Reduced creatinine clearance in primary osteoporosis in women.

We determined the relationship between bone mass and age, anthropometric variables, and serum and urine biochemical variables in 77 normal white women and 37 women with primary osteoporosis, 25 of whom had one or more vertebral compression fractures. Skeletal status was assessed by radiography of the hands with measurement of combined cortical thickness of the second metacarpal bones (CCT) or measurement of radial and lumbar bone density, or by both methods. Radial bone mineral content (RBMC) was measured by single-photon absorptiometry (SPA) and lumbar bone mineral density by dual-photon abso

Abnormal Trabecular and Cortical Bone Microarchitecture in Chronic Hepatitis C Infection and Associations With Select Inflammatory Cytokines.

BACKGROUND: Hepatitis C virus (HCV) infection is associated with reduced bone mineral density (BMD) and increased fracture risk. The structural underpinnings for skeletal fragility with HCV and contributions of inflammatory cytokines remain unknown. We used high-resolution peripheral quantitative computed tomography (HR-pQCT) to compare skeletal parameters by chronic HCV. METHODS: We conducted a cross-sectional study among 58 participants with chronic HCV and 58 participants without HCV. Volumetric BMD and cortical dimensions of the radius and tibia were determined by HR-pQCT; visceral fat ar

Regular Proton-Pump Inhibitor Intake is Associated with Deterioration of Peripheral Bone Mineral Density, Microarchitecture, and Strength in Older Patients as Assessed by High-Resolution Peripheral Quantitative Computed Tomography (HR-pQCT).

Proton-pump inhibitors (PPI) are widely used among older adults and PPI intake has been associated with increased fracture risk. However, their association with bone microarchitecture and strength remains unclear as prior clinical PPI studies were restricted to lower-resolution bone imaging techniques such as DXA. Using high-resolution peripheral computed tomography (HR-pQCT), we prospectively investigated whether long-term PPI use was associated with changes in volumetric bone mineral densities, bone microarchitecture, and strength in older patients. 189 patients aged ≥ 60 years (n = 24 persi

Deficits in bone strength, density and microarchitecture in women living with HIV: A cross-sectional HR-pQCT study.

PURPOSE: With the advent of combined antiretroviral therapy (cART), life expectancy has increased among persons living with HIV, but so too has risk for comorbidities including osteoporosis and fragility fracture. To explore whether HIV status and cART influence three-dimensional measures of BMD, bone microarchitecture and strength we aimed to compare these outcomes between women living with HIV (WLWH; n = 50; 50.4 ± 1.2 years, 44% postmenopausal) and without HIV (controls; n = 50; 51.8 ± 1.2 years, 52% postmenopausal). METHODS: Outcomes were lumbar spine, total hip and femoral neck areal BMD

Disuse osteoporosis of the upper limb: assessment of thirty patients.

Osteoporosis is a multifactorial skeletal disorder characterized by the decrease of bone mass and the alteration of bone microarchitecture that leads to the increase of fracture risks. Traditionally, osteoporosis has been classified into primary and secondary osteoporosis. Primary osteoporosis refers to osteoporotic conditions which are not related to other chronic illnesses and is usually associated with aging and decreased gonadal function, such as decreased level of estrogen, whereas secondary osteoporosis is the type of osteoporosis caused by other health problems. Disuse is one of the man

Efficacious Enactment of Ilizarov for Mangled Forearm: A Case Report on Our Resolution for the Revitalization of a "Nearly Lost Limb".

INTRODUCTION: Mangled is the evocative term for a grievance caused by crushing, resulting in a limb confiscation. Motor vehicle crashes and industrial/farm accidents are the leading causes of such occurrences. The verdict to reconstruct or to amputate still commonly lies with the surgical skills and experience of the treating surgeon. CASE REPORT: We present a case of 33-year-old male military personnel involved in a road traffic accident who presented to our center with a mangled forearm with segmental fracture radius and fracture shaft of ulna with vascular injury. He was immediately taken

Trampoline-associated injuries are more common in children in spring.

AIMS: Trampoline use is a popular pastime amongst children in New Zealand, and has many advantages for child development. However, recent reports claim that trampoline-associated injuries are still highly prevalent. In order to help prevent these injuries in the future, this study aims to provide more up-to-date epidemiological information in children, with emphasis on the time of year that injuries most commonly occur. METHODS: A retrospective review was carried out utilising a prospective maintained trauma database. The database was searched electronically for injuries involving trampolines

Correlation of radial head and coronoid process fractures in elbow trauma - a retrospective analysis of fracture patterns.

While the relationship between radial head fractures (RHF) and coronoid process fractures (CPF) is biomechanically established, the exact frequency of CPF in RHF patients without focus on elbow dislocations is underexplored. To better estimate the likelihood of CPF, the analysis of typical fracture constellations is useful. The aim of this study was therefore to analyze the correlation between RHF severity and the presence and type of CPF. This retrospective study analyzed 356 RHF patients, evaluating the prevalence and correlation of CPF using CT and intraoperative data. Only cases with confi

Ultrasound-guided vs. fluoroscopy-guided percutaneous leverage reduction for severely displaced radial neck fractures in children: a comparative analysis of clinical and radiological outcomes.

BACKGROUND: Ultrasound (US) is a promising alternative to fluoroscopy (FL) for guiding percutaneous leverage reduction of displaced radial neck fractures in children. However, few studies have compared these modalities. This study aims to compare both the accuracy and efficacy of FL-guided versus US-guided percutaneous pinning in the treatment of radial neck fracture. METHODS: Seventy-three children with displaced radial neck fractures were consecutively enrolled and assigned to the US or FL group. In all the patients, fractures were reduced with the percutaneous pin leverage technique. The p

Comparison of patient-specific implants with standard plates after 3-D-planned corrective osteotomy for distal radial malunions.

We compared 3-D-printed patient-specific implants (PSIs) with standard palmar locking plates for radiological accuracy after extra-articular corrective osteotomy for malunited distal radial fractures. Eleven patients were operated on with a PSI. Additionally, a plastic printed model of each patient was operated on with a standard plate in vitro. Three months postoperatively, radial alignment in patients and models was compared with the preoperative ideal planning using CT scans. Patients with PSIs showed smaller rotational deviations (median 7°) compared with standard plate models (9°). Deviat

Obese Children Have Different Forearm Fracture Characteristics Compared With Normal-weight Children.

BACKGROUND: Current estimates suggest that one third of children and adolescents are overweight and 1 in 5 are obese. Obese children are at increased risk of sustaining more complex fractures, failing nonoperative treatment, and experiencing more complications during treatment. The purpose of this study was to compare forearm fracture characteristics, treatment, and complications in grouped overweight and obese [OW+OB; body mass index-for-age percentile (BMI%) ≥85] pediatric patients compared with normal-weight (NW; BMI%≤84) patients. METHODS: This was a retrospective comparative study of pat

The Cost and Utility of Postreduction Radiographs After Closed Reduction of Pediatric Wrist and Forearm Fractures.

BACKGROUND: Formal radiographs are frequently obtained after reduction of closed pediatric wrist and forearm fracture performed under mini C-arm fluoroscopy. However, their utility has not been clearly demonstrated to justify the increased time, cost, and radiation exposure. We hypothesized that formal postreduction radiographs do not affect the rereduction rate of pediatric wrist and forearm fractures. We further sought to determine the time, monetary, and opportunity costs associated with obtaining these radiographs. METHODS: A total of 119 patients presented to our urban, level I pediatric

Evaluation of Bone Atrophy After Treatment of Forearm Fracture Using Nonlinear Finite Element Analysis: A Comparative Study of Locking Plates and Conventional Plates.

PURPOSE: Forearm diaphysis fractures are usually managed by open reduction internal fixation. Recently, locking plates have been used for treatment. In the long-term period after surgery, some patients present with bone atrophy adjacent to the plate. However, a comparison of locking and conventional plates as a cause of atrophy has not been reported. The aim of this study was to investigate long-term bone atrophy associated with use of locking and conventional plates for forearm fracture treatment. METHODS: In this study we included 15 patients with forearm fracture managed by either locking

Palmar locking plates for corrective osteotomy of latent malunion of dorsally tilted distal radial fractures without structural bone grafting.

The purpose of this retrospective study was to investigate the clinical and radiological outcomes following corrective osteotomy for nascent malunion of distal radial fractures with dorsal tilt using palmar-locking plates without using autologous structural bone grafting for dorsal cortex support. The fractures were dorsally angulated distal radial fractures (AO types A2, A3, and C1) with neglected or delayed treatment for 5 to 8 weeks. Fractures were repaired using 2.4-mm palmar locking plates. Dorsal cortical defects at the osteotomy sites were filled with incipient healing callus. Radiograp

Non-union of an undisplaced radial styloid fracture in a heavy smoker: revisiting the association of smoking and bone healing.

Isolated radial styloid fractures occur relatively infrequently, with non-union of such fractures, especially when undisplaced, being highly unusual. Smoking of tobacco, a common habit which is decreasing in prevalence in the developed world, has been proven to exert many adverse effects on tissue healing including bone union. We present a case of non-union of an undisplaced radial styloid fracture in the dominant hand of a young and healthy heavy smoker, emphasising the negative impact of tobacco smoke and its association with bone repair. We suggest that heavy tobacco users should also be fo
