Prospective blind comparative clinical study of two point fixation of zygomatic complex fracture using wire and mini plates.

BACKGROUND: The zygomatic maxillary complex (ZMC) fractures are one of the most frequent injuries of the facial skeleton due to its position and facial contour. Assaults, road traffic accidents and falls are the principal etiologic factors that may cause fractures of zygomatic bone. The different fixation methods are applied to treat the zygomatic bone fractures, with many more classifications which have been described in the literature for the ease of management. The type of the fracture, its severity and associated facial fractures usually interferes the treatment modality. PURPOSE OF STUDY

Does Shorter Time to Treatment of Pediatric Supracondylar Humerus Fractures Impact Clinical Outcomes?

BACKGROUND: Treatment of supracondylar humerus (SCH) fractures within 18 hours of presentation is a tracked quality metric for ranking of pediatric hospitals. This is in contrast with literature that shows time to treatment does not impact outcomes in SCH fractures. We aim to determine whether an 18-hour cutoff for pediatric supracondylar humerus fracture treatment is clinically significant by comparing the complication risks ofpatients on either side of this timepoint. Our hypothesis is that there will be no statistically significant differences based on time to treatment. METHODS: A retrosp

Arthroscopic Screw Fixation Technique for an Isolated Avulsion Fracture of the Lesser Tuberosity.

Isolated avulsion fractures of the lesser tuberosity of the humerus are rare and can be difficult to diagnose. This study presents a fully arthroscopic technique for anatomic reduction and screw fixation of these injuries. The described method allows precise visualization, fragment control, and secure fixation using standard arthroscopic portals and equipment. It also allows simultaneous assessment and treatment of concomitant intra-articular pathologies. This technique offers a minimally invasive alternative to traditional open surgery, providing advantages such as reduced postoperative pain,

A modified submental orotracheal intubation.

In patients with concomitant occurrence of maxillofacial and basilar skull fractures, open reduction and internal fixation is the treatment. It requires intermittent intra operative dental occlusion which precludes oral or nasal intubation. In such cases submental intubation (SMI) is a recognized technique in practice. We describe a modified technique for smooth exteriorization of the endotracheal tube (ETT) during SMI. As the SMI technique is unusual for the performer, emphasis is laid on the applied aspects to minimize probable complications during the procedure. With the modified technique

Supracondylar fractures of the femur.

Twenty-three fractures of the distal femur were treated by open reduction and internal fixation with a compression screw device. Eleven to 34 months of clinical and radiographic follow-up evaluation was available in 19 patients. No nonunions or implant failures and only one deep wound infection occurred. Excellent or good functional and radiographic results were achieved in 74% of patients. Fair results were noted in 16% of patients, and results in 10% were considered failures. The technique is readily mastered and analogous to use of the hip compression screw.

Hypotensive Anesthesia does not reduce Transfusion Rates during and after Acetabular Fracture Surgery.

BACKGROUND: Acetabular fracture open reduction and internal fixation (ORIF) is generally associated with high intraoperative blood loss. Hypotensive anesthesia has been shown to decrease blood loss and intraoperative transfusion in total joint arthroplasty and posterior spinal fusion. In this study, we assessed the effect of reduction in intraoperative mean arterial pressures (MAPs) during acetabular fracture surgery on intraoperative blood loss and need for transfusion. METHODS: Three hundred and one patients with acetabular fractures who underwent ORIF at an academic Level 1 trauma center w

Use of Suture Button in the Treatment of Syndesmosis Injuries.

A suture button device provides fixation of syndesmosis injuries of the ankle with or without concomitant malleolar fracture. The suture button device consists of a fiber thread attached between 2 small metal buttons. The buttons are placed on the cortical bone on the medial and lateral sides of the ankle, and the fiber thread is tightened to stabilize the syndesmosis. The suture button device offers stable yet dynamic fixation of the injured syndesmosis. The major steps of the procedure are (1) lateral skin incision straight to the bone; (2) open reduction and plate fixation of the fibular fr

C-arm assisted zygoma fracture repair: a critical analysis of the first 20 cases.

PURPOSE: Currently used open reduction and internal fixation techniques of zygoma fracture repair are not optimal. Surgical exposure of those sites needed to allow for accurate reduction and for rigid fixation has a high possibility of negative consequences. The objective of the present study was to present a single-incision, single-fixation site zygoma fracture repair technique using a single zygoma c-arm view to quantitatively determine its accuracy, complication rate, and practical aspects in a clinical series. MATERIALS AND METHODS: In a prospective study, consecutive patients with isolat

Utilization of Arthroscopy During Ankle Fracture Fixation Among Early Career Surgeons: An Evaluation of the American Board of Orthopaedic Surgery Part II Oral Examination Database.

BACKGROUND: Rotational ankle fractures are common injuries associated with high rates of intra-articular injury. Traditional ankle fracture open reduction and internal fixation (ORIF) techniques provide limited capacity for evaluation of intra-articular pathology. Ankle arthroscopy represents a minimally invasive technique to directly visualize the articular cartilage and syndesmosis while aiding with reduction and allowing joint debridement, loose body removal, and treatment of chondral injuries. The purpose of this study was to evaluate temporal trends in concomitant ankle arthroscopy during

Timely Fixation of Distal Radius Fractures: Improving Compliance With National Institute for Health and Care Excellence (NICE) and British Orthopaedic Association Standards for Trauma (BOAST) Guidelines.

Background Distal radius fractures are among the most common orthopaedic injuries, accounting for 8-15% of all adult fractures. Timely fixation reduces complications and improves outcomes. National guidelines recommend fixation within 72 hours for intra-articular and within 7 days for extra-articular fractures. This closed-loop audit evaluated compliance with these targets and the impact of targeted service level interventions between audit cycles. Methods A retrospective two-cycle clinical audit was performed at a tertiary trauma centre. The first cycle (March-July 2024) and second cycle (Jan

Locking Plate Use with or without Strut Support for Varus Displaced Proximal Humeral Fractures in Elderly Patients.

UNLABELLED: Varus displaced fractures of the proximal part of the humerus, particularly in osteoporotic bone, commonly require open reduction and internal fixation. However, surgical treatment methods remain controversial and have shown inconsistent results. A fibular allograft for indirect medial reduction and strut support has been used in an effort to prevent secondary postoperative varus displacement. However, the long-term outcomes of this method require confirmation. We hypothesized that placing a fibular strut parallel to the calcar screw could increase the biomechanical stability of th
