Primary tracheojejunal shunt operation for voice restoration following pharyngolaryngoesophagectomy.

A primary tracheojejunal shunt operation was performed for voice restoration following pharyngolaryngoesophagectomy with free jejunum reconstruction for advanced hypopharyngeal cancer. A fistula was created between the membranous part of the trachea and the lower part of the transplanted jejunum. The membranous part of the trachea was tubed to construct the tracheojejunal shunt. All three patients who had the tracheojejunal shunt operation retained phonatory function. Pitch formation was seen in the voice waveform with use of the tracheojejunal shunt. No leakage was seen at all during deglutit

Orofacial functions in patients with congenital and acquired maxillary defects: a fluoroscopic study.

By means of videofluoroscopic recordings, chewing, swallowing and speech were monitored in nine patients with congenital and acquired maxillary defects. All of them were rehabilitated with a maxillary obturator prosthesis. The defective region was seldom used for chewing. No leakage between the obturator and surrounding tissue was observed, either for solids or for liquids. All of the prosthetic reconstructions were surprisingly stable during function. In the phonetic analysis, speech production was judged to be restored almost to normal. Videofluoroscopy may be valuable as a complement to oth

Palliation of oesophagogastric neoplasms with Nd:YAG laser treatment.

The palliative effect of endoscopic Nd:YAG laser treatment of malignant strictures of the oesophagus and cardia was prospectively evaluated in 30 patients. Most (n = 28) were primarily treated for dysphagia, and swallowing was improved in 21 and normalized in 14 of these patients. Two patients were successfully treated for bleeding. Six patients developed complications (including two perforations), and four of these were fatal. The median survival was 4 months (range, 17 days to 14 months). It is concluded that in a great number of patients with inoperable cancer of the oesophagus and cardia,

[Supraventricular tachyarrhythmia of a variable degree of atrial synchronization caused by drinking cold water].

We report upon a patient with mitral valve prolapse of mild degree who complained of paroxysmal tachyarrhythmias after drinking ice-cold beverages. It was demonstrated that iced water ingestion was followed, in this patient, by transient episodes of supraventricular tachyarrhythmias. The reproducible induction of acute, transient rhythm disturbance can possibly be explained by the temperature changes of either the pericardium or the left atrial wall, which have been experimentally shown to follow iced water swallowing. Whether this is a direct cause-to-effect phenomenon or one involving a neur

Equilibrium theory revisited: factors influencing position of the teeth.

The major primary factors in the dental equilibrium appear to be resting pressures of tongue and lips, and forces created within the periodontal membrane, analogous to the forces of eruption. Forces from occlusion probably also play a role in the vertical position of teeth by affecting eruption. Respiratory needs influence head, jaw and tongue posture and thereby alter the equilibrium. "Deviate swallowing" is more likely to be an adaptation than a cause of tooth changes. Patients with failure of eruption have been recognized and alterations in the eruption mechanism may be more important clini

Dysphagia in unilateral cerebral lesions.

Dysphagia may in rare instances be a presenting feature of unilateral cerebral lesions. Normally, bilateral lesions are necessary to cause neurological disturbance of swallowing but there appear to be occasional subjects where this is not so. Cases are presented and the literature reviewed to illustrate this. The dysphagia may occur in isolation and is not necessarily accompanied by dysarthria, facial apraxia, or obvious paralysis. The lesion may be on either side and usually involves the low posterior frontal region. Cases that have come to necropsy have shown involvement of the lowest part o

Surgical management of cricopharyngeal achalasia in dogs results in low mortality but moderate incidence of recurrence or persistent clinical signs.

OBJECTIVE: To describe diagnostic and surgical management of cricopharyngeal achalasia (CA) in dogs and to identify patient or management factors that may contribute to postoperative outcomes. METHODS: Signalment, diagnostic imaging findings, surgical information, complications, and outcomes were retrospectively obtained. Fisher exact and Wilcoxon rank-sum tests were performed to identify predictors of unsatisfactory outcomes using sex, breed, age, videofluoroscopic swallow study findings, laterality of surgery, and muscles resected as variables. RESULTS: 26 dogs surgically managed for CA fr

Voluntary peak cough flow: A simple and effective tool to predict dysphagia across diverse etiologies.

BACKGROUND AND AIM: Swallowing and coughing share neural and muscular substrates and play crucial roles in airway protection. This study aimed to compare voluntary peak cough flow (PCF) between patients with various dysphagia etiologies and healthy controls, and to identify a PCF cutoff value for predicting dysphagia. METHODS: A total of 90 participants were included in the present case-control study. The study group (SG) consisted of 45 patients with FEES-confirmed dysphagia (M/F: 25/20) with a mean ± SD age of 66.64 ± 12.97, and the control group (CG) consisted of 45 age- and gender-matched

Gel-Based 3D Food Printing for Dysphagia Management: Advances in Personalized Nutrition, Texture Control, and Clinical Translation.

Dysphagia and age-related oral processing limitations are rising with population aging and the growing burden of neurological diseases. Texture-modified diets remain the most common non-pharmacological intervention, yet conventional pureeing and thickening often yield meals with low visual appeal, variable textures, and diluted nutrient density, which contribute to reduced intake and malnutrition risk. Extrusion-based three-dimensional food printing, especially when combined with gel-derived edible inks, offers a digital route to standardize geometry, portioning, and texture while enabling ind

Compression of the Left Brachiocephalic Vein by a Type II Right Aortic Arch: A Rare Vascular Anomaly With Unique Clinical Presentation.

Right-sided aortic arch (RAA) is an uncommon anatomical variation of the thoracic vasculature, occurring in approximately 0.1% of adults. In nearly half of these instances, the left subclavian artery follows an abnormal course. The left subclavian artery typically emerges from a tapered dilation at its origin from the aorta, referred to as Kommerell's diverticulum (KD). Based on available literature, only a few cases have been documented. We describe a 62-year-old female patient who presented with a painless chest bump persisting for four weeks. She reported no symptoms such as cough, shortnes

Exploration of Patient Adherence to Physician Recommendations in Laryngology Clinics.

INTRODUCTION AND OBJECTIVES: Effective patient adherence to physician-recommended care plans is critical for optimal health outcomes. Poor adherence can delay diagnosis, prolong symptoms, and worsen disease progression. Limited data exist on rates of patient adherence in laryngology and the barriers contributing to these gaps. This study aimed to quantify task completion rates following laryngology visits and identify factors associated with incomplete task completion. STUDY DESIGN AND METHODS: A mixed methods quality improvement initiative was conducted on consecutive adult patients evaluate
