Ultrasound-guided bilateral paravertebral continuous nerve blocks for a mildly coagulopathic patient undergoing exploratory laparotomy for bowel resection.

Regional anesthesia techniques commonly utilized in post-operative pain management are often considered contraindicated in coagulopathic patients. We report on successful postoperative pain control utilizing peripheral nerve blockade after exploratory laparotomy with small bowel resection in a mildly coagulopathic patient. In our case, complicated by abnormal PT, PTT and INR, a thromboelastogram (TEG) was performed before the procedure and found to be normal. An ultrasound-guided bilateral paravertebral blockade with continuous paravertebral catheters was then performed in this pediatric patie

Follow-Up on Percutaneous Vertebroplasty Using PMMA in Osteoporotic Patients.

We reviewed the follow-up findings of patients who received percutaneous vertebroplasty, in order to understand the effects of vertebroplasty on following issues: pain relief, height restoration, wedge deformity correction, kyphosis correction, and other radiological findings found in follow up studies, such as new fractures. Generally, the pain relieving effect after vertebroplasty is long lasting except when new fracture occurred, or presence of other problems that caused the back pain. Height restoration and wedge deformity correction last for more than one year. The effect of kyphosis angl

Technique of microvascular decompression. Technical note.

Vascular compression of the trigeminal nerve in the cerebellopontine angle is now generally accepted as the primary source or "trigger" causing trigeminal neuralgia. A clear clinicopathological association exists in the neurovascular relationship. In general, pain in the third division of the trigeminal nerve is caused by rostral compression, pain in the second division is caused by medial or more distant compression, and pain in the first division is caused by caudal compression. This discussion of the surgical technique includes details on patient position, placement of the incision and cran

[General practice of lumbosacral peridural infiltrations in rheumatology: considerations based on a review of the literature].

Recommendations about the practice of epidural steroid injections for low back pain and sciatica are formulated based on an extensive review of the published literature. These infiltrations should be reserved to trained physicians observing strictly aseptic conditions. Their indications are limited to the few special situations where classical treatments have failed, and only after exclusion of an infectious or tumoral process. To the present time no conclusive data have been published to prove that these procedures reduced the need for surgery in the case of herniated nucleus pulposus or spin

Selective hypophysectomy for metastatic pain. A review of ethyl alcohol ablation of the anterior pituitary in a Regional Pain Relief Unit.

Selective adenohypophysectomy, by the injection of up to 1 ml of absolute alcohol to the pituitary gland, was used to produce pain relief in 25 patients suffering from metastatic cancer. Thirty-five treatments were given and good analgesia was obtained in 74%. The median duration of pain relief was 6-7 weeks, although some treatments produced relief for up to 20 months. The procedure was most successful in relieving bone pain in patients with advanced carcinoma of the breast or prostate. The use of a fine-gauge needle for the intranasal trans-sphenoidal approach to the gland and the injection

Buttock necrosis following uterine artery embolization: a case report.

BACKGROUND: Uterine artery embolization (UAE) is an increasingly common procedure for the management of uterine leiomyoma (fibroid). Up to 70% of White women and more than 80% of women of African ancestry will develop benign fibroid tumors of the uterus, many of whom will have symptoms including increased menstrual bleeding, urinary symptoms, pain, and infertility. When medical management does not adequately treat symptoms, UAE is a minimally invasive surgical treatment option. Nontarget embolization is a rare complication of this procedure and can result in soft tissue necrosis. Few case repo

Vertebral Metastasis Treated by Vertebroplasty, a Cause of Respiratory Failure: Case Report and Literature Review.

Bone is a frequent site of metastases in advanced cancers including lung, breast, prostate, kidney, or myeloma. Lesions are commonly located on the spine. Neoplastic invasion of the vertebral body can result in painful vertebral fractures, leading to disability and substantial morbidity. Percutaneous vertebroplasty is a minimally invasive surgical procedure used to treat spinal fractures due to osteolytic tumors. It could result in pain reduction or resolution in 80-90% of patients with fractures, and it improves stability. Although considered safe, vertebroplasty has been associated over the

Treatment of a Heavily Calcified Celiac Artery Ostial Subtotal Occlusion Using Shockwave Lithotripsy: A Novel Approach.

PURPOSE: We present the novel use of adjunctive intravascular lithotripsy (IVL) before definitive intravascular stenting of a heavily calcified celiac artery ostial occlusion. CASE REPORT: A 79-year-old woman presented with chronic post-prandial abdominal pain and weight loss. Selective angiography revealed a sub-totally occluded celiac artery. Percutaneous endovascular intervention of the celiac artery was attempted but was unsuccessful because of heavy calcification. The patient returned for a repeat procedure. A guidewire was successfully advanced across the sub-totally occluded ostium. A

Please spare my teeth! Dental procedures and trigeminal neuralgia.

BACKGROUND: The correct diagnosis of trigeminal neuralgia (TN) is still a far cry and the patients suffer from unnecessary dental procedures before getting the definite treatment. In this study, we evaluated, if the patients have undergone dental procedures for their misdiagnosed TN before receiving definite treatment for the same. METHODS: A total of 187 patients received GKRS for their TN (excluding secondary TN) in two institutes from 2010 to 2019. We did a retrospective analysis of these patients' primary complaints on a standard questionnaire. RESULTS: One hundred and seventeen of the 1

Upper lip frenotomy for neonatal breastfeeding problems.

OBJECTIVES: Upper lip tie, without concomitant tongue tie, can prevent proper flanging of the upper lip during breastfeeding, resulting in a poor seal and suck for the infant with nipple pain and maternal dissatisfaction. Due to the lack of published studies on this subject, we report our technique and outcomes for in-office release of isolated upper lip tie. METHODS: Using CPT Code 40,806 for 'incision of labial frenulum', 22 mother-infant dyads with infant age under 60 days with breastfeeding problems and a restrictive upper lip frenum were identified. These infants underwent in-office rele

Endoscopy-Assisted Diagnosis and Revision of a Malpositioned Screw.

BACKGROUND: Pedicle screw insertion is a common procedure in spine surgery, and freehand, fluoroscopic, and robotic-assisted techniques all are used. These are indirect methods that use fluoroscopy, and direct visualization of canal involvement has not been possible. However, owing to the development of high-definition imaging modalities, delicate procedures that use endoscopy are possible. CASE DESCRIPTION: A 47-year-old man presented with severe radiating pain in his leg after undergoing L5-S1 level endoscopic transforaminal lumbar interbody fusion and percutaneous pedicular screw fixation.

Local infiltration of liposome bupivacaine in foot and ankle surgery: case-based reviews.

Foot and ankle surgical procedures, ranging from simple procedures, such as bunionectomy and correction of hammer toe, to more complex surgery, such as ankle fusion and ankle replacement, are extremely painful. Moreover, there is increasing interest in performing these procedures in an outpatient setting. Nerve blocks are extensively used in foot and ankle surgery, and commonly used techniques include sciatic nerve block with saphenous nerve augmentation; ankle block; and local, digital, or field block. Whereas more extensive blocks are associated with increased medical risk, higher cost, and

Meniscectomy.

To review the meniscus from a historical perspective especially on surgical management and general guidelines for arthroscopic meniscectomy procedures for various types of meniscal tears. We searched MEDLINE and PubMed for the years of 1980-2010 using the terms meniscus, meniscal repair, menisectomy, and arthroscopy. Orthopedic surgeons frequently encounter patients with pain or functional impairment of the knee joint and repair or resection of the injured meniscus is one of the most common orthopedic operative procedures. The object of meniscal surgery is to reduce pain, restore functional me

"Minimally invasive" lumbar spine surgery: a critical review.

BACKGROUND: Minimal-access technology has evolved rapidly with "tubular" or "percutaneous" approaches for decompression and stabilization in the lumbar spine. Potential benefits (smaller scars, diminished local pain, reduced blood loss, reduced postoperative wound pain, shorter hospital stays) have to be weighed against possible drawbacks (reduced orientation, steep learning curve, increased radiation exposure, dependency on technology, cost). While non-comparative case series are often rather enthusiastic, comparative studies and particularly RCTs are scarce and might convey a more realistic

Trochlear nerve palsy after repeated percutaneous balloon compression for recurrent trigeminal neuralgia: case report and pathogenic considerations.

OBJECTIVE AND IMPORTANCE: Repeated percutaneous balloon compression for the treatment of idiopathic trigeminal neuralgia is infrequent. When a second procedure is performed, the outcome is unknown. A patient developed an isolated trochlear nerve palsy after undergoing percutaneous trigeminal ganglion balloon compression for a second time. The mechanism of diplopia and the complications associated with this technique were studied. CLINICAL PRESENTATION: The patient was a 67-year-old woman with a history of medically refractory idiopathic trigeminal neuralgia involving all three divisions of th
