Economic Analysis of Using Free Fat Graft or Acellular Dermis to Prevent Post-parotidectomy Frey Syndrome.

A decision tree was constructed to determine the incremental cost-effectiveness ratio (ICER) of grafting techniques used to prevent Frey Syndrome. The authors performed a sensitivity analysis to calculate what the probability of preventing Frey Syndrome would have to be and maximum costs associated with using grafting techniques to warrant their use as more "cost-effective" choice than using neither. Decision pathways utilized were uses of (1) free fat graft (FFG), (2) acellular dermis, and (3) no grafting. The probability of developing Frey syndrome and costs were extracted from previous stud

Incidence of moisture-associated skin damage in an intensive care unit.

OBJECTIVES: To determine the incidence of moisture-associated skin damage (MASD) in the nappy area, identify predisposing factors and know the preventive measures and nursing records. METHOD: Descriptive longitudinal study (June 2014-April 2015) in a general ICU. Patients whose stay >48hours and without skin lesions were included. The skin was assessed daily until the appearance of MASD, discharge or a maximum of 14 days. Demographics, stay, MASD type, incontinence, number and consistency of stools, obesity, Braden scale and prevention were recorded. RESULTS: 145 patients (66.2% male) were s

A diagnostic challenge.

A 74 year-old woman presented with a 9 month history of a slowly enlarging exophytic lesion over her left nasal alar. Examination revealed a firm, cystic mass fixed to underlying skin. It was pedunculated with occasional superficial telangiectasia and a violaceous hue. A differential diagnosis of dermoid or sebaceous cyst, dermatofibroma, and neurofibroma was considered. Histopathological examination confirmed the rare diagnosis of chondroid syringoma (CS), which has a reported incidence of 0.01- 0.098% of excised skin tumors. It is unusual for CS to present in an elderly woman, as these benig

Porocarcinoma coexisting at a site of Bowen disease in a 63-year-old woman.

Porocarcinoma is an unusual, locally aggressive and potentially fatal neoplasm. Several cutaneous malignancies have been described in association with porocarcinoma, including squamous cell carcinoma, basal cell carcinoma and tricholemmal carcinoma. Previous reports have indicated that the occurrence of malignant tumours in combination with porocarcinoma is extremely rare, in particular with regard to Bowen disease (BD). We report an uncommon case of porocarcinoma occurring synchronously in a single BD lesion in a 63-year-old woman with multiple BD lesions. The clinical and histological findin

Dermoscopy of Nodular Hidradenoma, a Great Masquerader: A Morphological Study of 28 Cases.

BACKGROUND: Nodular hidradenoma is an uncommon, benign, adnexal neoplasm of apocrine origin which is a clinical simulator of other tumours. OBJECTIVE: The aim of this study was to evaluate the morphological findings of a large series of nodular hidradenomas under dermoscopic observation. METHODS: Dermoscopic examination of 28 cases of nodular hidradenomas was performed to evaluate specific dermoscopic criteria and patterns. RESULTS: The most frequently occurring dermoscopic features were: (1) in 96.4% of cases, a homogeneous area that covered the lesion partially or totally, the colour of w

Multiple apocrine poromas: a new case report.

Apocrine poromas are rare and distinctive benign adnexal neoplasms featuring tumor cells differentiating toward folliculosebaceous-apocrine units. We report an extremely rare case with multiple apocrine poromas in a single patient. Fifteen tumors were distributed on the head, neck, forearm and axilla of a 74-year-old man. All tumors were mostly composed of poroid cells that surrounded variably sized duct spaces, some of which exhibited decapitation secretion. The poroid cells were continuous with infundibulum-like structures that contained aggregates of mature sebocytes. The patient had no fam

Focal hyperhidrosis secondary to eccrine naevus successfully treated with botulinum toxin type A.

Eccrine naevus (EN) is a rare skin hamartoma included in the organoid group of epidermal naevi, histologically defined as focal hyperplasia and/or hypertrophy of eccrine glands. Clinically, EN usually presents as hyperhidrotic patches with no visible skin changes, frequently located on the forearms. The decision to treat EN or not usually depends on the grade of hyperhidrosis, but there is no therapeutic consensus because of the rarity of this condition. We present a case diagnosed as EN in an adult patient with severe localized hyperhidrosis, which was successfully treated with botulinum toxi

Successful reconstruction after resection of malignant eccrine poroma using retroauricular artery perforator-based island flap.

INTRODUCTION: Malignant eccrine poroma (MEP), or porocarcinoma, is a rare malignant tumor arising from the intraepidermal eccrine duct. It has propensity to arise on the lower limbs (44%), trunk (24%), or head and neck region (24%) but rarely occurs on the scalp. It is very difficult to find proper technique for the first time. In this paper, we introduce rare MEP developing in a longstanding eccrine poroma on the scalp (postauricular area) and successful reconstruction method using perforator-based island flap. PATIENT AND METHODS: A 52-year-old man who had an abrupt growth of a mass on the

Dermoscopic features of nonpigmented eccrine poromas in association with their histopathological features.

BACKGROUND: Nonpigmented eccrine poromas (EPs) occasionally mimic various skin tumours, but their dermoscopic features have not been clarified. OBJECTIVES: To evaluate the dermoscopic features of nonpigmented EPs in association with their histopathological features. METHODS: Retrospective analysis of the dermoscopic features of 10 histopathologically proven cases of nonpigmented EP at the Department of Dermatology, Shinshu University Hospital (Matsumoto, Japan). RESULTS: Specific features in vascular structures were observed in five of 10 nonpigmented EPs. Three cases showed a polymorphous

The anxiety disorder spectrum: fear imagery, physiological reactivity, and differential diagnosis.

This review considers recent research assessing psychophysiological reactivity to fear imagery in anxiety disorder patients. As in animal subjects, fear cues prompt in humans a state of defensive motivation in which autonomic and somatic survival reflexes are markedly enhanced. Thus, a startle stimulus presented in a fear context yields a stronger (potentiated) reflex, providing a quantitative measure of fearful arousal. This fear potentiation is further exaggerated in specific or social phobia individuals when viewing pictures or imagining the phobic object. Paradoxically, fear imagery studie

Paleodermatology.

The study of the human skin evolution, allows a new vision to ancient questions such as the significance of human skin color variation, the correlations between bipedality and loss of body hair, and the correlations among epidermal appendages and structures of the skin with the changes on environment and evolution of humans. Paleodermatology would be an appropriated term to designate the study of the human skin evolution. The etymology of this word comes from a Greek root (paleo = ancient). It is a multidisciplinary subject for researchers such as dermatologists, anthropologists, physiologists

Incomplete auriculotemporal nerve syndrome--mimicry of oral allergy syndrome.

Pollen allergies are commonly symptomatic as seasonal rhinoconjunctivitis. The majority of patients with pollen allergies develop an oral allergy syndrome due to cross-reactive homologous allergens in plant foods. Symptoms may vary from minor local oropharyngeal sensations and swelling to life threatening angioedema and glottis edema. We present the case of a 6-year-old male with suspected oral allergy syndrome who was referred for allergological work up. However, an incomplete auriculotemporal nerve syndrome was diagnosed, mimicking oral allergy syndrome.

Infiltrating apocrine adenocarcinoma with extramammary pagetoid spread.

Apocrine adenocarcinoma is a rare malignancy with invasive potential. Its clinical behavior and histologic appearance are highly variable, making accurate diagnosis difficult. We report on a case of apocrine carcinoma presenting with lymph node infiltration and extensive extramammary Paget's disease. Wide local excision with lymph node dissection is the mainstay of treatment. The role of adjuvant therapy has yet to be established.
