Biomechanical evaluation of manual weeding postures in nursery and landscaping tasks.

Manual weeding is a physically demanding task in nursery and landscaping work, often involving awkward postures that increase the risk of work-related musculoskeletal disorders (WRMSDs). This study examined the biomechanical demands of five common weeding postures: standing and bending; kneeling on one knee; kneeling on both knees; crouching; and sitting on a bucket. Twenty participants (55% male; aged 18-29 years, average age 21.5 ± 3.69 years) completed simulated weeding tasks while motion capture and surface electromyography (EMG) recorded joint motion and muscle activity. Standing and bend

Irreversible neurological manifestations in undiagnosed disseminated gonococcal infection.

Neisseria gonorrhoeae causes a common sexually transmitted infection with manifestations ranging from asymptomatic to urethritis and pelvic inflammatory disease to disseminated infections including septic arthritis. Serious complications may arise in unrecognised or inappropriately treated infections. We report a young, healthy woman who developed fever and joint pain and was diagnosed with an inflammatory arthritis. After starting immune suppressing treatments, she experienced right wrist drop and progressive muscle atrophy, joint contractures and sensory loss. Electrodiagnostic studies showed

Anterior shoulder instability: a review of pathoanatomy, diagnosis and treatment.

The glenohumeral joint is inherently predisposed to instability by its bony architecture. The incidence of traumatic shoulder instability is 1.7% in the general population. Associated injuries to the capsulolabral structures of the glenohumeral joint have been described and may play a role in predicting recurrent instability. Advanced imaging, computed tomography or MRI may be necessary to adequately evaluate for associated glenohumeral pathology. Treatment algorithms have traditionally included a period of non-operative management in all patients, however young athletic patients may often ben

Correlation of ultrasonography synovitis with disease activity and clinical response to etanercept treatment in juvenile idiopathic arthritis patients.

This study aimed to investigate the correlation of ultrasonography (US) of synovitis with disease activity and clinical response to etanercept (ETN) in juvenile idiopathic arthritis (JIA) patients. Eighty-two JIA patients who underwent ETN treatment for 24 weeks were consecutively enrolled. US evaluations of 28 joints (shoulder, elbow, wrist, metacarpophalangeal, and proximal interphalangeal of hands and knee) at baseline were performed using grey-scale US and power doppler (PD) US, and US synovitis was defined as grey-scale abnormalities or PD abnormalities. Clinical response was assessed acc

Digital tracking algorithm reveals the influence of structural irregularities on joint movements in the human cervical spine.

BACKGROUND: Disc height loss and osteophytes change the local mechanical environment in the spine; while previous research has examined kinematic dysfunction under degenerative change, none has looked at the influence of disc height loss and osteophytes throughout movement. METHODS: Twenty patients with pain related to the head, neck or shoulders were imaged via videofluoroscopy as they underwent sagittal-plane flexion and extension. A clinician graded disc height loss and osteophytes as "severe/moderate", "mild", or "none". A novel tracking algorithm quantified motions of each vertebra. This

Radiosynovectomy in the therapeutic management of arthritis.

Radiosynovectomy is a well-established therapy in arthritis and involves an intra-articular injection of small radioactive particles to treat a synovitis. In Europe, frequent indications are rheumatoid and poly-arthritis. Especially in Germany radiosynovectomy is the second common therapy in Nuclear Medicine with about 40,000-60,000 treated joints per year. In Spain, USA, Turkey, Argentines and Philippines the therapy is more use in hemophilic arthritis with excellent results. Especially in developing countries with low availability of clotting factors, the radiosynovectomy represent a cost ef

Ultrasound-guided percutaneous rotator interval approach for simulated adhesiolysis: A proof-of-concept feasibility and anatomical safety study.

BACKGROUND: Several key structures involved in frozen shoulder are located in the anterior shoulder capsule. While ultrasound-guided corticosteroid injections and capsular hydrodilatation have demonstrated favorable outcomes through an anterior approach, percutaneous interventions aimed at mechanical adhesion release remain insufficiently investigated. This study evaluated the feasibility and anatomical safety of an ultrasound-guided simulated percutaneous adhesiolysis of the rotator interval in a cadaveric model. METHODS: The procedure was performed in a single non-pathological Thiel-embalme

Risk of venous thromboembolism after knee, hip, or shoulder arthroplasty : a retrospective cohort study of 476,620 procedures using linked hospital and primary care data.

AIMS: Arthroplasty is a widely used surgery for end-stage arthritis, but venous thromboembolism (VTE) remains a major postoperative complication. Most previous studies have relied solely on hospital data; no study has examined the effect of national clinical guidelines on the rates of VTE in the UK. This study aimed to assess VTE trends after arthroplasty using linked primary and secondary care data, and the impact of the National Institute for Health and Care Excellence (NICE) guidelines in the UK on VTE rates after arthroplasty. METHODS: We used the Clinical Practice Research Datalink (CPRD

Factors influencing patient decision-making to undergo shoulder arthroplasty.

AIMS: Shoulder arthroplasty is effective in the management of end-stage glenohumeral joint arthritis. However, it is major surgery and patients must balance multiple factors when considering the procedure. An understanding of patients' decision-making processes may facilitate greater support of those considering shoulder arthroplasty and inform the outcomes of future research. METHODS: Participants were recruited from waiting lists of three consultant upper limb surgeons across two NHS hospitals. Semi-structured interviews were conducted with 12 participants who were awaiting elective shoulde

Does the presence of arthroscopically detected stage 1-2 glenohumeral osteoarthritis have any clinical impact on the outcome of arthroscopic rotator cuff repairs?

BACKGROUND: : Rotator cuff rupture (RCR) and glenohumeral osteoarthritis (GHO) are two common disorders of the shoulder joint. However, there are very few reports that examine the relationship between them. This study aimed to present at least two years' clinical results of arthroscopic rotator cuff repair of full-thickness and massive tears accompanied by arthroscopically detected early-stage osteoarthritis. METHODS: From August 2016 to December 2017, three hundred and twenty patients with total or massive rotator cuff tears were evaluated retrospectively. Thirty-five patients who were deter…

Individualized Techniques of Implant Coating with an Antibiotic-Loaded, Hydroxyapatite/Calcium Sulphate Bone Graft Substitute.

BACKGROUND: The treatment of fracture- or non-union-related infections has persistently been a major challenge for both patients and treating surgeons. With rising aging of patients and increasing comorbidities, combined with the heterogeneity of germs and any number of multi-resistance against standard antibiotics, a successful treatment is increasingly difficult. One potential solution could be a custom-made individualized antibacterial coating of standard implants with a biphasic degradable biocarrier (Cerament G/V, supplied by Bonesupport AB, Lund, Sweden) that releases high doses of antib
