Relevance of the pyramidal syndrome in amyotrophic lateral sclerosis.

INTRODUCTION: Pyramidal signs (hyperreflexia, spasticity, Babinski sign) are essential for the diagnosis of amyotrophic lateral sclerosis (ALS). However, these signs are not always present at onset and may vary over time, besides which their role in disease evolution is controversial. Our goal was to describe which pyramidal signs were present and how they evolved in a cohort of patients with ALS, as well as their role in prognosis. METHODS: Retrospective analysis of prospectively collected patients diagnosed with ALS in our centre from 1990 to 2015. RESULTS: Of a total of 130 patients with …

[A case of relapsing Guillain-Barré syndrome following Miller Fisher syndrome].

Miller Fisher syndrome (FS) is thought to be a variant of Gullain-Barré syndrome (GBS), both of which rarely relapse. We report a rare case of GBS that followed FS. A 38-year-old woman had ophthalmoplegia, ataxia and areflexia following an upper respiratory tract infection with a diagnosis of FS. Serum anti-GQ1b IgG antibody was found to be increased, but decreased through immunoadsorption as the neurological symptoms of the patient improved. She became completely asymptomatic three months after the onset of FS. Following a common cold two months later, however, she developed weakness of all f…

Effects of carbon disulfide on motor function and responsiveness to d-amphetamine in rats.

Male, albino rats of the Fisher strain were exposed to 2 mg carbon disulfide/l of air for 4 hours per day, 5 days per week for 6 weeks. Neurobehavioral effects were not observed after 3 weeks of dosing, but hindlimb extensor responses and performance on the inclined screen (motor coordination) were impaired after 6 weeks of exposure. Forelimb grip strength and exploratory locomotor activity were not affected. Three weeks after cessation of dosing, recovery of function was observed. Rats exposed to CS2 for 6 weeks were found to be stimulated less than air ventilated controls by 3 mg/kg of d-amp

Evidence for a physiological role of pulmonary arterial baroreceptors in sympathetic neural activation in healthy humans.

KEY POINTS: In an anaesthetised animal model, independent stimulation of baroreceptors in the pulmonary artery elicits reflex sympathoexcitation. In humans, pulmonary arterial pressure is positively related to basal muscle sympathetic nerve activity (MSNA) under conditions where elevated pulmonary pressure is evident (e.g. high altitude); however, a causal link is not established. Using a novel experimental approach, we demonstrate that reducing pulmonary arterial pressure lowers basal MSNA in healthy humans. This response is distinct from the negative feedback reflex mediated by aortic and ca…

Endoscopic suspension of the vesical neck in massive urethral dilatation.

Endoscopic suspension of the vesical neck was used to prevent urinary leakage in 2 female spinal cord patients with reflex neurogenic bladders, chronic urethral catheters and massive urethral dilatation. Factors responsible for the favorable results are discussed.

Abdominal Clonus as a Rare Visceromotor Manifestation in Spinal Cord Injury: A Case Report.

Spinal cord injury can lead to reorganization of spinal reflex circuits and loss of supraspinal modulation of autonomic and somatic functions. Although bladder distension is a well-recognized trigger of autonomic dysreflexia, associated somatic motor manifestations are rarely described. We report the case of a 69-year-old man with a long-standing traumatic complete C6 spinal cord injury and chronic neurogenic bladder. Neurological classification according to the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) was American Spinal Injury Association (ASIA)…

Diabetic gastroenteropathy: Associations between gastrointestinal symptoms, motility, and extraintestinal autonomic measures.

BACKGROUND: Diabetic gastroenteropathy can cause significant diagnostic challenges. Still, it remains unknown if measures of extraintestinal … otility capsule (Smartpill™) assessed panenteric transit times and motility indices. Cardiovascular reflex tests (VAGUS™) and cardiac vagal tone (eMotion Faros) estimated cardiovascular autonomic neuropathy, while the SUDOSCAN™ evaluated sudomotor function. KEY RESULTS: Proximal gastrointestinal symptoms were positively associated with the gastric motil …

Functional symmetry of the aortic baroreflex in female spontaneously hypertensive rats.

BACKGROUND: Altered baroreflex function is well documented in hypertension; however, the female sex remains far less studied compared with males. We have previously demonstrated a left-sided dominance in the expression of aortic baroreflex function in male spontaneously hypertensive rats (SHRs) and normotensive rats of either sex. If lateralization in aortic baroreflex function extends to hypertensive female rats remains undetermined. This study, therefore, assessed the contribution of left and right aortic baroreceptor afferents to baroreflex modulation in female SHRs. METHOD: Anesthetized f…

Diagnostic outcomes after colposcopy at a tertiary clinic for women aged 50-74 referred after detection of oncogenic human papillomavirus at primary screening.

MATERIAL … of CIN2+ detected between the two groups, 'HPV16/18' (9.9%) or 'HPV (not 16/18)' (14.5%), nor with reflex cytology types. The positive predictive value (PPV) of high-grade impression at colposcopy was 63.6%. There were 243 (52.4%) who had Type 3 transformation zone (TZ3) with 20 CIN2+ detected, 13 at FCV including all three cancers and five at Excision. …

Gene and protein expression of dorsal root ganglion sensory receptors in normotensive and hypertensive male rats.

The exercise pressor reflex (EPR), a neurocirculatory control mechanism, is exaggerated in hypertensive humans and rats. Disease-related abnormalities within the afferent arm of the reflex loop, including mechano- and metabosensitive receptors located at the terminal end of group III/IV muscle afferents, may contribute to the dysfunctional EPR in hypertension. Using control (WKY) and spontaneous hypertensive (SHR) rats, we examined dorsal root ganglion (DRG) gene and protein expression of molecular receptors recognized as significant determinants of the EPR. Twelve lumbar DRGs (6 left, 6 right…

Transcutaneous vagus nerve stimulation - A brief introduction and overview.

Invasive cervical vagus nerve stimulation (VNS) is approved for the treatment of epilepsies, depression, obesity, and for stroke-rehabilitation. The procedure requires surgery, has side-effects, is expensive and not readily available. Consequently, transcutaneous VNS (tVNS) has been developed 20 years ago as non-invasive, less expensive, and easily applicable alternative. Since the vagus nerve reaches the skin at the outer acoustic canal and ear, and reflex-responses such as the ear-cough-reflex or the auriculo-cardiac reflex have been observed upon auricular stimulation, the ear seems well su…
