The difficult elderly patient: curable hostile depression or personality disorder?

The aim of this article is to suggest that hostile behavior in elderly patients is often caused by a reversible mood disorder rather than by a personality disorder or dementia, even if these two last diagnoses are already well established. Sedatives, often prescribed for hostile behavior, can induce confusion, falls, and bedridden states, and can actually increase the rate of mortality. The mood disorder can be a hostile agitated depression or a mixed affective state. The hypothesis of a mood disorder calls for an antidepressant trial, possibly combined with a mood stabilizer.

Psychiatric symptomatology in crack cocaine abusers.

Many substance abusers present with elevated scores on self-report measures of psychopathological symptoms. We decided to examine this in a sample of 194 crack cocaine abusers. Results indicated that these individuals reported significantly greater psychopathological distress than manual-provided norms, and that this distress was not related to recency of cocaine use. Rank order correlation suggests that a pattern of scores specific to cocaine abusers may exist. Finally, while intake scores of psychopathological distress did not predict treatment retention/outcome, those successfully completin

Visual hallucinations in patients from an ophthalmology clinic and medical clinic population.

Although visual hallucinations have been associated with patients with visual disorders, no study has specifically examined whether visual hallucinations are indeed more prevalent than in a general medical population. In this study, 127 consecutive visual disorder patients and 100 consecutive general medical patients were screened for complex visual hallucinations. A total of 6.3% of visual disorder patients and 2% of general medical patients had visual hallucinations. Interestingly, the two medical patients with visual hallucinations also had visual disorders. Factors significantly associated

Comorbidity of substance abuse and schizophrenia: the role of pre-morbid adjustment.

Co-morbid substance use and abuse is common in schizophrenic patients, and the role of substance abuse in initiating and maintaining psychosis has important definitional and aetiological implications. We investigated the issue in a cohort of 131 schizophrenic patients. We found non-users (N = 67) were similar to pathological users (N = 64) in current symptomatology and clinical history. The pathological users did, however, have better pre-morbid adjustment levels. Only alcohol use and to some extent cannabis use contributed to this effect; use of stimulants or hallucinogens did not. These resu

[Schizophreniform conditions in adolescent drug users differentiation possibilities using objective personality inventories].

The authors report on schizophreniform states in juvenile drug consumers. The following psychodiagnostic instruments have been used in this investigation: a list of complaints, a self-estimation scale, a psychosis-depression-neurosis inventory (v. Zerssen), the Freiburg personality inventory (FPI, Fahrenberg, Selg and Hampel). According to clinical-psychopathological data in the course of psychosis, three groups of patients (N = 21) have been formed and defined as schizophrenia-, borderline-, intoxication-psychosis-groups. The report describes the procedure of two discriminant analyses by whic

Functional Brain Activity Associated With Intermittent Rhythmic Delta/Theta Activity: A Transdiagnostic Electroencephalography-Functional Magnetic Resonance Imaging Resting-State Study.

BACKGROUND: Intermittent rhythmic delta/theta activity (IRDA/IRTA) detected via electroencephalography (EEG) has been implicated in the pathophysiology of neuropsychiatric illnesses. Therefore, a combined EEG and functional magnetic resonance imaging (fMRI) approach was applied in a transdiagnostic group of patients with different causalities, i.e., autoimmune-mediated (in suspected autoimmune psychiatric syndromes [APS]) and primary psychiatric (borderline personality disorder [BPD]) causalities, as well as in healthy control (HC) participants, to characterize the brain regions functionally c

Prevalence and clinical correlates of intermittent explosive disorder in Turkish psychiatric outpatients.

OBJECTIVE: Intermittent explosive disorder (IED) is defined as the failure to resist aggressive impulses resulting in repeated acts of verbal and/or physical aggression. Although it is frequently encountered in clinical psychiatric practice, there is a paucity of data concerning IED in the scientific literature both internationally and in Turkey. The aim of this study was to evaluate the prevalence of IED and associated sociodemographic and clinical features in a clinical setting. METHODS: A total of 406 patients who were referred to our psychiatry outpatient clinic for the first time in a si

[Emergence of early childhood trauma in adult psychiatric symptomatology].

Trauma comes as a result of the subject's exposure to extremely negative and stressful events, such as natural or human-provoked catastrophes, wars, serious injuries, violent deaths, tortures, terrorist attacks, rapes and other sexual crimes. A child's exposure to traumatic circumstances of this level during the crucial period of self-structuring creates rather difficult conditions for its development. Moreover, if the child does not have the opportunity to elaborate and analyze all these stressful conditions and put them into words, serious consequences, both psychological and somatic, may oc

Dissociative experiences in obsessive-compulsive disorder and trichotillomania: clinical and genetic findings.

A link between dissociation proneness in adulthood and self-reports of childhood traumatic events (including familial loss in childhood, sexual/physical abuse and neglect) has been documented. Several studies have also provided evidence for an association between dissociative experiences and trauma in patients with various psychiatric disorders, including post-traumatic stress disorder, borderline personality, dissociative identity and eating disorders. Based on the relative paucity of data on dissociation and trauma in obsessive-compulsive disorder (OCD) and trichotillomania (TTM), the primar

Modeling the association between and predictors of two constructs of resilience.

PURPOSE: Resilience serves as a protective factor against adverse outcomes following exposure to traumatic events. The extant literature focuses on psychiatric resilience in the context of internalizing symptoms, though resilience is also important in relation to externalizing symptoms. Research is needed to clarify the predictors of resilience across contexts. The aims of the current study are twofold: 1. Determine the association between psychiatric resilience (PR) and alcohol resistance (AR) and 2. Test for differential prediction of each form of resilience by exogenous predictors. METHODS

Acoustic Analysis of Speech for Screening for Suicide Risk: Machine Learning Classifiers for Between- and Within-Person Evaluation of Suicidality.

BACKGROUND: Assessing a patient's suicide risk is challenging for health professionals because it depends on voluntary disclosure by the patient and often has limited resources. The application of novel machine learning approaches to determine suicide risk has clinical utility. OBJECTIVE: This study aimed to investigate cross-sectional and longitudinal approaches to assess suicidality based on acoustic voice features of psychiatric patients using artificial intelligence. METHODS: We collected 348 voice recordings during clinical interviews of 104 patients diagnosed with mood disorders at bas

Stereotactic radiation therapy for oligometastases or oligorecurrence within mediastinal lymph nodes.

BACKGROUND: Clean intermittent catheterization (CIC) is an established treatment for lower urinary tract dysfunction and has been used across age groups with voiding problems due to various etiologies. Although medically safe and straightforward, CIC may create social and emotional challenges for adolescents and their caregivers. During adolescence, when autonomy and peer acceptance become central, the need for continuous catheterization in daily life and, at times, dependence on another person can be difficult to cope with and may affect psychological well-being and peer relations. Psychopathological and psychosexual profile and therapy for patients with androgen insensitivity syndrome: a narrative review.

PURPOSE: The objective of this review is to present androgen insensitivity syndrome as a multidimensional disorder, with a focus on psychological issues, gender identity, and sexual functioning. It also discusses therapeutic options, such as psychological and psychiatric support. VIEWS: Androgen insensitivity syndrome is a rare disorder of sex development that occurs in individuals with an XY karyotype. It is caused by a mutation in the androgen receptor gene, leading to reduced sensitivity to androgens. There are three forms: mild, partial, and complete androgen insensitivity syndrome. Indiv

Needs of Veterans With Personality Disorder Diagnoses in Community-Based Mental Health Care.

BACKGROUND: Recent research suggests veterans are more likely to experience personality disorders (PDs) relative to the general population. However, due to longstanding stigma surrounding these conditions, research into the clinical and psychosocial needs of veterans with PDs has been greatly limited. METHODS: Data from a sample of 104,198 veterans receiving community-based, state-funded mental health services in 2022 was used for this study. The dataset included demographic, comorbid clinical needs, and psychosocial outcome patterns among veterans with and without documented PD diagnoses. R

Psychological Morbidity in Chronic Dermatological Disorders: A Review.

Skin diseases not only manifest physically but also significantly impact mental and emotional well-being due to the close biological connection between the skin and brain. Psychodermatology, a field that bridges dermatology and psychiatry, addresses the psychological aspects of dermatological disorders, which can profoundly affect quality of life. This review explores the psychological morbidity associated with common dermatological conditions, including acne vulgaris, alopecia areata, atopic dermatitis, hidradenitis suppurativa, leprosy, melasma, psoriasis, urticaria, and vitiligo. Heterogeneity of dementia with lewy bodies: Insights from clinical presentations, neuropathology, and biomarkers.

BACKGROUND: Dementia with Lewy Bodies (DLB) is a neurodegenerative disorder characterised by α-synuclein pathology, causing cognitive decline, motor and non-motor symptoms. This review explores the clinical and neuropathological heterogeneity of DLB, which complicates its early diagnosis, prognosis, and treatment. The staging of Lewy body (LB) pathology varies, with both brain-first and body-first hypotheses suggesting different origins and pathways for disease progression. Co-pathologies, such as amyloid-β plaques, tau tangles, and cerebrovascular changes, further influence the clinical prese

Psychedelic Use and Behavioral Addictions.

Recent clinical trials on classic psychedelics for depression and anxiety have raised questions about their potential to treat other psychiatric conditions. Evidence from pre-clinical and clinical research suggests psychedelics may help with substance use disorders, with ongoing registered trials. Behavioral addictions, sharing features with substance addictions, may also respond to psychedelic treatment. This study examined self-reported symptoms of problem gambling, sexual addiction, compulsive buying, and eating disorders in a community sample (N = 1107), alongside measures of well-being (s

Comorbidity of bipolar disorder and borderline personality disorder: Phenomenology, course, and treatment considerations.

AIM: Bipolar disorder (BD) and borderline personality disorder (BPD) are both serious psychiatric conditions that elevate the risk for harmful outcomes. Although these conditions represent distinct diagnostic entities, existing research suggests that approximately 20% of individuals with BD meet the criteria for comorbid BPD. Individuals with comorbid BD/BPD appear to have a markedly more severe and phenomenologically distinct clinical course when compared with those with BD alone. However, treatments have generally not been tested in this specific population, and currently, no formal treatmen
