Intravenous immunoglobulin and salicylate differentially modulate pathogenic processes leading to vascular damage in a model of Kawasaki disease.

OBJECTIVE: Kawasaki disease (KD) is a multisystem vasculitis affecting children and is characterized by immune activation in the acute stage of disease. Systemic inflammation eventually subsides, although coronary arteritis persists, resulting in aneurysm formation. KD is the leading cause of acquired heart disease among children in North America. Accepted treatment guidelines include high-dose intravenous immunoglobulin (IVIG) and aspirin in the acute phase. Although this therapy is effective, the cellular and molecular mechanisms involved are not clear. The aim of this study was to examine t…

Polyarteritis nodosa: a human temporal bone study.

PURPOSE: Polyarteritis nodosa is one of the common forms of vasculitis with multiorgan involvement. Hearing loss may be the presenting symptom of this disease. The aim of this histopathologic study was to evaluate temporal bone changes in polyarteritis nodosa and assess the correlation between otologic manifestations and histopathologic findings. MATERIALS AND METHODS: A retrospective human temporal bone analysis was performed at an otopathology laboratory in a tertiary academic medical center. Three cases (5 temporal bones) with pathologically confirmed diagnosis of polyarteritis nodosa were…

Systemic, secondary and infectious causes for cerebral vasculitis: clinical experience with 16 new European cases.

Cerebral vasculitis represents a rare form of vascular inflammatory involvement caused by heterogeneous conditions. In this study, the wide … or cerebral vasculitis included seven patients with Behçet syndrome and one case each of giant cell arteritis, Wegener's granulomatosis and Churg-Strauss syndrome, respectively. Vasculitis secondary to systemic diseases included two patients with systemic lupus erythematodes, one with sarcoidosis and one with ANA-positive systemic vasculitis. Two patients s …

Central nervous system involvement in pediatric rheumatic diseases: current concepts in treatment.

Central nervous system (CNS) manifestations are not rare in pediatric rheumatic diseases. They may be a relatively common feature of the disease, as in systemic lupus erythematosus (SLE) and Behçet's disease. Direct CNS involvement of a systemic rheumatic disease, primary CNS vasculitis, indirect involvement secondary to hypertension, hypoxia and metabolic changes, and drug associated adverse events may all result in CNS involvement. We have reviewed the CNS manifestations of SLE, Behçet's disease, Henoch-Schönlein purpura, polyarteritis nodosa, juvenile idiopathic arthritis, juvenile ankylosi…

Endomyocardial biopsy--technical aspects experience and current status. An Indian perspective.

Our study ellucidates the utility of endomyocardial biopsy (EMB) in various cardiac-muscle disorders seen in a tropical country like India. The procedure has been successfully performed in 501 patients (572 procedures) at our centre from April 1985 to December 1992. This included 60 infants and children. The indications were dilated cardiomyopathy (DCM) in 214, non-specific aortoarteritis in 91, rheumatic heart disease in 75, restrictive cardiomyopathy in 45, constrictive pericarditis in 14 and miscellaneous in 62 patients. There was no mortality, however, one patient developed cardiac tampona…

Small airway disease in rheumatoid arthritis.

Variety of pulmonary lesions are thought to be associated with rheumatoid arthritis (RA). These lesions traditionally have included pleurisy with or without effusion, Caplan's syndrome, pulmonary rheumatoid nodules, diffuse interstitial fibrosis, and pulmonary arteritis and hypertension. But little attention has been paid to the airways in RA. Recently, several reports have suggested an association between airflow limitation and RA, but its incidence is not known. Also whether there exists a parameter of disease activity of RA, suggesting the presence of small airway disease (SAD) is not clear…

Crowned Dens Syndrome Mimicking Atlantoaxial Infection in a Patient With Systemic Sclerosis: A Case Report.

OBJECTIVE: Crowned dens syndrome (CDS), caused by calcium pyrophosphate or hydroxyapatite deposition around the odontoid process, is an under-recognized cause of acute severe neck pain and headache. It can closely mimic septic arthritis, osteomyelitis, giant cell arteritis (GCA), polymyalgia rheumatica (PMR), or cervical spondylitis. We report a case of CDS in a patient with diffuse systemic sclerosis initially managed for presumed infection. METHODS: A 55-year-old man with diffuse cutaneous systemic sclerosis on mycophenolate mofetil and methotrexate presented with severe throbbing headache,…

Short-course prednisone and methotrexate in a cohort of patients with polymyalgia rheumatica: a potential therapeutic approach.

OBJECTIVES: The optimal treatment of polymyalgia rheumatica (PMR) is far from being fully elucidated. Only a few trials explored the role of conventional DMARDs: none of them adequately ruled out subclinical giant cell arteritis (GCA). The aim of this study was to assess whether the precocious administration of methotrexate (MTX) and a short course of glucocorticoids (GCs) could be safe and effective in recently diagnosed PMR. METHODS: Patients with a recent diagnosis of PMR were prospectively enrolled, upon exclusion of GCA, and treated with MTX and gradually tapered prednisone (PDN). As a co…

OBJECTIVE: To describe the experience of using olokizumab (OKZ) in patients with polymyalgia rheumatica (PMR) who have contraindications or an inadequate response to methotrexate and require steroid-sparing therapy, and to analyze treatment outcomes in this cohort. METHODS: A case series included 23 patients with PMR [including giant-cell arteritis (GCA) in 9 patients] treated with OKZ. The assessment of disease activity was carried out using the PMR-activity score (AS) index, as well as laboratory parameters [erythrocyte sedimentation rate (ESR), C-reactive protein (CRP)]. Rapid access strategies for polymyalgia rheumatica: current knowledge.

OBJECTIVES: Polymyalgia rheumatica (PMR) is frequently seen in primary care, but diagnosis remains challenging due to the absence of a definitive test and substantial clinical overlap with other conditions. Rapid access strategy (RAS) clinics may facilitate accurate diagnosis and have been widely adopted for giant cell arteritis, but their applicability to PMR has not been established. The aim of this review was to assess current evidence on RAS PMR clinic logistics to inform future clinic development. METHODS: A systematic review was conducted using PubMed and EMBASE (inception date to July …
