Inflammasome biology in preeclampsia: from Bnip3-mitophagy to Nlrp1-driven placental injury - a systematic review and translational roadmap.

INTRODUCTION: Although inflammasome activation has been repeatedly linked to preeclampsia, the field has tended to frame this biology around NLRP3 alone, leaving other sensors - particularly NLRP1 - and their mitochondrial upstream signals only partially examined. Recent experimental work hints at a more intricate narrative in which dysregulated BNIP3-mediated mitophagy and escalating mitochondrial ROS form a convergent pathway toward trophoblast injury. Yet no systematic review has stitched these elements together. CONTENT: Following PRISMA 2020 guidance, we synthesized evidence across exper

Longitudinal 3D-DCE MRI Assessment of Placental Perfusion Under Reduced Uterine Perfusion Pressure in Pregnant Rats.

Placental insufficiency is a major cause of preeclampsia and fetal growth restriction. The reduced uterine perfusion pressure (RUPP) rat model reproduces cardinal features of these disorders, but longitudinal changes in placental perfusion after RUPP are poorly documented. This study aimed to assess the acute and late effects of RUPP on placental perfusion between gestational day (GD) 14 and GD18 using 3D dynamic contrast-enhanced magnetic resonance imaging (3D-DCE MRI). Pregnant Sprague Dawley rats (n = 31) were randomized to RUPP or normal pregnant (NP) groups on GD14. 3D-DCE MRI scans were

Placental CYP450 gene dysregulation by maternal PFAS: a novel mechanism linking prenatal exposure to altered toddler growth.

BACKGROUND: Maternal per- and polyfluoroalkyl substances (PFAS) exposure has been linked to adverse health effects on offspring, but the mechanisms remain unclear. The present study investigates the relationship between maternal PFAS exposure and the expression of placental cytochrome P450 enzymes (CYP19A1, CYP2J2, and CYP2E1), and explores the potential role of these enzymes in linking maternal PFAS exposure to offspring development. METHODS: We included 350 mother-infant pairs from the Jiashan birth cohort. Thirteen PFAS compounds were measured in maternal plasma collected at 8-16 weeks of

Decoding psychotic and other psychiatric disorders through placental organ: From cytoarchitecture to stress-responsive signaling networks.

Psychosis-defined as a profound disconnection between objective external reality and internal mental representations-represents a critical mental health condition, especially when it emerges for the first time during pregnancy. First-episode psychosis (FEP) in pregnant women (FEP-PW), though rare, presents severe risks for both maternal and fetal health. Despite its clinical significance, the biological mechanisms underlying FEP-PW remain poorly understood. This narrative review investigates the human placenta as a valuable and dynamic organ for decoding psychiatric and psychological disorders

Maternal and perinatal placental dysfunction severity in extremely high values of sFlt1/PlGF ratio: A retrospective observational study.

OBJECTIVE: The soluble fms-like tyrosine kinase-1 (sFlt1)/placental growth factor (PlGF) ratio can rule out pre-eclampsia (PE) when negative, but the precise significance of extremely high values is not well known. This study sought to determine the median interval between obtaining an sFlt1/PlGF ratio ≥ 500 and delivery in order to clarify its prognostic contribution. METHODS: Between 2014 and 2021, data were collected from singleton ongoing pregnancies with an sFlt1/PlGF ratio ≥ 500 at Nantes University Hospital. Features able to influence the interval between obtaining an sFlt1/PlGF ratio

The role of placental anastomoses in severe brain injury after fetal demise in monochorionic twins: a multicenter retrospective study.

BACKGROUND: Monochorionic twins share a single placenta with vascular anastomoses including arterio-arterial and veno-venous anastomoses, which are low-resistance, superficial, and bidirectional anastomoses. Following single fetal demise, the survivor is at risk of brain injury due to acute exsanguination through arterio-arterial and veno-venous anastomoses into the low-pressure circulation of the deceased co-twin. OBJECTIVE: To assess whether the number and diameter of arterio-arterial and veno-venous anastomoses are associated with severe brain injury in the survivor following single fetal

The Cardiac-Fetal-Placental Unit: links between umbilical venous flow and maternal hemodynamics in fetal growth restriction.

Fetal growth restriction (FGR) is associated with significant perinatal morbidity and mortality, yet its identification remains challenging due to the limited sensitivity of conventional biometric assessment and the lack of a reliable tool for assessing placental function. Placental function depends on uteroplacental perfusion which, in turn reflects maternal cardiovascular adaptation to pregnancy. In this context, umbilical venous flow (UVF) has emerged as a quantitative parameter reflecting fetal blood supply and a potential surrogate marker of placental function. This narrative review aims

Polystyrene nanoplastics impair placental function and promote embryo resorption through ROS-mediated ferroptosis-related injury.

Polystyrene nanoplastics (PS-NPs) are widespread environmental pollutants that have attracted growing concerns regarding reproductive health. Nevertheless, their effects on spontaneous abortion (SAB) and the underlying mechanisms remain unclear. In this study, we investigated the reproductive toxicity of 50 nm PS-NPs using a pregnant mouse exposure model and HTR-8/SVneo trophoblast cells. In vivo, PS-NPs crossed the placental barrier, disrupted placental structure, altered pregnancy-related hormone levels, and increased embryo resorption. In vitro, PS-NPs reduced trophoblast cell viability and

Placental extracellular vesicles stimulate intestinal re-growth after massive small bowel resection in a rat model.

Extracellular vesicles (EVs) are released by most cells, including stem cells, into the extracellular environment and regulate cell function and tissue regeneration through cell-cell communication. The EVs derived from human placenta have a crucial role in regulating the maternal immune response for a successful pregnancy outcome. The purpose of the current study was to evaluate the effects of human villous trophoblasts (HVT)-derived EVs on structural intestinal adaptation, cell proliferation and apoptosis in a rat model of short bowel syndrome (SBS). HVT-EVs were isolated from cell media afte

Multiscale computational modeling to quantify how spiral artery remodeling alters wall shear stress on placental villi.

Proper placental development is essential for a healthy pregnancy. It depends on the remodeling of the maternal uterine vasculature to meet fetal demands while maintaining physiological intervillous space (IVS) hemodynamics for biochemical exchange. Terminal villi, the primary sites of feto-maternal exchange, exhibit impaired development in pregnancies complicated by intrauterine growth restriction and preeclampsia, which are also associated with incomplete spiral artery (SA) remodeling. Despite this association, the mechanistic link between maternal blood flow and villous development remains

Placental endoglin protein expression across gestation in normal pregnancy and preeclampsia: Implications for anti-angiogenic therapeutic strategies.

INTRODUCTION: Preeclampsia is a human-specific hypertensive disorder of pregnancy characterized by new-onset hypertension and proteinuria after 20 weeks of gestation. Placental dysfunction and angiogenic imbalance are central to its pathophysiology. Endoglin, a membrane-bound glycoprotein and precursor of soluble endoglin (sEng), contributes to impaired placentation. This study examined placental endoglin protein expression across normal gestation and compared it with expression in preeclampsia. METHODS: Placentas from normotensive pregnancies across all three trimesters and from women with p…

Placental iron-driven oxidative imbalance and changes in iron homeostasis in diabetes-in-pregnancy: A cross-sectional analytical study.

INTRODUCTION: Diabetes in pregnancy (DIP) is associated with adverse maternal and fetal outcomes. Placental iron homeostasis is tightly regulated to ensure adequate fetal supply while preventing oxidative injury. Dysregulated iron metabolism may promote ferroptosis. Altered placental iron homeostasis in DIP may reduce the Placental Iron Deficiency Index, thereby compromising fetal iron endowment. The study evaluated alterations in placental iron homeostasis and ferroptosis-related pathways in DIP compared with healthy pregnancies (HP). METHODS: In this cross-sectional analytical study, 120 pr

Heterogeneous graph neural networks reveal molecular mechanisms of folate deficiency in placental insufficiency through multiomics integration.

Placental insufficiency affects five to ten% of pregnancies worldwide, and folate deficiency has emerged as a key contributing factor. The molecular pathways linking folate metabolism to placental pathology remain poorly characterized. We developed a heterogeneous graph neural network framework that integrates genomic, transcriptomic, proteomic, and metabolomic data to investigate these mechanisms. Our network architecture explicitly models diverse node types and edge relationships within biological networks, addressing limitations of conventional approaches that treat molecular entities unifo

IGF-1Eb Isoform Immunoreactivity May Be Associated with Placental Dysfunction and Vascular Pathology in Fetal Growth Restriction (FGR).

BACKGROUND: Fetal growth restriction (FGR) is associated with placental dysfunction and adverse perinatal outcomes. Although insulin-like growth factor-1 (IGF-1) signaling is important for placental and fetal development, the mRNA expression, immunopositivity and potential biological significance of specific IGF-1 isoforms in FGR remain incompletely characterized. This study investigated placental IGF-1Eb mRNA expression and immunopositivity in FGR pregnancies compared with appropriate-for-gestational-age (AGA) pregnancies. METHODS: A total of 62 third-trimester human placentas were analyzed,

Trimester-dependent vertical transmission of H5N1 influenza virus through placental and mammary routes impairs offspring development.

Avian influenza H5N1 has pandemic potential and historically causes more severe disease in pregnant women than the general population. With increasing transmission of H5N1 detected among placental mammals, animal models are necessary for testing countermeasures, including during pregnancy. Pregnant outbred mice infected with a contemporary strain of bovine H5N1 during the second trimester equivalent causes in utero transmission, with infectious virus detected in the uterus, placenta, and fetus. Birth following third trimester infection results in offspring with decreased size, neurodevelopment

Distinct immune responses to HIV and CMV in Hofbauer cells across gestation highlight evolving placental immune dynamics.

BACKGROUND: Hofbauer cells are fetal macrophages in the placental villous core that contribute to maternal-fetal tolerance and antiviral defense, but how their immune programs change across gestation in response to viral exposure remains poorly understood. METHODS: Primary Hofbauer cells were isolated from early- to mid-gestation and term human placentas and exposed in vitro for 24 h to HIV-1 or cytomegalovirus (CMV), together with mock controls. Transcriptional responses were profiled by bulk RNA sequencing using differential expression, pathway enrichment, and co-expression network analyses

Impairment of placental function and maternal-fetal immunity cross-talk by per- and polyfluoroalkyl substances during early pregnancy.

Per- and polyfluoroalkyl substances (PFAS) are persistent environmental pollutants that have been associated with adverse pregnancy outcomes and immune dysfunction. However, the impact of PFAS on trophoblast-mediated immune interactions at the maternal-fetal interface remains unclear. In this study, we evaluated the effects of human-relevant PFAS on trophoblast function and immune crosstalk using a three-dimensional (3D) trophoblast spheroid model and a co-culture system with CD4+ T cells. JEG-3 spheroids were exposed to multiple PFAS, including perfluorooctane sulfonate (PFOS), perfluorooctan

Environmentally relevant polyethylene terephthalate (PET) microplastics induce functional and oxidative alterations in sheep placental cells In vitro.

Ruminants are among the livestock most at risk of microplastics (MPs) exposure due to the ingestion of contaminated forage, particularly with polyethylene terephthalate (PET). Yet, despite this high risk, the potential effects of microplastics on reproductive function in these species remain largely unknown. Here, we studied how environmentally relevant PET-MPs impact placental functionality and cellular responses in an in vitro sheep model. First, virgin PET was mechanically processed through progressive milling and sieving to obtain two fractions of 20-50 μm or <20 μm, which were characteris
