Causes of Blindness among Adult Yemenis: A Hospital-based Study.

PURPOSE: This hospital-based retrospective study was aimed to assess the causes of blindness among adults aged 17 years and over who attended a teaching eye hospital in Yemen. METHODS: The case notes of 3845 consecutive new patients over 12 months attending Ibn Al-Haitham Eye Center which is affiliated to the University of Science and Technology in Sana'a (the capital of Yemen) were retrieved and analysed. Data collected included age, gender, chief complaint and complete eye examination. RESULTS: 7.7 percent (296) were uniocularly blind and 11.2 percent (432) were binocularly blind (best cor

Physical exam and occult post-traumatic vascular lesions: implications for the evaluation and management of arterial injuries in modern warfare in the endovascular era.

Based on complexity of extremity wounds sustained in recent combat, arteriography had been used routinely in evaluations for delayed or occult arterial injuries. This report aims to quantitatively analyze the sensitivity and specificity of physical exam (PE) in predicting the presence of these injuries. United States service members sustaining extremity trauma in the Global War on Terrorism were evacuated to our medical center and evaluated by a senior vascular surgeon. Those with an abnormal PE, at risk based on wounding patterns, or previously treated for vascular injury underwent arteriogra

Concomitant injuries in patients with panfacial fractures.

BACKGROUND: Patients with panfacial fractures comprise a small portion of the overall facial fracture patient population. Because of the forces necessary to cause panfacial injury, these patients often have other concomitant injuries. The timing of operative facial fracture management remains controversial. METHODS: A 3-year review of all patients with facial fractures was conducted at Duke University Medical Center (2003-2005, 437 total patients). All patients with panfacial fractures, defined as fractures involving at least three of the four facial segments (frontal, upper midface, lower mi

Stability of the laser in situ keratomileusis corneal flap in rabbit eyes.

PURPOSE: To quantify the force required to dislodge a laser in situ keratomileusis (LASIK) corneal flap in rabbits and correlate that force with sources of injury to LASIK corneal flaps in humans. SETTING: Animal Vivarium and Department of Ophthalmology, Naval Medical Center, San Diego, California, USA. METHODS: Corneal flaps were cut in 56 eyes of 30 rabbits. A -8.00 diopter excimer ablation was also performed in 32 of the eyes. Injury testing, using a burst of CO2 from a modified paintball gun, was conducted from 1 to 9 days postoperatively. Impact force was compared to that generated by m

Effort thrombosis (Paget-Schroetter syndrome) in active young adults: current concepts in diagnosis and treatment.

Effort thrombosis or Paget-Schroetter syndrome most often affects young, active adults who are engaged in sports activities or whose professions require repetitive arm movements causing trauma to the axillary-subclavian vein and precipitating deep vein thrombosis. The presence of unilateral edema in the upper extremity is often thought to be attributable to trauma from an exercise regimen rather than acute deep vein thrombosis or compression of the subclavian vein by extrinsic anatomic structures. Because this syndrome occurs in young, active adults it has the potential for considerable long-t

Direct triggers for ulceration in patients with venous insufficiency.

BACKGROUND: More than 80% of leg ulcers are caused by venous insufficiency. The actual causes of venous ulcers, beyond the presence of venous insufficiency, are rarely referred to in the literature. This study evaluates the direct triggers for ulceration in patients with venous insufficiency. METHODS: Sixty-four patients with 110 chronic venous ulcers, treated in the Chronic Wounds Clinic of Soroka University Medical Center from 1999 through 2002, were included in the study. In each patient, the actual trigger of ulceration was determined by history taking and reviewing of the medical chart.

Management of destructive bowel injury in the open abdomen.

BACKGROUND: Little attention has been focused on destructive injuries of the bowel in patients requiring open abdominal management. We therefore reviewed our institutional experience for destructive bowel injury requiring open abdominal management with the vacuum pack technique (vac). METHODS: The trauma registry at a Level I trauma center was used to identify patients sustaining destructive bowel injury for an 11-year period beginning in May 1990. Patients were assessed for pertinent clinical and demographic information, and individuals requiring open abdominal management were compared with

Early versus delayed reduction and pinning of type III displaced supracondylar fractures of the humerus in children: a comparative study.

OBJECTIVES: To determine whether children with Type III displaced supracondylar fractures of the humerus that were pinned in a delayed fashion, defined as more than eight hours after fracture, had a greater need for open reduction than patients with similar fractures pinned more urgently, within eight hours after the trauma. Also, to determine whether those patients treated later fared any worse than patients treated earlier. DESIGN: Retrospective review of consecutive patients. SETTING: Level I pediatric trauma center at a tertiary children's hospital. PATIENTS: Fifty-eight consecutive pat

Snowboarding injuries in children and adolescents.

To study snowboarding injuries in children and adolescents, a 6-year retrospective study was conducted of patients 18 years old and younger admitted to a pediatric trauma center after snowboarding accidents. The study was designed to identify the most severely injured patients. Comparison was made to data from a recent comparable study of skiing injuries. Twenty-seven patients were identified, all but one male and none under 10 years old. Nineteen were injured in a fall, 6 collided with a stationary object, one collided with a skier, and in one case the mechanism of injury was unclear. The ave

Complex regional pain syndrome of the lower extremity: a retrospective study of 33 patients.

Even when diagnosed early and treated appropriately, patients with complex regional pain syndrome (CRPS), a condition that can lead to severe painful dysfunction of the limb, may continue to have long-term pain. A retrospective study was conducted of 33 patients with a positive history of CRPS I, CRPS II, or sympathetically maintained pain (SMP) of the lower limb who were treated in either a clinical setting or a pain management center. The average age of individuals diagnosed with CRPS was 43.5 +/- 12.6 (mean +/-SD) years with 60% being female. The most common diagnosis was CRPS I (75.8%) fol

Impact near the hip dominates fracture risk in elderly nursing home residents who fall.

Hip fractures among the elderly are a significant and rapidly growing public health problem. The prevailing view is that most hip fractures are the consequence of age-related bone loss or osteoporosis. However, because over 90% of hip fractures are the result of falls, we have undertaken a falls surveillance study to determine if factors related to the mechanics of falling are associated with increased risk of hip fracture. Case subjects with hip fracture and control subjects without hip fracture were sampled from falls recorded at the Hebrew Rehabilitation Center for Aged, a chronic care faci

[Admission and organization of emergency care for multiple injured patients. The experience of the Strasbourg Center of traumatology and orthopedics].

The admission of patients with severe multiple trauma and the organization of treatment must follow a few basic principles: 1) continuous, uninterrupted management by the various links of the medical chain, 2) single location: the various persons ensuring the treatment must go to the patient's side, and not the opposite, 3) pluridisciplinary, polyvalent care, and lastly, 4) single command: for us, the orthopedist-traumatologist must direct and coordinate care to the severely injured persons, and the so-called general surgeons is in charge of the others. Three structures of admission currently

Youth and Parent Perspectives on Patient-Centered Outcomes for a Pediatric Hospital-Based Violence Intervention Program.

BACKGROUND: Hospital-based violence intervention programs (HVIPs) aim to interrupt cycles of violence and support recovery among youth exposed to interpersonal violence. However, HVIP evaluations often rely on deficit-oriented outcomes (i.e., reinjury, mortality, criminal justice system involvement) that may not reflect priorities of youth and families. OBJECTIVE: To determine youth and parent perspectives on patient-centered outcomes, needs, and priorities following violent injury. METHODS: Focus groups were conducted with youth and parents connected to community-based organizations (CBOs)

Increased severity of abusive head trauma during the first year of the COVID-19 pandemic.

BACKGROUND: Abusive head trauma (AHT) is the leading cause of death from physical abuse in children. Reports regarding the impact of the COVID-19 pandemic on rates and severity of AHT are limited and with conflicting results. OBJECTIVE: To determine the number and clinical characteristics of AHT cases presenting to a pediatric tertiary care center during the first year of the COVID-19 pandemic compared to the two prior years. PARTICIPANTS AND SETTING: We performed a retrospective cohort study of patients <5 years old diagnosed with AHT at a single pediatric tertiary care center over a three-
