Multichannel polysomnographic recording for evaluation of infant apnea.

The preceding discussion has consisted of a review of the technical and clinical aspects of pediatric multichannel recordings, which have become a widely used procedure in the clinical evaluation of infants with various apnea syndromes. It has been shown that multichannel recordings are superior to two-channel pneumocardiograms. Multichannel recordings should therefore be considered in all infants with unexplained episodes of apnea, bradycardia or cyanosis, in order to clarify the type of apnea and to rule out underlying conditions such as gastroesophageal reflux or seizures. The role of multi

Unwarranted variation in training in isolated coronary artery bypass grafting in the United Kingdom.

OBJECTIVE: Several studies have reported comparable outcomes between trainees and consultants performing isolated coronary artery bypass grafting (CABG). However, the extent to which trainee operating opportunities vary between consultants and hospitals after accounting for patient risk profile remains unclear. METHODS: Adult patients undergoing first-time, elective or urgent isolated CABG in the United Kingdom between 2010 and 2019 were identified from the National Adult Cardiac Surgery Audit database. Procedures in which a trainee was recorded as the primary operator were compared with thos

Reasons for Regimen Modifications of Antineoplastic Agents in an Outpatient Oncology Clinic in Puerto Rico.

OBJECTIVE: Evidence-based modifications to antineoplastic therapy regimens are common. The reasons for those changes are varied and include dose rounding, adjustments based on organ function or changes in weight of the patient, and adverse reactions. Given this complexity, proper documentation, and use of computerized provider order entry (CPOE) systems are essential in promoting optimal decision making and patient outcomes. The objective of this study is to explore common reasons for therapeutic regimen modification and their documentation. METHODS: This was a descriptive, retrospective stud

Evaluation of AI-assisted chart review for acute myocardial infarction and cardiogenic shock.

INTRODUCTION: The Danish RETROSHOCK registry has provided valuable insights into patients with acute myocardial infarction complicated by cardiogenic shock (AMICS) and their clinical trajectories. However, continuously updating the registry through manual chart review is time-consuming. METHODS: Electronic medical records were retrieved from patients admitted to Odense University Hospital from 2018 to 2022, assigned an ICD-10 code suggestive of AMICS. A random sample of 100 consecutive patients was selected for testing the AI-assisted chart review. The AI tool was a natural language processin

The Use of the Fecal Immunochemical Test in the Acute-Care Hospital Setting.

OBJECTIVE: The fecal immunochemical test (FIT) is indicated for colon cancer screening in asymptomatic average-risk patients. However, the inclusion of orders for non-indication-based FITs in medical records has led to increased inappropriate use in emergency department (ED) and inpatient settings. This study aimed to evaluate the clinical impact of FIT results when the test is used for purposes other than routine screening. METHODS: We conducted a single-center, retrospective chart review using electronic medical records, analyzing patients who underwent the FIT in the ED or while admitted t
