Military personnel with self-reported ankle injuries do not demonstrate deficits in dynamic postural stability or landing kinematics.

BACKGROUND: The odds of sustaining non-contact musculoskeletal injuries are higher in Special Operations Forces operators than in infantry soldiers. The ankle is one of the most commonly injured joints, and once injured can put individuals at risk for reinjury. The purpose of this study was to determine if any differences in postural stability and landing kinematics exist between operators with a self-reported ankle injury in the past one year and uninjured controls. METHODS: A total of 55 Special Operations Forces operators were included in this analysis. Comparisons were made between operat

Prevention of venous thromboembolism in patients with below-knee immobilisation of the leg--survey of current practice in The Netherlands.

INTRODUCTION: Anticoagulant treatment might be used to prevent DVT and its complications in patients with leg immobilisation. METHODS: In April 2007, a questionnaire was sent to all 79 Dutch surgical and orthopaedic teaching hospitals and was used to obtain information in relation to use of thromboprophylaxis for patients with immobilisation for below-knee leg injuries in their department. RESULTS: A total of 79 questionnaires were sent, of which 69 were returned (response rate 88%). In total, 8 of the 69 departments never used thromboprophylaxis at all (12%), 27 of 61 (44%) only prescribed

Functional ultrasonography in the diagnosis of acute anterior cruciate ligament injuries: a field study.

Functional ultrasonography is a rapid and inexpensive method of diagnosing anterior cruciate ligament (ACL) injuries. In previous studies, we assessed the diagnostic accuracy of this innovative method by experienced sonographers. The objective of the present study was to investigate whether an examiner without specialist expertise in arthrosonography can achieve similar positive results and whether this technique is effective as a screening tool that can help reduce the number of undetected ACL injuries. After a short period of training, a single examiner prospectively measured anterior tibial

Iatrogenic arterial injury and foot gangrene. A case report.

BACKGROUND: Elective orthopaedic operations on the knee is rarely complicated by arterial injury. REPORT: A case report is presented of a 10 year old girl that developed gangrene of the foot following an elective orthopaedic operation on the proximal tibial metaphysis. A transfixion wire that breached the posterior tibial cortex was most probably responsible for popliteal artery injury that resulted in ischaemia and gangrene. The patient had an amputation of all the toes in the affected limb and recovered satisfactory limb function. CONCLUSION: In operations on the proximal tibial metaphysis

Young Athletes With Quadriceps Femoris Strength Asymmetry at Return to Sport After Anterior Cruciate Ligament Reconstruction Demonstrate Asymmetric Single-Leg Drop-Landing Mechanics.

BACKGROUND: Young athletes who have had anterior cruciate ligament (ACL) reconstruction demonstrate suboptimal rates of return to sport, high rates of second ACL injuries, and persistent movement asymmetries. Therefore, the influence of musculoskeletal impairments on movement mechanics in this population needs to be further evaluated. HYPOTHESIS: The primary hypothesis was that among young athletes who have had ACL reconstruction, those with greater quadriceps strength asymmetry would demonstrate altered single-leg drop-landing mechanics at return to sport compared with individuals with more

MRI Criteria for Healing at 1 Year After Repair of a Traumatic Meniscal Tear.

BACKGROUND: Meniscal repair for a traumatic meniscal tear is increasingly used to preserve the meniscus. Interpreting postoperative magnetic resonance imaging (MRI) scans remains challenging, especially in symptomatic patients. There is a lack of reliable MRI criteria to affirm the healed character of a traumatic meniscal injury repair. PURPOSE: To identify relevant MRI criteria for meniscal healing after meniscal repair. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: We prospectively included all patients with a traumatic meniscal injury who underwent either an isolated meniscal

Acute Synovitis after Trauma Precedes and is Associated with Osteoarthritis Onset and Progression.

Osteoarthritis (OA) is a whole-joint disease characterized by cartilage destruction, subchondral bone sclerosis, osteophyte formation, and synovitis. However, it remains unclear which part of the joint undergoes initial pathological changes that drives OA onset and progression. In the present study, we investigated the longitudinal alterations of the entire knee joint using a surgically-induced OA mouse model. Histology analysis showed that synovitis occurred as early as 1 week after destabilization of the medial meniscus (DMM), which preceded the events of cartilage degradation, subchondral s

Three-dimensional kinematic and kinetic gait deviations in individuals with chronic anterior cruciate ligament deficient knee: A systematic review and meta-analysis.

BACKGROUND: Altered joint motion that occurs in people with an anterior cruciate ligament deficient knee is proposed to play a role in the initiation of knee osteoarthritis, however, the exact mechanism is poorly understood. Although several studies have investigated gait deviations in individuals with chronic anterior cruciate ligament deficient knee in the frontal and transverse planes, no systematic review has summarized the kinematic and kinetic deviations in these two planes. METHODS: We searched five electronic databases from inception to 14th October 2013, with key words related to ant

Landing adaptations following isolated lateral meniscectomy in athletes.

PURPOSE: Objective functional outcomes following isolated radial lateral meniscus tears in the athlete between the ages of 14-25 are not clearly defined. The objective of this study was to determine whether patients following lateral meniscectomy demonstrate lower extremity asymmetries relative to control athletes 3 months after surgery. We hypothesized that following lateral meniscectomy, athletes aged 14-25 years old would demonstrate altered landing biomechanics compared to sex, age, height, weight, and sport-matched controls. METHODS: A total of 18 subjects were included in this study. Ni

Arthrofibrosis following ACL reconstruction--reasons and outcome.

INTRODUCTION: Arthrofibrosis is a complication that severely influences the clinical outcome after anterior cruciate ligament (ACL) reconstruction. This retrospective clinical study analyses risk factors and outcome after arthrolysis in a large population. MATERIAL AND METHODS: Two hundred twenty-three patients who had undergone arthrolysis after ACL reconstruction were examined. Range of motion (ROM) was reduced due to arthrofibrosis of the joint in 70% (n=156). Other reasons, such as cyclops syndrome or osteoarthritis were found in 30% (n=67). The mean time interval between arthrolysis and

Back injuries and pain in adolescents attending a ski high school.

This study compared overuse injuries, small abnormalities, and pain alone in different types of skiing and activity levels. Subjects were 45 ski high school athletes aged 15-19 years. We found the back and knees significantly more prone to activity-related injuries and pain than other body regions. Thirty subjects (67%) had low back pain on the first examination, and 17 had pain caused by overuse of the back. This was more frequent among active, young competitive boys in the cross-country skiing group, with mature height less than 99%, than among noncompetitive boys in the same group or among

Changes in bone scans after anterior cruciate ligament reconstruction: a prospective study.

BACKGROUND: Whether anterior cruciate ligament reconstruction retards the progression of osteoarthrosis is not established. Bone scintigraphy can be useful for monitoring the course of osteoarthrosis. Bone scan findings are abnormal in the majority of patients with anterior cruciate ligament deficiency. Three uptake patterns can be distinguished. HYPOTHESIS: Reconstruction corrects the three abnormal bone scan patterns seen in patients with anterior cruciate ligament deficiency. STUDY DESIGN: Prospective cohort study. METHODS: We performed bone scintigraphy in 80 patients, before and 2 year
