The validity of psychiatric diagnoses: the case of 'specific' developmental disorders.

We tested whether developmental coordination disorder (DCD) and mixed receptive expressive language disorder (RELD) are valid diagnoses by assessing whether they are separated from each other, from other childhood disorders, and from normality by natural boundaries termed zones of rarity. Standardized measures of intelligence, language, motor skills, social cognition, and executive functioning were administered to children with DCD (n = 22), RELD (n = 30), autistic disorder (n = 30), mental retardation (n = 24), attention deficit/hyperactivity disorder (n = 53) and to a representative sample o

Preschool boys with oppositional defiant disorder: clinical presentation and diagnostic change.

OBJECTIVE: Little is known about the clinical presentation and course of oppositional defiant disorder (ODD) when first diagnosed in the preschool years. Patterns of ODD symptomatology, comorbidity, persistence of disorder, and predictors of diagnostic outcome were examined in clinic-referred preschool boys. METHOD: Boys (aged 4-5.5 years) with a DSM-III-R diagnosis of ODD were prospectively followed over a 2-year period. Multiple assessment procedures were used, including a modified version of the Diagnostic Interview Schedule for Children and parent and teacher ratings. RESULTS: Ninety-two

Psychopharmacology in autism.

Autism is a neurobiological disorder. The core clinical features of autism include impairment in social interaction, impairments in verbal and nonverbal communication, and restricted, repetitive, and stereotyped patterns of behavior, interests, and activities. Autism often has coexisting neuropsychiatric disorders, including seizure disorders, attention deficit hyperactivity disorder, affective disorders, anxiety disorder, obsessive-compulsive disorder, and Tourette disorder. No etiology-based treatment modality has been developed to cure individuals with autism. However, comprehensive interve

Psychopathology in children and adolescents with developmental disorders.

Children and adolescents with developmental disorders suffer from a wide range of psychopathology. However, there are no published studies examining this subject exclusively in this population using recent diagnostic criteria. The primary purpose of this paper is to report on the diagnosis encountered in a clinical setting using DSM-III-R. The medical records of all individuals assessed in a specialized program during a 1-year period were reviewed looking at their demographic features, diagnoses, and target behaviors. Our sample consisted of 233 subjects and contained significantly more boys t

Right body side performance decrement in congenitally dyslexic children and left body side performance decrement in congenitally hyperactive children.

OBJECTIVE: Simple and complex visuomotor performance of the right and left sides of the body was investigated in 37 children with left hemisphere lesions, 35 children with right hemisphere lesions, 53 developmentally dyslexic children, 29 developmentally hyperactive children, and 35 "normal" children who had endured a very mild head injury with no sequelae. BACKGROUND: Lateralized soft signs, EEG topography, metabolic brain imaging, and neuropsychological test profiles suggest a predominance of left hemisphere dysfunction in dyslexia and right hemisphere dysfunction in hyperactivity. METHOD:

Shifts in attentional demand are accompanied by sex-specific patterns of insula-frontal cortical interactions in mice.

The insula is a brain region that is correlated with attention in humans. Aberrant insula function is also implicated in attentional impairments in several psychiatric disorders, such as attention deficit hyperactivity disorder (ADHD), major depressive disorder (MDD), and schizophrenia. Although the insula's role in attention has been well-studied in humans, we do not yet have an understanding of how the insula communicates with other brain areas to promote attentional processing. Here, we show that neurons in the mouse insula send direct axonal projections to the frontal cortex, and that thes

Examining individual differences in reading and attentional control networks utilizing an oddball fMRI task.

The aim of the current study was to develop an fMRI task capable of characterizing individual differences in reading and attentional domains. Forty-nine students with a range of reading and attentional control abilities completed an event-related fMRI oddball task consisting of printed word and false font stimuli. Reading network activation was assessed by contrasting printed words with false font stimuli. Left inferior frontal gyrus and superior/middle temporal gyrus showed a main effect of stimulus type. The magnitude of the difference in activation between words and false font was correlate

Evaluating the construct validity of the King-Devick test in a psychological outpatient clinical sample.

The King-Devick test (K-D) has demonstrated sensitivity as a screener measure of ocular motor and cognitive problems. Despite its empirical support in the assessment of patients with certain injuries and disorders (e.g., concussion, reading disorders), less is known about the construct validity of the K-D. This study examined this topic in an outpatient, diagnostically heterogeneous clinical sample. A total of 70 individuals seen for an outpatient psychoeducational evaluation completed the K-D in addition to measures of intellectual abilities, speeded reading ability, simple and sustained atte

Behaviour profiles after tuberculous meningitis.

The purpose of the study was to investigate child behaviour in children who recovered from tuberculous meningitis (TBM) and to compare behaviour profiles of stage II and stage III patients. The mean age of the cohort of 74 children at the time of evaluation was 10 years and 7 months. At follow-up all patients underwent a thorough neurological examination and a psychometric test battery, which included intellectual assessment and evaluation of behaviour by means of the CBCL/6-18. Results indicated elevated mean scores (T > 60) on CBCL/6-18 scales which measure problems with anxiety, depression,

Perceptual, motor and attentional deficits in seven-year-old children: neurological and neurodevelopmental aspects.

This study presents the results of neurological assessments of a representative sample of seven-year-old Swedish children with perceptual, motor and attentional deficits and their controls (N = 141). Among those children diagnosed as suffering from minimal brain dysfunction (MBD), the majority showed neurodevelopmental deviations indicative of the "clumsy child syndrome'. However, 20 per cent had slight signs of choreoathetosis, diparesis, hemiparesis or ataxia. Other associated neurodevelopmental deviations are considered. The correlation between neurological findings and background factors i

Major mental disorders in offspring of mothers with polycystic ovary syndrome.

PURPOSE: Polycystic ovary syndrome (PCOS) is a prevalent endocrine disorder linked to metabolic and psychiatric comorbidities. Emerging evidence suggests maternal PCOS may also impact offspring neurodevelopment. This population-based cohort study investigates the association between maternal PCOS and the risk of major psychiatric disorders in offspring, including autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), depression, schizophrenia, and bipolar disorder. METHODS: Using data from Taiwan's National Health Insurance Research Database, 8,930 offspring of mothe

Frequency of mental health disorders in a sample of elementary school students receiving special educational services for behavioural difficulties.

OBJECTIVE: Despite being essential for defining and planning special educational services, very few data are available in Quebec regarding the nature and extent of behavioural difficulties presented by children who receive special educational services at school. This study provides a picture of the frequency of disruptive behaviour disorders (that is, attention-deficit hyperactivity disorder [ADHD], oppositional defiant disorder [ODD], and conduct disorder [CD]) and internalized disorders (including generalized anxiety disorder [GAD] and major depressive episode [MDE]) in a sample of elementar

Subtle symptoms associated with self-reported mild head injury.

We conducted a survey on the relationship between mild head injury incidence and a variety of psychological and educational symptoms in a sample of 1,345 high school and 2,321 university students. Once figures were adjusted to represent a 50:50 gender ratio, 30% to 37% of subjects reported having experienced a head injury incident, with 12% to 15% of the total group of subjects reporting such an incident with loss of consciousness. We found significant relationships between the incidence of such mild head injury and gender, sleep difficulties, social difficulties, handedness pattern, and diagn

Associations of Different Types of Maternal Diabetes and Body Mass Index With Offspring Psychiatric Disorders.

IMPORTANCE: Maternal obesity, pregestational type 1 and 2 diabetes, and gestational diabetes have been reported to increase the risk of autism spectrum disorder and attention-deficit/hyperactivity disorder in the mothers' offspring. However, the associations of maternal diabetes disorders and body mass index jointly with psychiatric disorders among offspring are less well documented, especially for type 2 diabetes. OBJECTIVE: To examine the associations of different types of maternal diabetes, separately and together with maternal obesity, with psychiatric disorders in the mothers' offspring.
