Bilateral myoepithelioma of the nasopharynx: a case report.

Myoepithelioma is a rare salivary tumour which is usually encountered in the parotid gland. Outside the parotid, myoepithelioma may arise from minor salivary glands of the hard palate and less frequently in bronchi or breast. In this report we describe the unique case of a metachronous bilateral nasopharyngeal myoepithelioma arising from the tubaric regions. Its microscopic features, immunophenotype, and the differential diagnosis together with a review of literature are discussed.

A new combined approach to the nasopharynx in head and neck plastic surgery: technique and application.

This article presents a combined approach to the nasopharynx, which has not been previously described. The technique is applicable to cases of recurrent nasopharyngeal carcinoma which exhibit lateral extension. We describe the technique and report a case in which it was used. A review is presented of all other techniques currently described. The authors would not advocate the use of a combined approach to the nasopharynx in all cases, but there certainly appears to be a place for this technique.

Influenza A virus infection and cardiac arrhythmia during the neonatal period.

Influenza virus infection during pregnancy increases the risk of spontaneous abortion, preterm contractions, fetal compromise and early neonatal mortality. We describe a case of fetal and neonatal cardiac arrhythmia coinciding with an influenza A virus infection. Viral RNA was detected in the neonate's nasopharyngeal secretions and also in its serum, indicating influenza viremia. The cardiac arrhythmia resolved spontaneously without antiarrhythmic treatment.

Persistent hypophyseal (craniopharyngeal) canal.

Two cases with a broad persistent hypophyseal canal connecting pituitary fossa and nasopharynx are presented. Both had nasopharyngeal surgery in early childhood and presented later with hypopituitarism. Coronal CT demonstrated the defects with no visible pituitary tissue. The spectrum of basal skull defects is discussed. Children with midline nasal polyps should have CT or MRI of the skull base prior to surgery to prevent inadvertent hypophysectomy.

Congenital pneumonia due to Mycoplasma pneumoniae.

A case of probable vertical transmission of Mycoplasma pneumoniae is presented. The presence of M pneumoniae was demonstrated by the polymerase chain reaction (PCR) in the nasopharyngeal aspirate of a newborn who developed pneumonia shortly after birth. This result was confirmed by performing a second PCR, amplifying another part of the genome of M pneumoniae. It is concluded that M pneumoniae can be added to the long list of pathogens known to cause congenital pneumonia.

Epstein-Barr virus and epithelial cells: implications for the otolaryngologist.

Epstein-Barr Virus (EBV) is an oncogenic herpesvirus associated with two human malignancies. Initially linked in the etiology of Burkitt's lymphoma, a B cell lymphoma, it has subsequently been implicated in the causation of nasopharyngeal carcinoma, an epithelial tumor of considerable world health importance. More recently a number of head and neck tumors have been shown to harbor the virus, suggesting a possible role for EBV in their development.

Nasal Septal Angiofibroma.

Juvenile nasopharyngeal angiofibroma is a benign vascular tumor seen predominantly in adolescent males in the second decade of life. Extranasopharyngeal angiofibroma includes vascular fibrous masses that occur outside the nasopharynx. The diagnosis of an angiofibroma is based on the clinical presentation and imaging, with biopsies being avoided to avoid excessive bleeding. Computed tomography scan is considered sufficient for the diagnosis of extranasopharyngeal angiofibroma as it clearly delineates and identifies the tumor.

An etiological role for the Epstein-Barr virus in the pathogenesis of classical Hodgkin lymphoma.

Although a pathogenic role for the Epstein-Barr virus (EBV) is largely undisputed for tumors that are consistently EBV genome positive (eg, nasopharyngeal carcinoma, endemic Burkitt lymphoma), this is not the case for classical Hodgkin lymphoma (cHL), a tumor with only a variable EBV association. In light of recent developments in immunotherapeutics and small molecules targeting EBV, we believe it is now timely to reevaluate the role of EBV in cHL pathogenesis.

Absence of Bordetella pertussis Among Infants Hospitalized for Bronchiolitis in Finland, 2008-2010.

In 169 Finnish infants hospitalized for bronchiolitis at age <6 months in 2008-2010, nasopharyngeal aspirates were tested by polymerase chain reaction for Bordetella pertussis and 16 viruses. Respiratory viruses were detected in 89% (71% with respiratory syncytial virus), but no infant had B. pertussis. The latter finding may reflect a positive effect from the broadening of the Finnish pertussis vaccination program in 2005.

A Novel Staphylococcal Cassette Chromosome mec Type XI Primer for Detection of mecC-Harboring Methicillin-Resistant Staphylococcus aureus Directly from Screening Specimens.

Methicillin-resistant Staphylococcus aureus (MRSA) screening using real-time PCR has decreased in sensitivity due to the emergence of variant staphylococcal cassette chromosome mec element (SCCmec) types. We have designed and validated a novel SCCmec XI primer, which enables for the first time the rapid detection of mecC-harboring MRSA directly from nasopharyngeal swabs without prior cultivation.

Detection of norovirus in respiratory secretions in children with respiratory tract infection.

To evaluate whether norovirus (NoV) can be a possible cause of respiratory tract infection, 562 nasopharyngeal samples collected from children admitted for influenza-like illness were tested for NoV. Three (0.5%) were positive NoV GII.4. The data show that NoV can be found in the respiratory secretions of children with respiratory symptoms and that respiratory involvement can precede gastrointestinal manifestations.

Mycotic infection with mucormycosis.

Mucormycosis comprises infections caused by fungi in the class Zygomycetes (order Mucorales). Clinical infections usually manifest as nasopharyngeal, cutaneous, pulmonary, or disseminated disease. Generally, the infection is found only in immunocompromised hosts. The underlying immunologic defects that are responsible for predisposing different populations of patients to the development of mucormycosis are not well understood. This article retrospectively reviews the experience at the Medical Center of Delaware with this relatively rare fungal infection.

Pulmonary hypertension due to chronic upper airway obstruction: a clinical review and report of four cases.

Partial airway obstruction due to the enlargement of the tonsils and adenoids is a well recognized clinical entity, but cardiorespiratory changes due to chronic obstruction have infrequently been reported. Four children with severe nasopharyngeal obstruction due to tonsil and adenoid hypertrophy, who developed pulmonary hypertension and cardiac failure, were studied. Relief of upper airway obstruction by adenotonsillectomy resulted in a regression of the presenting signs and symptoms.

Rats with one olfactory bulb removed and the contralateral naris closed can detect odors.

Rats with one olfactory bulb removed were trained to detect 0.5%-0.005% concentrations of amyl acetate. These rats had only a slight decrement in performance when the naris ipsilateral to the intact olfactory bulb was closed. Thus, vapors inhaled through one naris can stimulate olfactory receptor neurons in the contralateral nasal vault. This internasal stimulation is probably mediated by the nasopharyngeal canal or nasopharynx.

CT of the nasal cavity.

CT demonstrates nasal anatomy extremely well. The intimate relationship of the nasal cavity to the cerebral, orbital, paranasal sinus, nasopharyngeal, and oral tissues is noted. Examples of benign and malignant neoplasms, inflammatory processes, including intrinsic and extrinsic mass lesions are utilized to demonstrate the value of CT in assessing the nasal cavity in conjunction with its surrounding tissues. Congenital and traumatic lesions also are demonstrated to the same advantage.

A Comprehensive Approach to Juvenile Angiofibroma:Diagnosis, Staging, and Surgical Management.

Juvenile angiofibroma of nasopharynx is a benign but locally aggressive tumor predominantly affecting adolescent males, characterized by hypervascular growth in the nasopharynx. Early diagnosis and appropriate management are crucial to prevent complications. Symptoms typically include nasal obstruction, epistaxis, and hearing loss. Imaging techniques, such as contrast-enhanced CT and MRI, play a pivotal role in assessing tumor size, vascularity, and involvement of surrounding structures. Angiography, followed by embolization, is commonly used to reduce intraoperative bleeding risk. The treatme

Superficial punctate keratopathy in a pediatric patient was related to adenoid hypertrophy and obstructive sleep apnea syndrome: a case report.

BACKGROUND: Known causes of superficial punctuate keratopathy (SPK) in children include entropion, viral infection, blepharokeratoconjunctivitis (BKC), and toxicity of eye drops. However, there are some SPK patients whose causes could not be identified well. Herein, we describe the history, diagnosis, treatment, and prognosis of a rare case. CASE PRESENTATION: To report a case of superficial punctate keratopathy (SPK) which coexisted with floppy eyelid syndrome (FES) and presented as intermittent red eye and blurred vision in an 11-year-old boy who slept in the prone position. His condition d

Myoepithelial Carcinoma of the Nasopharynx: Report of a Rare Case and a Review of the Literature.

Salivary gland carcinomas are very rare in the nasopharynx, with the most frequent histologic types being adenoid cystic carcinoma, mucoepidermoid carcinoma and adenocarcinoma, not otherwise specified. Myoepithelial carcinoma (MECA) is a rare tumor of the salivary glands, and there are only three previously reported cases of nasopharyngeal MECA. The case presented is the fourth reported case of MECA in the nasopharynx. Due to the morphologic heterogeneity of MECA, immunohistochemistry is indispensable in ascertaining the diagnosis. MECA is a locally aggressive tumor, but the long-term prognosi
