Procedures related to profound hypothermia and circulatory arrest have produced a great improvement in the operative results of cardiac repair in neonates and infants. Adrenocortical carcinoma rarely produces pure primary aldosteronism. We document the occurrence of such a case. Through tumor steroid analysis, we show that the content of aldosterone per gram of tumor tissue is diminished compared to aldosterone producing adenomas. We also demonstrate the occurrence of angiotensin II receptors on the tumor, a finding hitherto noted only on aldosterone producing adenomas.

Metabolic and biochemical studies of primary aldosteronism have led to major contributions to our understanding of the metabolic effects of mineralocorticoids and the control of aldosterone in man. Our current understanding of the syndrome and the progress that has been made in its differential diagnosis, its classification into various types and the localization and lateralization of a unilateral adenoma are reviewed. The perioperative anaesthetic management is presented and specific problems of importance to the anaesthetist are discussed.

We studied the circulatory, psychological, plasma catecholamine, cortisol, and lipid responses to a real-life stress--third molar extractions--in 21 patients and the effects of sedation with intravenous diazepam and of inclusion of epinephrine with the local anesthetic. Seven children with precocious puberty were treated with cyproterone acetate and their adrenal function studied. In each child the drug caused significant adrenal hypofunction which was secondary to adrenocorticotrophic hormone suppression. None of these children had clinical evidence of adrenal hypofunction and it is assumed that the drug has glucocorticoid-like properties. Children receiving cyproterone acetate should carry information to warn doctors of this effect since they may require steroid cover during illness.

A man bitten by a pet dog had fulminant thrombotic thrombocytopenic purpura within 72 hours. Rhabdomyolysis was severe and acute anuric renal failure was present. The suspected causative agent was Pastuerella multocida or clostridia with possible endotoxemia, though early treatment precluded a definitive diagnosis. The patient responded dramatically to a combined therapeutic approach of antibiotics, steroids, splenectomy, and supportive hemodialysis.

Benign intracranial hypertension (BIH) and empty sella syndrome occurred in a patient with Cushing syndrome resulting from adrenal adenoma, removal of which was followed by return of cerebrospinal fluid pressure to normal. This case does not clarify the pathophysiology of BIH, but it does raise questions about the treatment of BIH with corticosteroids, which are of unproved efficacy and may sometimes cause the same clinical syndrome.

Estradiol and testosterone both lowered endogenous liver glycogen and at 20-fold higher doses impaired triamcinolone acetonide mediated glyconeogenesis in adult adrenalectomized male rats.
