Bed rest in twin pregnancy: identification of a critical period and its cost implications.

Since twin pregnancies often result in poor perinatal outcomes, many physicians advise prolonged bed rest. Recommendations concerning the timing of bed rest conflict and are made with little assessment of costs. This review of twin pregnancies in the North Central Illinois perinatal region establishes that twins are most vulnerable if they are born between 27 and 34 weeks' gestation. If bed rest is to be imposed, it probably should be timed so as to influence this vulnerable period. Intervention (bed rest in the hospital from 27 to 34 weeks' gestation) would cost between $5,720 and $6,909 per

Systematic overestimation of mortality by SAPS II in contemporary ICU practice: a retrospective study analyzing ICU patient outcome using the 2013-2024 French National Health Database.

BACKGROUND: The present study re-evaluates the performance of the Simplified Acute Physiology Score (SAPS) II to predict in-hospital mortality of Intensive Care Unit (ICU) patients by analyzing the French nationwide hospital stay billing database between 2013 and 2024. METHODS: From 2,702,204 adult ICU admissions, 80% (n = 2,161,763; (mean SAPS II = 41 ± 21.3) were used to fit logistic regressions of the in-hospital mortality probability as polynomial functions of the SAPS II up to the 5th degree. The remaining 20% (n = 540,441; (mean SAPS II = 41 ± 21.3)) of the analytical sample was used as…

Impact of stress hyperglycemia ratio, derived from glycated albumin or hemoglobin A1c, on mortality among ST-segment elevation myocardial infarction patients.

BACKGROUND: Stress hyperglycemia ratio (SHR), associated with adverse outcomes in patients with ST-segment elevation myocardial infarction (STEMI), has several definitions. This study aims to assess the prognostic value of SHR, derived from hemoglobin A1c (HbA1c) or glycated albumin (GA), to mortality. METHODS: The study comprised 1,643 STEMI patients who underwent percutaneous coronary intervention (PCI) in two centers. SHR1 was calculated using fasting blood glucose (FBG)/GA, while SHR2 was calculated using the formula FBG/(1.59*HbA1c-2.59). The primary endpoints were in-hospital death and

Inpatient mortality and its predictors after pericardiocentesis: An analysis from the Nationwide Inpatient Sample 2009-2013.

BACKGROUND: National registries have provided data on in-hospital outcomes for several cardiac procedures. The available data on in-hospital outcomes and its predictors after pericardiocentesis are mostly derived from single center studies. Furthermore, the outcomes after pericardiocentesis for iatrogenic pericardial effusion and the impact of procedural volume on in-hospital outcomes in the United States are largely unknown. METHODS: We used national inpatient database files for the years 2009-2013 to estimate the inpatient outcomes after pericardiocentesis in all-comers and in the subgroups
