Risk Factors for Post-Tonsillectomy Hemorrhage: A 10-Year Retrospective Cohort Study at a Tertiary Military Hospital in Thailand.

OBJECTIVE: Tonsillectomy is one of the most frequently performed surgical procedures worldwide, and several patient-and procedure-related factors have been associated with post-tonsillectomy hemorrhage (PTH). Severe hemorrhage may require reoperation under general anesthesia to achieve hemostasis. The aim of this retrospective cohort study was to identify pre-, intra-, and postoperative factors associated with PTH. METHODS: Records of 549 patients who underwent tonsillectomy at a tertiary military hospital in Thailand between 2010 and 2020 were reviewed, with PTH ascertained by detailed manua

Ten-year Follow-up of Implant Treatment after Bilateral Lateral Approach to Sinus Elevation with Autologous Bone: A Case Report.

Maxillary sinus floor elevation is an option in cases where bone volume in the maxillary molar region is clearly insufficient to allow implant therapy. However, few studies have reported long-term observation of change in bone volume in the same case over a period of 10 or more years. This report describes a case of implant placement following maxillary sinus floor elevation using iliac bone with long-term monitoring of changes in peri-implant bone volume. The patient was a 58-year-old man who visited our clinic in April 2008 with the chief complaint of chewing problems due to missing teeth. T

Cannabinoids and General Anesthetics: Revisiting Molecular Mechanisms of Their Pharmacological Interactions.

Cannabis has been used for recreation and medical purposes for more than a millennium across the world; however, its use's consequences remain poorly understood. Although a growing number of surgical patients are regular cannabis consumers, little is known regarding the pharmacological interactions between cannabis and general anesthetics; consequently, there is not a solid consensus among anesthesiologists on the perioperative management of these patients. The existing evidence about the molecular mechanisms underlying pharmacological interactions between cannabinoids and anesthetic agents, b

Comparison of Continuous Thoracic Epidural Analgesia Versus Bilateral Erector Spinae Plane Block for Pain Management in Coronary Bypass Surgery.

Background Effective pain control is vital for patients undergoing heart surgery. Utilizing a multimodal approach to analgesia is essential, as poor pain management can result in hemodynamic and systemic complications. This study aimed to compare perioperative pain management techniques in patients undergoing coronary artery bypass grafting (CABG), specifically evaluating continuous thoracic epidural analgesia and ultrasound-guided bilateral erector spinae block. Methods This randomized comparative study was conducted at a tertiary care centre over a period of six months, with approval from th

Comparative Evaluation of Analgesic Efficacy of Ultrasound-Guided Pericapsular Nerve Group Block and Femoral Nerve Block During Positioning of Patients With Hip Fractures for Spinal Anesthesia: A Prospective, Double-Blind, Randomized Controlled Study.

INTRODUCTION: Hip fractures cause severe pain during positioning for spinal anesthesia (SA). Intravenous systemic analgesics can lead to various complications in elderly patients, hence peripheral nerve blocks are emerging as a standard of care in pain management for hip fractures, among which femoral nerve block (FNB) is widely known and practiced. Pericapsular nerve group (PENG) block is a recently described technique that blocks the articular nerves of the hip with motor-sparing effects and is used to manage positional pain in hip fractures. This study aims to evaluate the analgesic efficac

Effect of common OPRM1, COMT, SLC6A4, ABCB1, and CYP2B6 polymorphisms on perioperative analgesic and propofol demands on patients subjected to thyroidectomy surgery.

BACKGROUND: Perioperative anesthetic and/or analgesic demand present considerable variation, and part of that variation appears to be genetic in origin. Here we investigate the impact of common polymorphisms in OPRM1, COMT, SLC6A4, ABCB1, and CYP2B6 genes, on the intra-operative consumption of remifentanil and propofol, as well as the postoperative analgesic needs, in patients subjected to thyroidectomy surgery. METHODS: We conducted a prospective cohort study with 90 patients scheduled to undergo elective thyroidectomy, under total intravenous anesthesia achieved by target control infusion (
