Optimisation before Crohn's surgery using Exclusive enteral Nutrition (OCEaN): A randomised controlled trial to determine whether preoperative exclusive enteral nutrition is more clinically and cost effective compared with standard care in patients undergoing surgery for Crohn's disease.

BACKGROUND: Exclusive enteral nutrition (EEN) is commonly used in paediatric Crohn's disease patients as a first-line therapy for inducing remission. Despite advances in immunosuppressive and biologic treatments, 23%-47% of patients with Crohn's disease will require surgery, with approximately 22% needing repeat surgeries. Potential suggested benefits of EEN in the perioperative setting include reduced steroid usage, reduced operative complications, and reduced need for stoma formation. METHODS: A multi-centre, two-arm, parallel group, open-label, pragmatic randomised controlled trial to asse

Too late to operate? Early surgical assessment in the era of advanced medical therapy for Crohn's disease.

BACKGROUND: The expanding availability of advanced medical therapies has profoundly reshaped the management of Crohn's disease. However, an unintended consequence may be the progressive deferral of surgical involvement, with patients often referred only after exposure to multiple treatment lines and with more advanced, high-risk disease. OBJECTIVE: To argue that the key paradigm shift should not be 'early surgery' per se, but rather early surgical assessment as a standard component of multidisciplinary care. METHODS: In this opinion letter, we discuss the implications of delayed surgical inv

Primary K-pouch creation versus J-to-K pouch conversion: An IDEAL stage 3 report.

AIM: Conversion of a failed ileoanal J-pouch to a continent ileostomy (K-pouch) is an uncommon salvage option that may preserve continence and small-bowel length. Outcomes after J-to-K pouch conversion compared with primary K-pouch creation remain poorly defined. This IDEAL stage 3 study compared short- and long-term outcomes between these two approaches. METHOD: We performed a retrospective review of patients undergoing primary K-pouch creation or J-to-K pouch conversion at a tertiary referral centre between 2013 and 2025. Demographic, clinical and operative characteristics, 30-day postopera

MRI prognostic features in rectal cancer neoadjuvant trials: A systematic review of reporting gaps across two decades.

BACKGROUND: Magnetic resonance imaging (MRI) is central to staging and treatment planning in rectal cancer, particularly in identifying candidates for neoadjuvant therapy. Prognostic MRI features such as tumour depth (mrT), extramural venous invasion (EMVI) and tumour deposits (TDs) are increasingly recognised as markers of systemic risk, yet their reporting in randomised controlled trials (RCTs) remains unclear. This systematic review aimed to evaluate the reporting frequency, consistency and integration of pre-treatment MRI prognostic variables in RCTs assessing neoadjuvant treatment for rec

Circular stapler platform and the risk of anastomotic leakage after elective left colectomy and high anterior resection.

AIM: The impact of different circular stapler platforms on the risk of anastomotic leakage following elective left colectomy and high anterior resection remains poorly defined. We compared the risk of anastomotic leakage across commonly used circular stapler platforms using advanced confounding control methods. METHODS: This pre-planned post hoc analysis pooled individual patient data from the published iCral-2 and iCral-3 studies and from the iCral-4 study, whose primary results are currently under peer review. Adults undergoing elective left colectomy or high anterior resection with standar

A systematic review and meta-analysis of preoperative MRI-based pelvimetry in predicting surgical difficulty in rectal cancer: Implications for personalised surgical planning.

BACKGROUND: Magnetic resonance imaging (MRI)-based pelvimetry has been investigated for predicting surgical difficulty in total mesorectal excision (TME) for rectal cancer. However, heterogeneity in pelvimetric parameters, surrogate outcomes, and study design has resulted in inconsistent evidence. This systematic review aimed to synthesise the evidence on commonly used MRI parameters and difficulty surrogates and assess their methodological robustness and clinical relevance. METHODS: PubMed, Embase, and the Cochrane Library were systematically searched through December 2025 for studies evalua

'Everyone has their own way-there's no protocol'. An Australian qualitative study of clinician perspectives on the management of low anterior resection syndrome.

AIM: To identify clinicians' perceived challenges and barriers in the management of low anterior resection syndrome (LARS) through exploration of attitudes and decision-making practices around screening, diagnosis and management. METHOD: Exploratory interpretive qualitative design. Semi-structured interviews were undertaken and a reflexive thematic qualitative analysis was conducted using an iterative-inductive approach. RESULTS: Eleven colorectal surgeons, six physiotherapists and three specialist nurses were interviewed. Data were grouped into four major themes and 13 sub-themes. Variation

Health-related quality of life and functional outcomes of colon cancer patients in the Netherlands: A population-based study.

BACKGROUND: Colon cancer treatment has evolved significantly through earlier detection, less invasive surgery, optimised perioperative care and shortening chemotherapy duration from 6 to 3 months in 2017. These multidisciplinary improvements may contribute to better health-related quality of life (HRQoL), yet population-based data on their real-world impact on HRQoL and functional outcomes remain limited. METHODS: This cross-sectional study used data from the Dutch Prospective PLCRC cohort, including patients with stage I-III colon cancer who completed patient-reported outcome measures (PROMs

How safe is teaching of highly specialized rectal surgery? A propensity score-matched 10-year cohort study.

AIM: This study aimed to assess the impact of an institutional teaching program of highly specialized rectal procedures on intra- and early postoperative outcomes. METHODS: We performed a retrospective 10-year cohort study of consecutive patients undergoing low anterior resection (LAR) or abdominoperineal resection (APR) for rectal cancer <12 cm from the anal verge or restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) at a tertiary high-volume academic centre (2014-2023). Procedures were classified as expert (consultant-only) or teaching procedures (performed ≥75% by the tra

Quality of life assessment for colorectal cancer follow-up: A latent profile analysis of EORTC measures.

AIM: To identify distinct health-related quality of life (HRQoL) profiles among colorectal cancer (CRC) patients using patient-reported outcome measures (PROMs), with the aim of informing more personalised follow-up strategies. METHODS: Baseline HRQoL data from CRC patients enrolled in the FUTURE-primary study, a multicentre implementation study on home-based follow-up, were analysed. Patients completed the EORTC QLQ-C30 and QLQ-CR29 questionnaires. Aggregated symptom and function scales were developed using factor analysis and expert consensus. Latent profile analysis (LPA) was applied to id

A simple MRI/PET-derived index predicts relapse-free survival after neoadjuvant chemoradiotherapy in locally advanced rectal cancer.

AIM: Reliable pretreatment biomarkers for predicting resistance to standard chemoradiotherapy (CRT) in locally advanced rectal cancer (LARC) remain lacking. This study evaluated the prognostic value of the tumour area (TA)-to-maximum standardized uptake value (SUVmax) [T/S] ratio, a novel imaging index derived from magnetic resonance imaging (MRI) and 18F-fluorodeoxyglucose positron emission tomography-computed tomography (PET-CT), for predicting oncological outcomes in LARC. METHOD: We retrospectively analysed 68 patients with LARC who underwent preoperative CRT followed by curative resectio

Use less, waste less: Rethinking consumable use in proctology.

Proctology is a useful test case for low-waste surgery because the central premise of routine sterile set-up is contestable. These procedures are common, short and material intensive, but they are performed at a mucocutaneous site that cannot be rendered sterile. The distal gut contains thousands of bacterial species, with one third of them potential pathogens. Antiseptic preparation reduces this burden, but does not eliminate it. In this context, the relevant question is not whether every opened sterile item can be recycled, but whether it needed to be opened at all. This article argues for a

Damage control surgery in perforated diverticulitis reduces the stoma rate: A multicentre international retrospective cohort study.

AIM: Perforated diverticulitis of the left colon with peritonitis remains life-threatening. However, the optimal surgical strategy is still under debate. The presented study compares the impact of a routinely applied two-stage damage control strategy (DCS) in perforated purulent or faeculent diverticulitis with currently used non-DCS strategies. METHODS: This international multicentre retrospective cohort study was conducted in nine European hospitals. In five hospitals, the therapeutic algorithm covered the routine application of a two-stage DCS as a surgical standard for treating perforated
