OBJECTIVE: To evaluate the combined effect of systemic corticosteroid and antibiotic therapy on the course of septic arthritis.
METHODS: The murine model of hematogenously acquired Staphylococcus aureus arthritis was used. Mice were treated with corticosteroids and antibiotics, and were followed up individually. Arthritis was evaluated clinically and histopathologically. Serum samples and bacterial isolates were also analyzed.

Neonatal septic arthritis has always been considered as separate from its counterpart in older children. The condition is uncommon but serious. Affected neonates usually survive, but with permanent skeletal deformities. Ten cases of neonatal septic arthritis were diagnosed between January 1989 and December 1993 in the neonatal intensive care units of two referral hospitals in the state of Kelantan, Malaysia. A retrospective study of 191 cases of septic arthritis was undertaken at Royal Darwin Hospital in the tropical north of Australia. Incidence was 9.2 per 100,000 overall and 29.1 per 100,000 in Aboriginal Australians (RR 6.6; 95% CI 5.0-8.9). Males were affected more than females (RR 1.6; 95% CI 1.2-2.1). We studied the results of prolonged intravenous therapy with antibiotics through a central venous silicone-elastomer catheter that had been peripherally inserted in thirty-five orthopaedic patients. Forty-three children with 45 hip joint infections were admitted to the hospital between 1983 and 1988. The duration of symptoms was the single most important prognostic feature. Almost all children who were treated within four days of symptoms had a satisfactory outcome. Concomitant osteomyelitis of the proximal femur produced by far a worse prognosis than if the infection was confined to the synovium of the hip joint. Septic arthritis is rare in haemophiliacs. Four new cases who were also HIV positive are reported. In three, the knee was involved, and in the fourth the elbow. The organism was streptococcus pneumoniae and staphylococcus aureus in one patient each, and salmonella in two. We retrospectively reviewed all patients who underwent bone scanning for possible osteomyelitis at the Naval Regional Medical Center (Portsmouth, VA, U.S.A.) between 1980 and 1983. Among 63 children, there were 20 sites of osteomyelitis. Streptococcus pneumoniae is the leading pathogen in children with sickle cell disease. Forty children younger than 20 years of age who had sickle cell disease and septicemia, meningitis, or osteomyelitis/septic arthritis were identified. The causes included Streptococcus pneumoniae (20%) and gram-negative organisms (mainly Salmonella) (70%). The gram-negative infections occurred in the first decade of life in 45% of our patients. We believe that this pattern of infection is different and related to the mild nature of sickle cell disease in our patients and to their persistent splenic function.

A case of gonococcal septicemia with monoarticular arthritis and adult respiratory distress syndrome is presented. Prompt treatment of the infection and early treatment for the adult respiratory distress syndrome prior to the full development of the syndrome appeared to ameliorate the course of illness to the extent that intubation and mechanical ventilation were not required. The patient was successfully traeted in a community hospital with a relatively short course of therapy and full recovery.
