Clinical diagnostic accuracy of suspected sources of bacteremia and its effect on mortality.

BACKGROUND: Although much has been written about bacteremia, evidence of the clinical diagnostic accuracy of bacteremia sources in the absence of microbiological results and the impact of diagnostic accuracy on mortality is scarce. METHODS: This is a retrospective study of bacteremia episodes over a 2-year period at a general hospital in Madrid. Congruence analyses between clinically presumed and definite sources, acquisition, causative organism, empirical treatment and progression to death were performed. RESULTS: The study included 323 bacteremia episodes. Clinicians' diagnostic accuracy w

Efficacy and safety of vancomycin versus 13 alternatives in MRSA-confirmed skin and soft tissue infections: a meta-analysis of 39 randomized controlled trials.

PURPOSE: Vancomycin is the standard therapy for suspected or confirmed Methicillin-resistant Staphylococcus aureus (MRSA) skin and soft tissue infection (SSTI). Although newer anti-MRSA agents claim improved efficacy and safety, evidence in microbiologically confirmed MRSA-SSTI remains limited and heterogenous. Thus, this study aimed to systematically compare the efficacy and safety of alternative agents versus vancomycin in adults with confirmed MRSA-SSTI. METHODS: Three databases were searched until August 05, 2025, to retrieve randomized controlled trials (RCTs) from published meta-analyse

Healing cascades and infections in wounds monitored using a wearable sensor of gaseous flux.

Capabilities for quantitative monitoring of chronic wounds remain an unmet clinical need, as existing diagnostic approaches rely on semiquantitative evaluation of symptoms that lack sensitivity especially during early stages of infection. Here, we present a scheme for tracking wound physiology that leverages a miniature, wireless skin-interfaced device for noncontact, transient measurements of the flux of volatile organic compounds (VOCs) and water vapor from the wound microenvironment. Unlike emerging smart bandage platforms that rely on physical contact with the fragile wound bed to interrog

Herd-level factors associated with Streptococcus uberis, Staphylococcus aureus, and Streptococcus agalactiae mastitis: Dairy herd data from 2013, 2018, and 2023.

Streptococcus uberis is an important cause of bovine intramammary infection (IMI), primarily transmitted via environmental pathways, while some strains show potential for contagious transmission. This retrospective observational study aimed to determine and evaluate the herd characteristics associated with 1) increased proportional morbidity (PM) of S. uberis IMI, 2) increased PM of Staphylococcus aureus IMI, and 3) presence of Streptococcus agalactiae in a herd. Data consisted of Finnish national dairy herd improvement records and quarter-level milk sample qPCR analyses for 2013, 2018, and 20

Multifunctional Ionic Composite Hydrogel with Curcumin Capsule for Promoting Infected Wound Healing.

Wound infection complicated by excessive inflammation remains a persistent challenge in clinical healthcare. Curcumin (Cur), a natural diketone compound, holds therapeutic potential in treating infected wounds. However, its application is limited by poor aqueous dispersibility and low stability. To overcome this dilemma, a versatile capsule as an effective Cur carrier is developed by encapsulating Cur in succinic acid-modified cyclodextrin via hydrophobic interactions, and then loading it into a Zn2+-mediated crosslinked injectable hydrogel for infected wound healing. The composite hydrogel ex

Self-Powered Microneedle Patch with Synergistic Mild Microwave Thermal Therapy for Scarless Healing of Infected Diabetic Wound.

Diabetic wound healing remains a major challenge, as persistent bacterial infections and chronic inflammation can delay healing and lead to scar formation. Herein, a novel self-powered microneedle patch (SPMNP), combined with mild microwave thermal (MT) therapy, is developed to promote the scarless healing of infected diabetic wounds. The SPMNP is fabricated by integrating a piezoelectric nanogenerator (PENG) with a microneedle patch (CFeZ/MNP) loaded with Curcumin@Fe3O4-COOH@ZIF-8 nanoparticles (CFeZ NPs). The CFeZ NPs endow the CFeZ/MNP with a mild MT effect under microwave irradiation, enab

Xenopus laevis as an infection model for human pathogenic bacteria.

Animal infection models are essential for understanding bacterial pathogenicity and corresponding host immune responses. In this study, we investigated whether juvenile Xenopus laevis could be used as an infection model for human pathogenic bacteria. Xenopus frogs succumbed to intraperitoneal injection containing the human pathogenic bacteria Staphylococcus aureus, Pseudomonas aeruginosa, and Listeria monocytogenes. In contrast, non-pathogenic bacteria Bacillus subtilis and Escherichia coli did not induce mortality in Xenopus frogs. The administration of appropriate antibiotics suppressed mort

Risk Factors, Diagnoses, and Clinical Characteristics of Synchronous and Metachronous Periprosthetic Joint Infections Following Total Joint Arthroplasty.

BACKGROUND: Although multiple PJIs (periprosthetic joint infections), either synchronous or metachronous, occur in up to 20% of patients who have an index PJI after total joint arthroplasty (TJA), there remains a lack of understanding regarding multiple PJI risk factors, diagnosis, and management. Therefore, we asked: (1) What risk factors have been identified for synchronous and metachronous PJI after TJA compared to single-PJI patients? (2) What is the clinical relation of additional PJIs to the index PJI (time and location)? (3) Are the infectious organisms in multiple PJIs the same? and (4

Biofilm and the effect of sonication in a chronic Staphylococcus epidermidis orthopedic in vivo implant infection model.

BACKGROUND: In diagnosing chronic orthopedic implant infections culture of sonicate represents a supplement to tissue cultures. However, the extent to which biofilm forms on implant surfaces and the degree of dislodgement of bacteria by sonication remains unclear. In this in vivo study using a low bacterial inoculum, we aimed to determine whether a variable effect of sonication could be observed in a standardized in vivo model. MATERIALS AND METHODS: Seven Wistar rats underwent surgery with intramuscular implantation of two bone xenograft implants, each containing two steel plates. The grafts

Bacteria-mimetic nanomedicine for targeted eradication of intracellular MRSA.

Drug-resistant infections caused by intracellular bacteria, such as methicillin-resistant Staphylococcus aureus (MRSA), which are often hidden inside macrophages, pose a significant threat to human health. Various nanomedicines have been developed to combat intracellular MRSA; however, their poor uptake and fast clearance from macrophages often result in insufficient enrichment of antibacterial agents intracellularly, leading to low antibacterial efficacy. Here, we developed bacterial membrane-coated mesoporous SiO2 nanoparticles (MSN) loaded with vancomycin (Van), a classic antibiotic. These

Bayesian modeling of the impact of antibiotic resistance on the efficiency of MRSA decolonization.

Methicillin-resistant Staphylococcus aureus (MRSA) is a major cause of morbidity and mortality. Colonization by MRSA increases the risk of infection and transmission, underscoring the importance of decolonization efforts. However, success of these decolonization protocols varies, raising the possibility that some MRSA strains may be more persistent than others. Here, we studied how the persistence of MRSA colonization correlates with genomic presence of antibiotic resistance genes. Our analysis using a Bayesian mixed effects survival model found that genetic determinants of high-level resistan

Innovative cold atmospheric plasma (iCAP) decreases corneal ulcer formation and bacterial loads and improves anterior chamber health in methicillin resistant Staphylococcus aureus keratitis.

Bacterial keratitis is a vision-threatening infection of the cornea that is typically treated with antibiotics. However, antibiotics sometimes fail to eradicate the infection and do not prevent or repair the damage caused directly by the bacteria or the host immune response to the infection. Our group previously demonstrated that treatment of Pseudomonas aeruginosa keratitis in rabbits with innovative cold atmospheric plasma (iCAP) resulted in reduced edema, ulcer formation, and bacterial load. In this study, we investigated the efficacy of iCAP treatment in methicillin-resistant Staphylococcu

Recent advancements in nanotechnology-based bacteriophage delivery strategies against bacterial ocular infections.

Antibiotic resistance is growing as a critical challenge in a variety of disease conditions including ocular infections leading to disastrous effects on the human eyes. Staphylococcus aureus (S. aureus) mediated ocular infections are very common affecting different parts of the eye viz. vitreous chamber, conjunctiva, cornea, anterior and posterior chambers, tear duct, and eyelids. Blepharitis, dacryocystitis, conjunctivitis, keratitis, endophthalmitis, and orbital cellulitis are some of the commonly known ocular infections caused by S. aureus. Some of these infections are so fatal that they co

Vancomycin MIC creep progresses in methicillin-resistant Staphylococcus aureus despite the national antimicrobial stewardship campaign: Single facility data in Japan.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is a leading cause of nosocomial and community infections, and vancomycin (VCM) is widely recommended as a first-line therapeutic drug. Minimum inhibitory concentrations (MICs) of VCM ≤2 μg/mL are defined as susceptible, but increases in these levels, known as "VCM MIC creep" have been reported. The aim of this study was to investigate VCM MIC creep during the promotion of a national antimicrobial stewardship campaign. METHODS: We collected data from 2013 to 2020 on S. aureus isolated at the clinical microbiology laboratory at Oka
