BACKGROUND: The prevalence of nonmelanoma skin cancer in the United States is alarming. It can be most appropriately treated if the earliest manifestation of cutaneous squamous cell carcinoma is recognized.
METHODS: Clinical, histologic, and molecular biology, considerations were reviewed to determine whether actinic keratosis is the earliest clinical manifestation of cutaneous squamous cell carcinoma.

One and a half years after heart transplantation an Australian man developed his first skin cancer. In the period until his death, 7 years after the transplantation, 34 histologically confirmed squamous cell carcinomas developed, 17 intra-epidermal carcinomas, and 9 basal cell carcinomas. Most skin cancers were confined to chronically sun exposed sites. Exposure to sunlight and human papillomavirus are important factors in the development of skin cancer in renal transplant recipients, and further studies are needed to establish the role of these risk factors in cardiac transplant recipients.

We present a case of hypercalcemia associated with a renal pelvis squamous neoplasm. Severe hyperleukocytosis was also present, resembling a leukaemoid reaction. After failure of standard therapy, we performed a trial of low dose epirubicin in this patient, obtaining a short-term clinical remission. Although a palliative non-toxic effect seems to be achievable with this approach, further studies are needed to ascertain the role of chemotherapy in the long-term control of this condition.

Records were reviewed for 44 patients with primary carcinoma of the ureter. The lesion was transitional cell carcinoma in 40 patients, squamous carcinoma in 3 and unclassified carcinoma in 1. The distal ureter was the site of origin in 73 per cent of the patients. The over-all 5-year survival rate of 67 per cent was influenced by the grade and the stage of the tumors, did not have a significant influence on survival.
