Formation of the pre-metastatic niche by COL9A1 + cancer-associated fibroblasts via SDC4 engagement promotes multi-organ metastasis in gastric cancer.

BACKGROUND: Metastatic dissemination represents the primary cause of mortality in gastric cancer (GC), with multi-organ involvement posing a formidable therapeutic challenge. While organ-specific adaptations are well-studied, the conserved cellular programs that confer metastatic competence within primary tumors before dissemination remain poorly defined. METHODS: We performed integrated single-cell and spatial transcriptomic profiling on paired primary GC tumors and multi-organ metastases. Computational trajectory inference, regulon analysis, and cell-cell communication networks were employe

Key Clinical Trials Shaping the Future of Superficial High-Risk Bladder Cancer Management.

High-risk non-muscle-invasive bladder cancer (HR-NMIBC) represents a biologically aggressive disease state with substantial risk of recurrence, progression, and need for radical cystectomy. While intravesical Bacillus Calmette-Guérin (BCG) remains first-line for HR-NMIBC is standard of care, both recurrence and progression have led to the development of novel intravesical agents therapies. This article summarizes key clinical trials that are reshaping the management of HR-NMIBC, with particular emphasis on BCG-unresponsive disease. Approved intravesical and systemic agents demonstrate meaningf

Y-box Binding Protein 1 as a Target Molecule of a Novel Coumarin-based Anti-metastatic Agent.

BACKGROUND/AIM: We previously demonstrated that a novel coumarin-based compound, 7,8-dihydroxy-3-(4'-hydroxyphenyl)coumarin (DHC), suppresses metastasis of LM8 osteosarcoma cells both in vitro and in vivo. However, the molecular target of DHC remains unclear. The aim of this study was to develop novel DHC-derived compounds and to identify their target molecule. MATERIALS AND METHODS: Novel coumarin-based compounds were designed and evaluated for their effects on LM8 osteosarcoma cell invasion and migration. To identify a candidate target molecule of the newly synthesized compound SSKP-0076, l

KRT14 Drives Basal Muscle-Invasive Bladder Cancer Progression and Lung Metastasis by Directly Binding to and Stabilizing IGF2BP1.

Basal-type muscle-invasive bladder cancer (BMIBC) is characterized by aggressive metastasis and poor prognosis but lacks molecularly defined therapeutic targets. Through integrative analyses of clinical cohorts and BBN-induced mouse models, we identified KRT14 as a core oncogenic driver that orchestrates the KRT14-IGF2BP1 signaling axis to promote BMIBC progression and lung metastasis. Mechanistically, residues K294 and E295 within the KH2 domain of IGF2BP1 specifically recognize conserved residues D226 and E227 within the nuclear export signal of KRT14. This direct interaction facilitates IGF

Impact of biopsy-related variables on MRI assessment of tumor invasion in early oral squamous cell carcinoma.

OBJECTIVE: To evaluate the impact of incisional biopsy technique and biopsy-to-MRI interval on the concordance between radiological and pathological depth of invasion (DOI) in early-stage oral squamous cell carcinoma (OSCC). METHODS: A retrospective cohort study included 47 patients with early-stage OSCC who underwent incisional biopsy, MRI, and surgical resection. After excluding non-invasive tumors and cases with unreliable radiological DOI (r-DOI) assessment, 22 patients were analyzed. r-DOI was independently measured by two radiologists and compared with pathological DOI (p-DOI) from surg

Efficacy of cisplatin-based neoadjuvant chemotherapy and risk factors for residual extravesical disease in muscle-invasive bladder cancer: insights from a nationwide cohort.

BACKGROUND: Cisplatin-based neoadjuvant chemotherapy (NAC) improves survival in muscle-invasive bladder cancer (MIBC) as long as disease progression does not occur during treatment. However, predictors of NAC sensitivity remain elusive in clinical practice. This study evaluated the efficacy of NAC followed by radical cystectomy (NAC-RC) in cStage II-IIIA MIBC and identified the risk factors associated with residual extravesical disease. METHODS: Clinical data from 1474 patients who underwent radical cystectomy for cStage II-IIIA urothelial carcinoma were collected from 36 institutions of the

Assessing risk stratification in Bacillus Calmette-Guérin-treated high-grade Ta nonmuscle-invasive bladder cancer patients.

PURPOSE: According to European Association of Urology (EAU) guidelines, nonmuscle-invasive bladder cancer (NMIBC) is stratified by pathological stage and three clinical risk factors (age >70, size >3 cm, multiple lesions). High-grade (HG) Ta can be categorized as intermediate-risk (IR) or high-risk (HR). However, the 2021 EAU stratification is based on non-BCG-treated patients. This study investigated the predictive value of EAU risk groups and factors in TaHG treated with BCG. METHODS: We retrospectively reviewed NMIBC patients treated with BCG from 2005 to 2022. Patients were stratified by

Interobserver Reproducibility of Pelvicalyceal Invasion in Renal Cell Carcinoma Nephrectomies Among Genitourinary Pathologists.

Pelvicalyceal invasion (PCI) is a relatively novel pT3a staging parameter for renal cell carcinoma (RCC) nephrectomies. While interobserver reproducibility staging studies of sinus/vascular invasion in RCC exist, a similar evaluation for PCI has not been performed. Moreover, in our experience, there is also diagnostic variability in how pathologists interpret PCI. Herein, we explore interobserver reproducibility among genitourinary (GU) pathologists. Twenty hematoxylin and eosin-stained digitized slides from RCCs (all grossly approaching the renal pelvis) were distributed to 31 GU pathologists

Ablative Radioembolization (SIRT) of a Large Inoperable Renal Cell Carcinoma Invading the Liver That Resulted in Hematuria Control and Complete Necrosis by mRECIST Criteria.

We report the successful application of radioembolization (SIRT) in a 77-year-old man with end-stage renal disease on hemodialysis and repeated episodes of macroscopic hematuria due to a large renal cell carcinoma of the right kidney extending to liver segment VI. A compassionate SIRT therapy was performed with resin microspheres through the upper pole renal artery and the feeding segmental artery of liver segment VI. Hematuria was resolved after treatment, and 4 months later, a follow-up CT scan revealed tumor size reduction and complete tumor necrosis (Response Evaluation Criteria in Solid T

Natural History and Genomic Landscape of Chemotherapy-Resistant Muscle-Invasive Bladder Cancer.

PURPOSE: Patients with residual invasive bladder cancer after neoadjuvant chemotherapy (NAC) and radical cystectomy have a poor prognosis. Data on adjuvant therapy for these patients are conflicting. We sought to evaluate the natural history and genomic landscape of chemotherapy-resistant bladder cancer to inform patient management and clinical trials. METHODS: Data were collected on patients with clinically localized muscle-invasive urothelial bladder cancer treated with NAC and cystectomy at our institution between May 15, 2001, and August 15, 2019, and completed four cycles of gemcitabine

Nasoethmoidal Dermoid Cyst With Intracranial Extension. Case Presentation and Surgical Management.

Nasoethmoidal dermoid cysts are rare congenital ectodermal malformations. Unlike other dermoid cysts, those located in the midline can exhibit intracranial extension. Clinically, they may manifest with sebaceous discharge through a fistulous orifice and other morphologic alterations, occasionally presenting with complications at the onset. The authors present a clinical case of a nasoethmoidal dermoid cyst with an intracranial extension, treated with combined interdisciplinary surgical intervention. An adequate imaging evaluation before the surgical planning and complete lesion resection are c

Successful management of a difficult airway in a case of advanced thyroid cancer.

Papillary thyroid carcinoma is the most prevalent endocrine malignancy of the head and neck region. It makes up to 80% of all thyroid cancers, and has a 10-year survival rate of up to 95%. Differentiated thyroid carcinomas have good prognosis after a complete surgical extirpation as long as it is not associated with invasion of the surrounding structures. The advanced papillary thyroid carcinoma can invade the neighbouring structures of the thyroid gland, such as strap muscles, recurrent laryngeal nerve, trachea, oesophagus, larynx, pharynx, and carotids. Whenever papillary thyroid carcinoma i

Evaluating the cost-utility of intravesical Bacillus Calmette-Guérin versus radical cystectomy in patients with high-risk non-muscle-invasive bladder cancer in the UK.

AIMS: Approximately 75% of bladder cancer (BC) cases present as non-muscle-invasive BC (NMIBC). In patients with high-risk NMIBC, the mainstay treatment is intravesical Bacillus Calmette-Guérin (BCG), with immediate radical cystectomy (RC) as an alternative treatment option. The aim of the present study was to evaluate the cost-utility of BCG versus RC in patients with high-risk NMIBC from the UK healthcare payer perspective. MATERIALS AND METHODS: A six-state Markov model was developed that covered controlled disease, recurrence, progression to muscle-invasive BC, metastatic disease, and dea

Conservative treatment for high-risk NMIBC failing BCG treatment: who benefits from adding electromotive drug administration (EMDA) of mitomycin C (MMC) to a second BCG induction cycle?

PURPOSE: Radical cystectomy (RC) is the standard treatment for high-risk non muscle-invasive bladder cancer (NMIBC) failing first BCG treatment. A second BCG course is an option for those patients who refuse RC or are not eligible for it, but its success rate is quite low. Aim of the present study was to determine whether the addition of intravesical electromotive drug administration of mytomicin-C (EMDA-MMC) improved the efficacy of second BCG course. METHODS: Patients with high-risk NMIBC having failed first BCG treatment and having refused RC were offered a second BCG induction course eith
