Association between degree of bone-erosion and synovial fluid-levels of tumor necrosis factor alpha in the knee-joints of patients with rheumatoid arthritis.

OBJECTIVE: To determine whether concentrations of cytokines and matrix-degrading enzymes in synovial fluid of patients with rheumatoid arthritis or osteoarthritis are associated with the degree of bone-destruction in the same joint. METHODS: Determination of Interleukin-1 alpha, IL-1 beta, IL-1-receptor-antagonist, IL-6, IL-8, tumor necrosis factor alpha (by ELISA), collagenase-activity and caseinase-activity (by substrate-assays) in the SF (knee) of patients with RA (n42) or OA (n35). The degree of bone-destruction was assessed radiographically. RESULTS: SF cytokine- and enzyme-levels were

[Joint loose bodies in both upper ankle joints in neural muscular atrophy of the Charcot-Marie-Tooth-Hoffmann type (hereditary motor sensory neuropathy I)].

A case is reported in which a patient with Charcot-Marie-Tooth-Hoffmann disease (HMSN I) complained about bilateral ankle pain, stiffness and swelling. Radiologically multiple loose bodies in both ankle joints were detectable. The etiology of loose bodies or secondary chondromatosis is still unclear. Different reasons are discussed. Regarding the case reported on here we discuss possible etiological connections and pathophysiological pathways that may explain influence of Charcot-Marie-Tooth-Hoffmann disease (hereditary motor sensory neuropathy I) on the development of loose bodies at both ank…

Changes in extracellular potassium concentration in cat spinal cord in response to innocuous and noxious stimulation of legs with healthy and inflamed knee joints.

In 20 cats anaesthetized with alpha-chloralose and spinalized at the thoracolumbar junction we investigated the role of stimulation induced accumulation of extracellular potassium in the spinal cord in the processing of nociceptive discharges from the knee joint. For that we electrically stimulated the posterior articular nerve of the knee. We further performed innocuous and noxious stimulation of the knee and of other parts of the leg and studied the effect of an acute inflammation of the knee on [K+]0 in the spinal cord. Innocuous stimulation of the skin (brushing or touching) and innocuous

Altered cumulative joint moments and increased joint moment symmetry during sit-to-stand transitions for transfemoral bone-anchored limb users: A case series.

BACKGROUND: Sit-to-stand transitions are demanding activities for people with unilateral transfemoral amputation. Movement asymmetries during sit-to-stand place greater stresses on the intact limb joints compared to the amputated limb joints, potentially contributing to musculoskeletal overuse injury and pain. Bone-anchored limbs address socket-related prosthesis issues, but their impact on cumulative joint moments during sit-to-stand is yet to be fully understood. The purpose of this study was to investigate changes in cumulative joint moments and moment symmetry during sit-to-stand transitio…

Additional Operations Following Sacroiliac Joint Fusions: A Minimum 3-year Follow-up Cohort Study of Patients Operated for Chronic Sacroiliac Joint Pain.

STUDY DESIGN: A retrospective analysis of prospectively collected registry data. OBJECTIVE: To examine the subsequent pattern of surgeries and outcome of sacroiliac joint fusions (SJF) in patients previously operated in the lumbar spine as well as those scheduled for primary SJF. SUMMARY OF BACKGROUND DATA: Patients with SI joint pain often have a history of previous back surgery, but it is unclear how often patients require back surgery after SI-joint fusion. METHODS: We obtained data from the national Swedish spine registry on all patients who underwent a SJF between 1998 and 2019 and ide

Editorial Commentary: Combined Transosseous and Capsular Repair for Triangular Fibrocartilage Complex Tears With Distal Radioulnar Joint Instability-A Step Toward Comprehensive Joint Stabilization.

Triangular fibrocartilage complex (TFCC) tears involving the foveal attachment are a known cause of distal radioulnar joint instability. While transosseous repair of the deep TFCC fibers is a widely accepted technique for restoring distal radioulnar joint instability, persistent instability remains a clinical challenge in a subset of patients. Combining transosseous repair with volar and dorsal capsular repair is an important technique to consider when treating Atzei Class 2 TFCC tears. This evolving approach reflects a more comprehensive appreciation of the TFCC as a complex stabilizing unit,

Total Hip or Knee Arthroplasty in Patients with a History of Native Joint Septic Arthritis: A Meta-analysis and Systematic Review of Risk Factors of Prosthetic Joint Infection.

BACKGROUND: Septic arthritis is a debilitating diagnosis, and many of these patients elect to undergo total knee arthroplasty (TKA) and total hip arthroplasty (THA). The purpose of this systematic review and meta-analysis is to assess the risk of prosthetic joint infection (PJI) in those who undergo primary TKA or THA with a history of septic joint arthritis. Secondarily, we will evaluate patient-specific or surgical factors that could increase the risk of primary arthroplasty failure. METHODS: A comprehensive search of PubMed, CINAHL Plus, Embase, and Scopus was performed. Preferred Reportin

Deviations in femoral joint lines using calipered kinematically aligned TKA from virtually planned joint lines are small and do not affect clinical outcomes.

PURPOSE: Kinematically aligned total knee arthroplasty (KA TKA) strives to restore the native distal and posterior joint lines of the femur. Because the joint lines of a virtually planned femoral component on the native femur can serve as surrogates of those of the native femur, the present study determined position and orientation deviations of the femoral joint lines following calipered KA TKA from virtually planned joint lines and whether these alignment deviations affect clinical outcomes. Our hypotheses were that the alignment deviations for most knees would be less than 2 mm and/or 2° an

Self-rated walking disability and dynamic ankle joint stiffness in children and adolescents with Juvenile Idiopathic Arthritis receiving intraarticular corticosteroid joint injections of the foot.

BACKGROUND: Children and adolescents with Juvenile Idiopathic Arthritis (JIA) exhibit deviations in ankle dynamic joint stiffness (DJS, or moment-angle relationship) compared to healthy peers, but the relationship between ankle DJS and self-reported walking impairments has not been studied. This secondary analysis aimed to investigate the relationship between ankle DJS and self-reported walking disability in juveniles with JIA, and to determine whether intraarticular corticosteroid foot injections (IACI) were associated with long term changes in ankle DJS. RESEARCH QUESTIONS: Is ankle DJS alt

The severity of basilar invagination and atlantoaxial dislocation correlates with sagittal joint inclination, coronal joint inclination, and craniocervical tilt: a description of new indexes for the craniovertebral junction.

BACKGROUND: Joint-distraction and intra-operative manipulation surgeries to correct basilar invagination (BI) and atlantoaxial dislocation (AAD) are becoming standard procedures. However, current data are unable to aid in the understanding of normal and abnormal morphology of the C1/C2 joints. OBJECTIVE: To study various aspects of C1/C2 joint morphology to create normative and patient data on joint abnormalities that could provide the surgeon with objective data for surgical planning and approach. METHODS: Seventy patients (age, 15-45 years) were compared with an equal number of age- and se…
