Interventional vitamin C: A strategy for attenuation of coagulopathy and inflammation in a swine multiple injuries model.

BACKGROUND: Coagulopathy and inflammation induced by hemorrhagic shock and traumatic injury are associated with increased mortality and morbidity. Vitamin C (VitC) is an antioxidant with potential protective effects on the proinflammatory and procoagulant pathways. We hypothesized that high-dose VitC administered as a supplement to fluid resuscitation would attenuate inflammation, coagulation dysfunction, and end-organ tissue damage in a swine model of multiple injuries and hemorrhage. METHODS: Male Sinclair swine (n = 24; mean body weight, 27 kg) were anesthetized, intubated, mechanically ve

Flail Chest: Less Deadly than Originally Thought.

INTRODUCTION: Flail chest is considered a highly morbid condition with reported mortality ranging from 10 to 20%. It is often associated with other severe injuries, which may complicate management and interpretation of outcomes. The physiologic impact and prognosis of isolated flail chest injury is poorly defined. METHODS: This is a National Trauma Databank study. All patients from 1/2007 to 12/2014 admitted with flail chest were extracted. Patients with head or abdominal AIS ≥3, dead on arrival, or transferred, were excluded. Primary outcome was mortality; secondary outcomes were need for me

Child Abuse and Neglect and Subclinical Cardiovascular Disease Among Midlife Women.

OBJECTIVE: A childhood history of abuse or neglect may be associated with elevated adult cardiovascular disease (CVD) risk. No studies have examined associations between child abuse/neglect and subclinical CVD using a validated measure of abuse and neglect. We hypothesized that midlife women with a history of childhood abuse or neglect would have increased subclinical CVD beyond standard CVD risk factors. We tested moderation of associations by sleep, hot flashes, and race/ethnicity. METHODS: Two hundred ninety-five midlife women completed the Child Trauma Questionnaire, physiologic hot flash

Alterations in Amygdala-Prefrontal Functional Connectivity Account for Excessive Worry and Autonomic Dysregulation in Generalized Anxiety Disorder.

BACKGROUND: Generalized anxiety disorder (GAD) is characterized by the core symptom of uncontrollable worry. Functional magnetic resonance imaging studies link this symptom to aberrant functional connectivity between the amygdala and prefrontal cortex. Patients with GAD also display a characteristic pattern of autonomic dysregulation. Although frontolimbic circuitry is implicated in the regulation of autonomic arousal, no previous study to our knowledge combined functional magnetic resonance imaging with peripheral physiologic monitoring in these patients to test the hypothesis that core sympt

How metformin acts in PCOS pregnant women: insights into insulin secretion and peripheral action at each trimester of gestation.

OBJECTIVE: Metformin has been reported to reduce the risk of gestational diabetes (GD) in women with polycystic ovarian syndrome (PCOS). However, little is known about the mechanisms of action of this drug during pregnancy. In the attempt to fill this gap, we performed a prospective longitudinal study providing a detailed examination of glucose and insulin metabolism in pregnant women with PCOS undergoing metformin therapy. RESEARCH DESIGN AND METHODS: We enrolled 60 women with PCOS who conceived while undergoing metformin treatment. An oral glucose tolerance test and a euglycemic-hyperinsuli

Twenty-four hour cardiopulmonary stability in a model of assisted newborn Fontan circulation.

BACKGROUND: Morbidity and mortality after stage-1 palliation of hypoplastic left heart syndrome is high as a result of adverse physiologic conditions imposed by the systemic-to-pulmonary arterial shunt. Conversion to a systemic venous source of pulmonary blood flow (Glenn/Fontan) substantially decreases instability and mortality risk. Cavopulmonary assist has the potential to eliminate critical dependence on the problematic systemic arterial shunt. We studied this support modality during a 24-hour period in a neonatal animal model of univentricular Fontan circulation. METHODS: Lambs (8.1 +/-

Testing the test: the reliability of echocardiography in the sequential assessment of valvular regurgitation.

BACKGROUND AND OBJECTIVE: Substantial variability in serial echocardiographic qualitative assessment of valvular regurgitation may exist. Reader variability is generally well understood, but acquisition variability (portions of variability caused by equipment, sonographers, physiologic changes) has been less frequently assessed, particularly in combination with reader variability. We attempted to determine the relative contributions of acquisition and reader variability as components of total test-retest variability for aortic (AR) and mitral (MR) regurgitation. METHODS: Outpatient echocardio

Multimodal engagement estimation in paediatric robot-assisted gait training: integrating physiological sensing and biomechanical interaction.

BACKGROUND: Patients' engagement plays a crucial role in the effectiveness of robot-assisted gait training (RAGT), particularly in paediatric neurorehabilitation, where motivation and active participation strongly influence functional outcomes. Despite its clinical relevance, engagement assessment is still largely based on subjective observation, limiting objectivity and scalability. Objective modelling of engagement in children with cerebral palsy remains underexplored, especially when relying on physiological signals acquired during therapy, due to logistical, ethical, and technological chal

Postsynaptic Calcium Extrusion at the Mouse Neuromuscular Junction Alkalinizes the Synaptic Cleft.

Neurotransmission is shaped by extracellular pH. Alkalization enhances pH-sensitive transmitter release and receptor activation, whereas acidification inhibits these processes and can activate acid-sensitive conductances in the synaptic cleft. Previous work has shown that the synaptic cleft can either acidify because of synaptic vesicular release and/or alkalize because of Ca2+ extrusion by the plasma membrane ATPase (PMCA). The direction of change differs across synapse types. At the mammalian neuromuscular junction (NMJ), the direction and magnitude of pH transients in the synaptic cleft dur

Doppler echocardiography of normal and abnormal embryonic mouse heart.

To evaluate normal embryonic mouse heart development using Doppler echocardiography and to quantify changes in normal embryonic mouse cardiac function with increasing gestational age from the time of cardiac septation, a new method was applied using Doppler echocardiography. Trisomic embryos were screened to evaluate a model of abnormal cardiac anatomy. The development of the embryonic heart in mice has been well studied anatomically, but there are limited physiologic studies. A new method has been developed to assess the mouse fetal heart in a similar fashion to the current use of echocardiog

Dynamic-Bonded Conductive Pressure-Sensitive Adhesive Elastomer for Antibacterial Wound Repair and Real-Time Strain Monitoring.

Effective monitoring of mechanical stimuli such as pressure and torsion is essential for guiding wound management, yet current pressure-sensitive adhesives rarely integrate stable sensing, tissue-compatible adhesion, and antibacterial protection. Here, we develop a multifunctional conductive pressure-sensitive adhesive elastomer (PTAHCA) constructed from a dynamic thioctic-acid-based polymer network reinforced with clay nanotubes and ionic components. Synergistic dynamic covalent and non-covalent interactions stabilize the polymer matrix and markedly enhance its mechanical robustness and funct

Plant Acoustics for Evaluating Vase Life in Chrysanthemums: A Novel and Noninvasive Method.

Vase life is a key determinant of cut flower quality and market value. Conventional vase life assessment relies on visual inspection and physiological monitoring over several days to weeks, making it labor- and time-intensive. This study introduces a rapid and noninvasive approach using plant acoustics to assess postharvest vase-life-related variation in cut chrysanthemum flowers. Six chrysanthemum cultivars were grown under two supplemental lighting treatments (Hybrid and LED) and two planting densities (54 and 74 plants m-2). Acoustic monitoring was compared with optical microscopy for the a

Visualization of mitochondrial monoamine oxidase dynamics in live cells and DILI via a ratiometric NIR probe with "ICT+ESIPT" mechanism.

Monoamine oxidase (MAOs) is a key enzyme that maintains amine homeostasis and participates in liver pathological processes. Real time in situ monitoring of its activity is crucial for elucidating the mechanism of drug-induced liver injury (DILI). This study successfully constructed a ratio type fluorescent probe Cy-1 with mitochondrial targeting ability and near-infrared emission characteristics. This probe is based on MAOs catalyzed oxidative deamination reaction, and achieves fluorescence response through a clever "ICT (Intramolecular Charge Transfer) + ESIPT (Excited-State Intramolecular Pr

IoMT-enabled implant sensors for early detection of neurovascular complications in craniofacial distraction osteogenesis.

Craniofacial distraction osteogenesis (DO) is an established surgical technique for correcting complex craniofacial deformities and managing upper airway obstruction. Neurovascular complications, including injury to the inferior alveolar, infraorbital, and facial nerves, are recognized risks of the procedure. Current postoperative surveillance depends on periodic clinical examination and interval imaging, neither of which provides continuous insight into the mechanical forces transmitted to adjacent neurovascular structures during the active distraction phase. This letter examines whether impl

Small-Molecule Functionalized Needle Transistor Biosensor Enables Direct and Selective Monitoring of SO2 In Vivo.

In vivo electrochemical analysis with implantable microelectrodes enables real-time, high-resolution monitoring of chemical signals with exceptional sensitivity. However, directly detecting electrochemically inert molecules, such as SO2, remains challenging. To address this limitation, we designed and synthesized a small-molecule SO2 recognizer (SO2-SMR), which is capable of recognizing SO2 selectively. Then, we immobilized it onto the surface of a needle-based graphene field-effect transistor (FET), thereby achieving in vivo detection of SO2. Upon reacting with SO2, the SO2-SMR modulates the

[Clinical Guidelines for Monitoring Neuropsychiatric Medications in Patients Undergoing Bariatric Surgery].

Metabolic bariatric surgery induces significant anatomical and physiological changes in the gastrointestinal tract that may substantially affect the pharmacokinetics of orally administered drugs. These changes include, among others, reduced gastric volume, increased gastric pH, bypass of absorption segments in the small intestine, alterations in drug metabolism, as well as rapid weight loss and changes in body composition. Many neuropsychiatric medications are characterized by a narrow therapeutic window; therefore, the effects of metabolic bariatric surgery may lead either to reduced drug abs
