This article reviews the Montreal experience of hearing preservation in acoustic neuroma surgery. The medical records since 1995 of 36 patients who underwent acoustic neuroma extirpation with the intent to preserve hearing were examined. Intraoperative monitoring was conducted using auditory brainstem response measurement with electrocochleography via a transtympanic electrode. The role of intraoperative monitoring in guiding surgical technique and its correlation with postoperative hearing outcome are discussed. Trauma histories were obtained from 711 subjects in a large study of anxiety disorders, with the intent of determining the prevalence and nature of psychological trauma in this group. Twenty-seven percent of subjects reported significant trauma; 35% of these (10% of all subjects) met DSM-II-R criteria for posttraumatic stress disorder (PTSD). Subjects reporting sexual trauma were significantly more likely to have PTSD. The rate of PTSD was not higher in subjects with panic disorder than in those with other anxiety disorders.

Prompt recognition of suicidal intent can prevent suicide in the elderly. Some psychiatric disorders, such as depression, psychoses and organic brain syndromes, can predispose these persons to suicide, as can medical disorders that result in pain, disability or dysfunction. Other factors that increase the risk for suicide include chemical dependency and changing life events. Anti-depressant medications with a low anticholinergic and sedative profile are preferred because of age-related physiologic changes. Electroconvulsive therapy is reserved for serious cases.

One hundred fifty-five patients with squamous cell carcinoma of the pyriform sinus were treated with curative intent at the University of Florida between 1962 and 1984. All patients were staged according to the AJCC staging system and had a minimum 2-year follow-up; 84% had at least 5 years of follow-up. The locoregional control rates, the incidence of treatment-related complications, and survival rates are presented for radiotherapy as well as for surgery, with or without adjuvant irradiation.

Caustic ingestion may cause devastating injuries of the upper gastrointestinal tract and the respiratory system. We report here the successful treatment of a 37-year-old patient who ingested hydrochloric acid (100 mL; 24%) in suicidal intention. An oesophagogastroduodenoscopy revealed extensive necrosis of the gastric mucosa. Nearly one-half of patients with cryptogenic stroke have a patent foramen ovale (PFO).
