Cutaneous sinus of dental origin: a diagnosis requiring clinical and radiologic correlation.

Cutaneous sinuses may arise from chronic dental infections. These sinus tracts usually appear as suppurative lesions of the chin or neck. Diagnosis is made by palpating the lesion and by radiologic examination demonstrating periapical dental abscess. Treatment with dental extraction or root canal results in resorption of the inflammatory fistula. Since many patients with sinus tracts of dental origin do not have any complaints of tooth or mouth pain, the correct diagnosis may be overlooked by the unsuspecting clinician.

Scanning electron microscopic study of apical and intracanal resorption.

Apical radicular and intracanal surfaces of extracted teeth with apical lesions were examined by means of scanning electron microscopy. The distribution of apical and intracanal resorption areas varied with the presence of a granuloma or a cyst. Teeth bearing granulomas showed an apical resorption centered on the main foramina whereas the hard tissue underlying a cyst showed little or no resorption. Intracanal resorption was always marked in the apical third and more scattered in the middle and cervical third. The resorption disrupted the anatomical structures.

[Preservation of the germs of the permanent teeth in the surgical treatment of tooth-containing cysts in children].

The working scheme is offered depicting the major stages of development of teeth-containing cysts. This provided a basis for determination of indications to preservation of the follicles of permanent teeth in different degrees of resorption of the cortical plate surrounding the follicle. The technique was used to treat 25 children with positive immediate and long-term results.

Clinical evaluation of subcutaneously administered DDAVP.

1-deamino-8D-arginine vasopressin was given subcutaneously at the dosage of 0.3 micrograms/Kg. b.w. to 24 mild factor VIII deficient patients (16 mild, 2 moderate hemophiliacs and 6 patients with von Willebrand's Disease), to treat bleedings (10 episodes) or to prevent bleeding during and after dental extractions (6 extractions) and surgery (11 interventions). None of the patients who underwent surgery bled. The vasopressin analogue was effective in the early treatment of muscle hematomas and promptly stopped all mucosal hemorrhages. Most of the patients treated for "spontaneous" bleedings per

Management of Class III furcally involved abutments for fixed prosthodontic restorations.

Class III furca-involved molars provide formidable challenges, but the literature suggests that such teeth can be successfully treated. Under specific circumstances described in this article, reasonable longevity may be expected when advanced furcal involvement of abutments has been treated with (1) open-flap root debridement, (2) root separation with metal furcal replacement, (3) root resection as a conventional FPD or splint abutment, or (4) root resection as a vertical stop for a fixed cantilever prosthesis. Well-controlled maintenance therapy is essential for success.

Cardiomyopathy in patients with hereditary motor and sensory neuropathy.

Twelve affected persons in the third decade of life or later, and the 14 nearest age- and sex-matched unaffected relatives, of a large kindred with autosomal dominant hereditary motor and sensory neuropathy not linked to the Duffy blood group (HMSN type IA) were assessed for arrhythmia and cardiomyopathy. Cardiac abnormalities were no more frequent in the affected persons than in the unaffected relatives. The heart of a patient with HMSN type II who had died of complications of cardiomyopathy was found to have rheumatic disease-type myocarditis with Aschoff bodies. The results of this study pr

Treatment of localized gingival recessions with coronally displaced flaps and citric acid. An experimental study in the dog.

The present study was designed to examine the effect of citric acid conditioning on the result of healing following treatment of localized gingival recessions with coronally displaced flaps. In 3 beagle dogs, localized gingival recessions were surgically created at the buccal surfaces of canines, premolars and molars. All experimental sites were allowed to accumulate plaque for a period of 6 months. Following preparation of notches in the buccal root surfaces at the level of the gingival margin, buccal and lingual mucoperiosteal flaps were raised. The exposed portion of the root surfaces was s

[Necrotizing gingivitis in an infant with chronic benign neutrocytopenia].

A necrotizing inflammation in the mucosa of the superior maxilla is described in an infant suffering from benign chronic neutrocytopenia. A connection between primary dentition which occurred at the same time and the basic illness is assumed.

Hereditary motor sensory neuropathy type I presenting as scapuloperoneal atrophy (Davidenkow syndrome) electrophysiological and pathological studies.

A 14 year old boy with scapuloperoneal muscular atrophy, pes cavus, areflexia and distal sensory loss (Davidenkow syndrome) is described. Nerve conduction velocities were diminished. Sural nerve biopsy demonstrated a reduction in the number of myelinated fibers and early "onion-bulb" formation. These observations support the hypothesis that the scapuloperoneal amyotrophy associated with distal sensory loss may represent a variant of type I hereditary motor sensory neuropathy.

New attachment-reattachment following reconstructive periodontal surgery.

The present study was designed to evaluate if the conditions for reformation of a connective tissue attachment are less favorable on root surfaces which have lost their fibrous attachment because of periodontal disease than on root surfaces surgically deprived of their attachment apparatus. In each of 4 Green monkeys, 2 maxillary and 2 mandibular teeth were selected for experimentation. Periodontal breakdown was produced and allowed to progress to the mid-root level around one of the experimental teeth in both the maxilla and mandible by placing elastic ligatures around the neck of these teeth

Management of mandibular fractures in unreliable patient populations.

There is a high incidence of complications following treatment of fractures of the dentulous mandible at our institution, which has led us to scrutinize the criteria for maintaining teeth in the line of fracture and to evaluate other factors that may compromise healing of the fractured mandible, especially in an unreliable patient population. We have reviewed the opinions of various authors and agree that management decisions must be individualized. We recommend prompt and stable immobilization, adequate observation, use of a "tickle file" to keep track of missed appointments, and long-acting

Effects of incisor repositioning on monkey periodontium after expansion through the cortical plate.

The anterior teeth of five adult pigtail monkeys were moved lingually to correct a previously induced extreme labial displacement. (Roots had been moved through the labial bone to create bone dehiscence, loss of attachment, and gingival recession.) Eight months later, repositioning was performed with fixed appliances (Fig. 3). The teeth were retained in their more normal arch position for 5 months, after which clinical and histologic measurements were made of several periodontal parameters. The canines were not moved and served as reference teeth. Measurements were made to record changes in th

[Ballerina steps, symptom disclosing hypertrophic Charcot-Marie disease with dominant transmission. Value of electrophysiological studies].

Three children referred for electromyographic investigations presented difficulty in standing upright and in walking. They walked on the points of the feet. The disorder had appeared several years after the development of normal walking. Electrophysiological tests demonstrated a severe peripheral neuropathy with slowing of motor conduction in all four limbs and severe disturbances of sensory conduction. The purely peripheral nature of the lesion was confirmed by studying cortical somatesthetic potentials. E.M.G. and stimulus-detection examinations in family ascendants free from any clinical di
