The effect of eradication of H. pylori infection on G-cell number was also evaluated. Antral mucosal biopsy specimens were examined using immunohistochemical techniques sp…

Short report: a non-metronidazole triple therapy for eradication of Helicobacter pylori infection--tetracycline, amoxicillin, bismuth.

Triple therapies using bismuth, metronidazole and tetracycline or amoxicillin were the first truly successful anti-H. pylori therapies. Metronidazole resistance has become an increasing problem that has severely limited the usefulness of the original triple therapy. Resistance to tetracycline or amoxicillin has not been reported and both are effective against H. pylori. We therefore tested a new triple therapy consisting of 500 mg tetracycline, 500 mg amoxicillin, and 2 tablets of bismuth subsalicylate each administered four times daily (with meals and at bedtime) for 14 days during treatment

Helicobacter pylori associated chronic gastritis and peptic ulceration in patients taking non-steroidal anti-inflammatory drugs.

Helicobacter pylori is now recognized as a frequent cause of histological chronic gastritis, and this has radically changed our understanding of this common condition. In the light of these developments, the traditional view that non-steroidal anti-inflammatory drugs are one of the common 'environmental' causes of chronic gastritis has been re-examined. Gastric mucosal biopsies have been studied from 430 patients undergoing routine upper gastrointestinal endoscopy, 99 of whom had recently been taking non-steroidal anti-inflammatory drugs. No significant association was found between the use of…

Paediatric Helicobacter pylori infection and allergic and inflammatory diseases: a contemporary systematic review with meta-analysis.

UNLABELLED: Helicobacter pylori (H. pylori) infection is prevalent worldwide, and its potential protective role in allergic and immune diseases has been a topic of speculation. This systematic review aims to analyse recent literature to establish a consensus on this relationship. Articles published in the past 12 years regarding association of H. pylori and various allergic diseases were screened. Thirty-five articles from three databases were assessed for bias using the ROBINS-E tool. Objective data from each article were collected for this review. Among the reviewed articles, 14 focused on a…

To eradicate or not? Helicobacter pylori in patients with inflammatory bowel disease: an updated systematic review and meta-analysis.

BACKGROUND AND AIMS: The management of Helicobacter pylori (H. pylori) in patients with inflammatory bowel disease (IBD) presents a common clinical dilemma. While standard guidelines recommend H. pylori eradication to prevent gastric pathology, emerging evidence suggests a potential complex relationship with IBD. This study aims to critically evaluate this relationship through an updated systematic review and meta-analysis to inform clinical decision-making. METHODS: A comprehensive literature search on four major databases, PubMed, Embase, Medline, and Web of Science, was conducted, and all …

The Investigation and Treatment of Helicobacter pylori for the Prevention of Gastric Cancer: A Narrative Review and Assessment of Management Approaches.

BACKGROUND: The rate of gastric cancer has reduced globally in all ages in recent years, but some data have started to emerge of a slowing in the decrease or even an increase in those aged >50 years, particularly in wealthier countries. Helicobacter pylori infection is associated with 89% of gastric cancer diagnoses worldwide, and its eradication is associated with a risk reduction. This review aims to assess the various approaches to diagnosing this carcinogenic pathogen. SUMMARY: A test-and-treat approach (noninvasive H. pylori testing for symptomatic patients) towards young people has been…

Metagenomic analysis of the intragastric and oral microbiome associated with gastric carcinogenesis after Helicobacter pylori eradication.

BACKGROUND: Few reports have investigated bacterial microbiome in relation to gastric cancer after eradication of Helicobacter pylori (H. pylori). We aimed to conduct a genetic analysis of the gastric and oral microbiome, to identify its association with development of gastric cancer after H. pylori eradication. METHODS: The patients with prior endoscopic submucosal dissection (ESD) history after eradication (cancer patients) and control patients after eradication who underwent upper gastrointestinal endoscopy at Kawasaki Medical School Hospital from May 2021 to March 2023 were enrolled. The NfsB-like nitroreductase RdxA in Helicobacter pylori is a flavin reductase: possible implications for metronidazole resistance.

The microaerophilic bacterium Helicobacter pylori infects a large proportion of the global human population and is the leading cause for peptic ulcer disease and gastric cancer. The H. pylori eradication therapy rests on four compounds, including the 5-nitroimidazole metronidazole. Unfortunately, metronidazole resistance in H. pylori is an increasingly pressing and severe problem. In most metronidazole-resistant H. pylori isolates mutations in the rdxA gene can be found. RdxA is an NfsB-like nitroreductase and was reported to reduce metronidazole to toxic intermediates. Since nitroreductases c…

PCR versus histology for detection of Helicobacter pylori in gastric biopsies: increased sensitivity of molecular testing.

BACKGROUND: Accurate diagnosis of Helicobacter pylori infections is essential for timely eradication; however, histopathological diagnostic performance is limited in some cases such as patchy mucosal colonization. This retrospective study evaluated the diagnostic yield of PCR in archived formalin-fixed, paraffin-embedded (FFPE) gastric biopsy specimens previously evaluated by histology, and compared endoscopic, histologic, and clinical characteristics of histology-negative/PCR-positive and histology-positive/PCR-positive cases. METHODS: FFPE gastric biopsies (n = 830), obtained during routine…

Vitamin D Level in Blood Serum and Its Connection with Helicobacter pylori Infection: A Systematic Review.

Helicobacter pylori (H. pylori) is widely recognized for its role in gastric pathologies, including ulcerative disease and malignant transformation of the gastric mucosa. Lower serum concentrations of 25-hydroxyvitamin D may impair immune responses, potentially influencing colonization dynamics and persistence of H. pylori infection. A systematic review was conducted to critically evaluate the current evidence on the association between serum vitamin D levels and H. pylori infection. PubMed, Scopus, Cochrane Library, and Web of Science databases were comprehensively searched for relevant artic…

Molecular detection of Helicobacter pylori in saliva of Sri Lankan adults with periodontitis, gastritis or both conditions.

BACKGROUND: Periodontitis is an immuno-inflammatory disease affecting the tooth-supporting structures, primarily caused by dysbiosis of the oral microbiome. The involvement of one of the gastric pathogens, Helicobacter pylori, has been reported among individuals with periodontitis. However, the evidence regarding the association between oral H. pylori, periodontitis, and gastritis remains inconsistent and has not been investigated in Sri Lanka. Therefore, this study aimed to detect the oral H. pylori in a cohort of Sri Lankan adults and to evaluate its association with periodontitis and gastri…

Evidence-Based Guidelines for the Diagnosis and Treatment of Helicobacter pylori Infection in Korea: 2025 Revised Edition.

Since the 2020 Korean guidelines for Helicobacter pylori treatment, clarithromycin resistance rates have risen from 17.8% to 33.3%, dual-priming oligonucleotide-based polymerase chain reaction-guided tailored therapy has been adopted, and potassium-competitive acid blockers (P-CABs) have become available. This fourth revision addressed these changes. Nine key questions were addressed through systematic review and meta-analysis. Thirteen recommendations were evaluated using a modified Delphi process involving 64 experts. Twelve recommendations achieved a first-round consensus; one required revi…

New concepts of chitosan-based mucosal formulations for supporting control of the gastric pathogen Helicobacter pylori.

Helicobacter pylori (H. pylori) is a human gastric pathogen that colonizes the stomach by adapting to acidic pH, penetrating the gastric mucus layer, adhering to epithelial and mucosal receptors, and delivering virulence factors that promote inflammation and immune evasion. Antibiotic-based regimens have markedly reduced the burden of H. pylori-associated disease; however, increasing antimicrobial resistance, treatment failure, recurrence, and disruption of the gut microbiota remain important clinical challenges. Future therapeutic strategies should therefore combine bacterial control with mod…
