Dietary Advice to Support Glycaemic Control and Weight Management in Women with Type 1 Diabetes during Pregnancy and Breastfeeding.

In women with type 1 diabetes, the risk of adverse pregnancy outcomes, including congenital anomalies, preeclampsia, preterm delivery, foetal overgrowth and perinatal death is 2-4-fold increased compared to the background population. This review provides the present evidence supporting recommendations for the diet during pregnancy and breastfeeding in women with type 1 diabetes. The amount of carbohydrate consumed in a meal is the main dietary factor affecting the postprandial glucose response. Excessive gestational weight gain is emerging as another important risk factor for foetal overgrowth…

Ultrasound predictors of adverse outcome in pregnancy complicated by pre-existing and gestational diabetes.

INTRODUCTION: Ultrasound assessment of fetuses subjected to hyperglycemia is recommended but, apart from increased size, little is known abo … ated at the 36-week scan. A composite adverse outcome was defined as the presence of one or more of perinatal death, arterial cord pH less than 7.1, admission to Neonatal Unit, 5-minute Apgar less than 7, severe hypoglycemia, or cesarean section for fetal compromise. A chi-squared test was used to test the association of estimated fetal weight at the 90th centile …

Effects of gestational and pregestational diabetes mellitus on the foetal heart: a cross-sectional study.

We examined the foetal cardiac structural and functional characteristics in diabetic pregnancies versus non-diabetic, healthy pregnancies. Between August 2015 and April 2016, 32 pregnant women with pregestational diabetes, 36 pregnant women with gestational diabetes, and 42 healthy pregnant women were scheduled to have foetal echocardiograms to assess cardiac structure and function. In the diabetic groups, the foetal interventricular septum (IVS) thickness was significantly greater than in non-diabetics (p < .05) but none had an IVS >2 SD from normal. The peak velocity of tricuspid E, and the

[In case of fetal macrosomia, the best strategy is the induction of labor at 38 weeks of gestation].

Macrosomic fetuses are at increased risk of obstetric complications, and notably shoulder dystocia, responsible for a severe neonatal morbidity. In case of fetal macrosomia, three options are: (i) the elective cesarean delivery, but this is recommended only when the estimated fetal weight is≥4500g for diabetic women and 5000g for non-diabetic women; (ii) the expectative management, but children with birth weight≥4500 had significantly increased risk of perinatal mortality, neonatal asphyxia, trauma, and cesarean delivery; (iii) the induction of labor which, reducing the possibility of fetal gr

Two pregnancies after preimplantation genetic diagnosis for osteogenesis imperfecta type I and type IV.

Osteogenesis imperfecta (OI) is an autosomal dominant genetic disorder characterized by the presence of brittle bones and decreased bone mass (osteopenia), as a result of mutations in the genes that encode the chains of type I collagen, the major protein of bone. The clinical features of the disease range from death in the perinatal period to normal life span with minimal increase in fractures. The present report describes two polymerase chain reaction (PCR)-based assays allowing preimplantation genetic diagnosis (PGD) on the one hand for OI type I, the mildest form, and on the other hand for

The impact of the definition of preeclampsia on disease diagnosis and outcomes: a retrospective cohort study.

BACKGROUND: The diagnostic criteria for preeclampsia have evolved from the traditional definition of de novo hypertension and proteinuria to a broader definition of hypertension with evidence of end-organ dysfunction. Although this change is endorsed by various societies such as the International Society for the Study of Hypertension in Pregnancy and the American College of Obstetricians and Gynecologists, there remains controversy with regard to the implementation of broader definitions and the most appropriate definition of end-organ dysfunction. OBJECTIVE: This study aimed to assess the im

Maternal calcium, phosphorus, and supplement intake before and during pregnancy and their association with preterm birth risk: based on a large cohort study.

BACKGROUND: Preterm birth (PTB) is a global epidemic, defined as delivery before 37 weeks of gestation, and is an important risk factor for neonatal death, morbidity and abnormal childhood development. Premature birth is currently regarded as a complex disease influenced by multiple factors. Common risk factors include nutritional deficiency during pregnancy, maternal obesity, environmental exposure, infection and inflammation, among which maternal nutrition during pregnancy is an important modifiable factor. OBJECTIVE: To assess the relationship between maternal dietary calcium, phosphorus i

Hepatitis B Seroprevalence and Associated Factors Among Inpatients from 2014 to 2023: A Single-Centre Retrospective Study in a Malaysian Tertiary Hospital.

BACKGROUND: Hepatitis B virus (HBV) remains a global health burden, causing significant health problems, including morbidity and mortality. The World Health Organization (WHO) aims to reduce new infections by 90% and deaths by 65% by 2030, with a focus on the Western Pacific and Southeast Asia. In Malaysia, HBV incidence rose from 2.26 per 100,000 in 2010 to 12.65 per 100,000 in 2015, classifying the country as having an intermediate burden with a seroprevalence of 1.5% to 9.8%. The major transmission routes include perinatal transmission, blood exposure, sexual contact, and needle sharing. Th

Prevalence and Determinants of Home Delivery among Women with Easy Access to Health Facilities in Sub‑Saharan African Countries: A Multi‑level Mixed Effect Analysis.

Introduction: Most maternal deaths are associated with home deliveries, which account for half of births in low‑income countries. To develop appropriate policies and methods that could aid in addressing the issues, it is important to understand the burden of home delivery despite having easy access to health facilities in low‑income nations such as sub‑Saharan Africa. In addition, identifying and prioritizing determinants could help executives to review their perinatal policies. Therefore, this study aimed at assessing the prevalence and factors associated with home delivery among women who ha

A Clinical Practice Guideline for the Management of Pregnancy Alloimmunized to Red Blood Cell Antigens.

IMPORTANCE: Red blood cell alloimmunization is typically associated with the transplacental transfer of incompatible fetal red blood cells into maternal circulation. Subsequent pregnancies can be affected by fetal anemia, hydrops fetalis, and perinatal death. Most cases of Rhesus D (RhD) alloimmunization due to pregnancy can be prevented by the proper administration of Rhesus immune globulin. However, an emerging practice of using low-titer, O, RhD-positive whole blood (LTOWB) in cases of life-threatening hemorrhage has the potential to increase the exposure of the female population to a new s…

Is severe maternal morbidity a risk factor for postpartum hospitalization with mental health or substance use disorder diagnoses? Findings from a retrospective cohort study in Maryland: 2016-2019.

BACKGROUND: Perinatal mental health conditions and substance use are leading causes, often co-occurring, of pregnancy-related and pregnancy-associated deaths in the United States. This study compares odds of hospitalization with a mental health condition or substance use disorder or both during the first year postpartum between patients with and without severe maternal morbidity (SMM) during delivery hospitalization. METHODS: Data are from the Maryland's State Inpatient Database and include patients with a delivery hospitalization during 2016-2018 (n = 197,749). We compare rate of hospitaliza

Confusing cause and effect in public health policy.

When studying the literature on current public health policy challenges, a number of rankings can be found, created from data collected and processed by different methods and from different sources, ordering the challenges by their significance. Inevitably, in global studies, there are errors caused by averaging data across populations that differ significantly in population size and the causes of the decline in disability-adjusted life years (DALYs), potentially in combination with years of life lost (YLL) and years of life lived with disability (YLD), that may serve as indicators of the heal
