[Surgical approach to the posterior surface of the liver (author's transl)].

Anterior incision does not afford a direct approach to lesions located in the posterior part of the liver, or to right subphrenic and retrohepatic abcesses, and may contaminate the peritoneal cavity in suppurative collections. Posterior incision is a more direct approach. It can be extrapleural and extraperitoneal, or transpleural with exclusion of the pleura when resecting a lower rib; a posterior thoraco-phreno-laparotomy gives a wider exposure. When the pleural cavity has been opened it should be drained and excluded before dealing with the subphrenic lesion. Such an incision is advocated i

Chest radiograph appearances following the removal of pleural tubes.

Appearances on chest radiographs following the insertion and removal of 40 intrapleural tubes in 34 patients are described. Forty-three appearances were identified that were definitely related to tubes. The commonest of these were pleural densities and tube tracks, both of which usually resolved within seven days. Densities accounted for most of the appearances lasting longer than this. Less common appearances were skin holes due to skin puckering around the tube at its site of entry, and small air-fluid levels at the tube tip. A separate group of 14 intrapleural tubes is described: on their r

Thoracic trauma.

The physiologic equilibrium of chest injury patients is frequently precarious, and mild stress during examination and treatment may precipitate acute decompensation and death. This is particularly true with the respiratory system, where the normally large respiratory reserve capacity may be rapidly lost. Accurate assessment of the nature of the thoracic injury and the severity of that injury must be determined in order to formulate a therapeutic plan. Many thoracic injuries, such as pneumothorax, pulmonary contusions, or rib fractures, will be self-limiting. Other conditions must be recognized

Clinical features of 22 cases with "inter-sterno-costo-clavicular ossification". A new rheumatic syndrome.

We present 22 cases with inter-sterno-costoclavicular ossification. Clinical and pathological findings show that abnormal ossification observed in this situation is due to non-suppurative chronic inflammation of the soft tissues around the sterno-costo-clavicular region such as the costo-clavicular ligament. We have classified X-ray findings into three stages according to the extent of the ossification; localized, generalized, and hyperostotic, and show that the disease progressed in this sequence. A considerable number of the patients showed abnormal X-ray findings in the spine or the sacro-i

Optimising chest computed tomography (CT) scan ranges in paediatric patients to reduce radiation exposure.

AIM: The aim of this study was to accurately position the scan range of unenhanced chest computed tomography (CT) scans for paediatric patients by clarifying the upper and lower scan boundary ranges to meet clinical needs while minimising radiation risks. MATERIALS AND METHODS: This two-cohort study included 3174 children (aged 3-14 years) undergoing unenhanced chest CT scans. Cohort 1 (n=1894) was used to retrospectively analyse lung boundary variability to determine optimal scan ranges, while cohort 2 (n=1,280) was used to validate the accuracy in delineating lung boundaries and reducing ra

The effectiveness of an early management guideline for older patients following blunt thoracic injury.

BACKGROUND: Thoracic injury is a leading cause of trauma related morbidity and mortality worldwide. Older patients (aged ≥65 years) who have sustained rib fractures following blunt chest wall trauma are at greater risk of serious pulmonary complications, prolonged hospital stay, and death when compared with the general population. The Blunt Chest-Wall Injury and Rib Fracture Pathway (BCWP) was introduced at our institution in July 2021 and advocates for early, aggressive analgesia, specialist support from the Acute Pain Service as well as consideration of regional techniques and Intensive Care

Effect of early analgesia with erector spinae plane block to reduce ventilation after severe chest trauma: the multicentre randomised controlled TARGET study protocol.

INTRODUCTION: Blunt chest trauma is frequently associated with rib fractures, and inadequate pain control may lead to respiratory complications and prolonged ventilatory support. In spontaneously breathing patients, pain impairs coughing and secretion clearance, promoting atelectasis and respiratory failure, while in mechanically ventilated patients, it may delay weaning. Current French guidelines recommend intravenous multimodal analgesia as first-line therapy followed by thoracic epidural or paravertebral block if pain remains uncontrolled. However, these techniques have limited applicabilit

A novel concept for splint fixation of midshaft clavicle fractures: a biomechanical feasibility study.

BACKGROUND: Minimally invasive treatment of clavicle shaft fractures has become increasingly popular due to reduced soft tissue damage, smaller incisions, lower rates of infections, and minimal scarring. Most commonly described options are titanium elastic nailing and plate osteosynthesis. Clinical studies report promising results with good healing tendencies, particularly in children and adolescents. However, there are still some downsides to consider—titanium elastic nails have been associated with soft-tissue irritation, implant associated discomfort, and notable complications rates includi

Engineering application of a load-induced beam-type bearing model for deep water-bearing coal specimens.

To investigate the bearing characteristics of deep coal mass under long-term spray-water conditions, this study took the rib coal mass at the head area of the third belt conveyor in a kilometer-deep roadway of the Huainan mining area as the research object, and conducted wave velocity tests, mechanical tests, and acoustic emission monitoring experiments on both dry and water-bearing coal specimens. The results show that although the peak strength of deep coal specimens decreases under water-bearing conditions, the overall continuity and load transfer capacity are enhanced compared with shallow

Would initial thoracic correction be a key factor in orthotic intervention for adolescent idiopathic scoliosis with double-curve?

PURPOSE: To investigate the influence of in-orthosis correction on treatment outcomes of adolescent idiopathic scoliosis (AIS) with double-curve, and to explore baseline radiographic features associated with achieving high in-orthosis correction. METHODS: This retrospective study analyzed 101 skeletally immature patients with double-curve AIS treated with customized thoracic-lumbar-sacral orthosis (TLSO). Radiographic measurements from pre-orthosis standing and supine, first in-orthosis, and final out-of-orthosis follow-up visits were analyzed. Initial in-orthosis correction rates for the tho

Single-stage orthognathic correction of campylorrhinus lateralis in foals using transection and fixation: Surgical description and outcome of four horses.

BACKGROUND: Campylorrhinus lateralis ('wry nose') is a rare congenital maxillofacial deformity of foals resulting in compromise of the airway and severe class 4 malocclusion. Various surgical techniques to correct the deformity have been described, but contemporary data on single-stage transection and fixation, including use of a rib graft, and long-term functional and cosmetic outcomes remain underreported. OBJECTIVES: To describe the transection-and-fixation technique and perioperative management used to correct wry nose in horses, and to report postoperative complications and long-term out

Case Report: Retained gallstone migrating to the chest wall after cholecystectomy presenting as chronic abscess, sinus tract, and inflammatory pseudotumor.

BACKGROUND: Gallbladder perforation and gallstone spillage are common during laparoscopic cholecystectomy, occurring in up to 40% and 6%-10% of cases, respectively. While most retained stones remain asymptomatic, 2.3%-8.5% of patients may present with intra-abdominal abscess, granuloma formation, abdominal wall sinus, fistula or a mass mimicking a malignancy (1). We report a unique case of a retained gallstone migrating to the chest wall 2 years after cholecystectomy, forming a chronic abscess, sinus tract, and inflammatory pseudotumor. The role of the "Eve" procedure in metacarpal reconstruction revised: review of literature and long term results.

INTRODUCTION: The "Eve" procedure is a vascularized composite flap consisting of rib, cartilage, serratus anterior muscle, and fascia. We aimed to re-assess its role in metacarpal reconstruction by means of a literature review and by evaluating the long term outcomes in two cases after a follow-up of more than 30 years. The long term structural stability of the bone and the functional value of the neo-joint were the focus of assessment. METHODS: A literature review was conducted using PubMed with key terms related to the Eve procedure and metacarpal reconstruction. The two cases published in

Beyond the Slopes and Highways: Endovascular Repair of Blunt Traumatic Aortic Injuries after Skiing versus Motor Vehicle Accidents.

Background: Blunt traumatic aortic injury (BTAI) is a potentially fatal condition, typically resulting from high-velocity trauma. To date, little is known about this type of injury among skiers, who form the largest patient cohort with aortic injuries in the alpine region of Tyrol, Austria. Methods: This retrospective, single-center study at the University Hospital of Innsbruck analyzed patients who underwent endovascular treatment for blunt traumatic aortic injury from 2005 to 2023. Patient data were extracted from electronic and digitalized medical history records. Subsequent analyses compar

Cobalt-chromium versus titanium alloy rods for correction of adolescent idiopathic scoliosis based on 1-year follow-up: a multicenter randomized controlled clinical trial.

OBJECTIVE: For instrumented correction surgery for adolescent idiopathic scoliosis (AIS), surgeons are increasingly switching from titanium (Ti) alloy rods to stiffer cobalt-chromium (CoCr) rods. The authors conducted the first multicenter randomized controlled clinical trial to investigate whether these materials affect the outcomes in terms of spine correction and quality of life (QOL). This trial was registered at UMIN Clinical Trials Registry on September 3, 2012, under the identifier UMIN000008838 (level of evidence 1). METHODS: Female AIS patients (Lenke types 1-3, patient age 10-19 yea
