BACKGROUND: Intraoperative temporary portocaval shunt (TPCS) has been performed during liver transplant to improve hemodynamics and renal function as well as to decrease bleeding during hepatectomy. Immunosuppression after solid-organ transplantation can lead to opportunistic infections as cryptococcosis. A 69-year-old female, cadaveric renal-transplant recipient, manifested with ulcerative lesions on the 7th month of immunosuppression. Histological examination confirmed the diagnosis of cryptococcosis. Treatment with systemic and topical antifungal therapy for 7 months concluded in significant cutaneous improvement. This case presentation encourages high clinical suspicion of opportunistic pathogens such as Cryptococcus spp. and highlights the necessity of appropriate management.

Recipients of liver transplantation (LT) remain at higher risk (adjusted for other risk factors) of de novo malignancies (DNMs). BACKGROUND: Replicative senescence is associated with telomere shortening. In native kidneys, obtained prior to transplantation, we recently described and validated a significant association between shorter intrarenal telomere length and renal arteriosclerosis. After renal transplantation, animal experiments suggested that ischaemia-reperfusion injury, acute rejection episodes and cytomegalovirus disease associate with accelerated renal allograft senescence. This paper describes a case of cardiac arrest during femtosecond laser enabled penetrating keratoplasty (FLEK) in a young, healthy patient with no known cardiac risk factors and discusses the risk of cardiac arrest during intraocular surgery.
